Effervescent powder for preparation of oral solution Pikoprep
Effervescent powder for preparation of oral solution Pikoprep

Pikoprep is a laxative drug.

Release form and composition

The drug is produced in the form of an effervescent powder for the preparation of a solution for oral administration, which is a white crystalline mass with a weak orange aroma. The finished solution is opaque, white or white with a yellowish tinge, with a characteristic orange flavor. The powder is packed in four-layer sachets of 16.1 g each, 2 sachets in a carton box and instructions for use of Pikoprep.

Composition for 1 sachet:

  • active ingredients: sodium picosulfate monohydrate - 10 mg; magnesium oxide - 3.5 g; anhydrous citric acid - 12 g;
  • auxiliary components: sodium saccharinate dihydrate, potassium bicarbonate, orange flavor [lactose, ascorbic acid, dry orange extract, butylhydroxyanisole (E320), xanthan gum (E415)].

Pharmacological properties

Pharmacodynamics

The active components of the drug are sodium picosulfate and magnesium citrate, which is formed from magnesium oxide and citric acid when the powder is dissolved. Sodium picosulfate exerts its properties in the colon, exerting a local laxative effect. Magnesium citrate retains water in the colon, acting as an osmotic laxative.

Due to the powerful flushing effect and stimulation of peristalsis, the intestines are cleansed. Such preparation is necessary before surgery, as well as before endoscopic or X-ray examinations.

Pharmacokinetics

The components of the drug show their activity in the large intestine and are practically not absorbed into the blood.

Indications for use

Pikoprep is used to cleanse the intestines before surgery and in preparation for colonoscopy or X-ray examination.

The drug should not be used as a regular laxative.

Pikoprep, instructions for use: method and dosage

An effervescent solution prepared from Pikoprep powder is taken orally.

The day before the procedure, it is necessary to reduce the number of meals. In order to prevent dehydration during the period of use of the drug, it is recommended to drink a sufficient amount of liquid.

To prepare the solution, the contents of one sachet are poured into 150 ml of water, stirred for 2-3 minutes, cooled to a comfortable temperature and drunk.

Pikoprep can be prescribed only to children and adolescents over 9 years old, and adult patients.

If the operation or study is scheduled for the first half of the day, then the contents of the first sachet from the package must be taken in the afternoon or early evening (at 16-18 hours), drinking the drug with at least 1250 ml (5 glasses of 250 ml) of clear liquid or water (you can use plain water, coffee, tea, non-carbonated soft drinks or fruit juice without pulp). The second sachet is taken in the interval from 22 to 24 hours, drinking at least 750 ml (3 glasses of 250 ml) of clear liquid or water. It is recommended to drink the last glass no later than 1 hour before the planned procedure.

If the operation or study is scheduled for the second half of the day, then the contents of the first sachet from the package must be taken in the evening (at 17-21 h) on the day preceding the procedure, drinking Pikoprep at least 1250 ml (5 glasses of 250 ml) of clear liquid or water … The second sachet is taken in the morning on the day of the study or surgery (5-9 hours before the procedure) with at least 750 ml (3 glasses of 250 ml) of clear water or liquid. The last glass is drunk no later than 1 hour before the planned procedure.

Side effects

  • digestive system: often - nausea, rectal pain; rarely - abdominal pain, vomiting, loose stools; in isolated cases - transient aphtoid ileal ulceration;
  • metabolism: rarely - hypokalemia, hyponatremia;
  • nervous system: often - headache; rarely - confusion, epilepsy, seizures, a large seizure (without hyponatremia);
  • skin and subcutaneous fat: rarely - pruritus, rash (including maculopapular and erythematous), purpura, urticaria;
  • immune system: rarely - hypersensitivity reactions, anaphylactic reactions.

Overdose

An overdose of Pikoprep can lead to profuse diarrhea and further dehydration. It is necessary to take measures to restore the water-electrolyte balance.

special instructions

Pikoprep is forbidden to be used as a laxative on a regular basis (not as a preparation for surgery or research).

With signs of hyponatremia, it is urgent to take measures aimed at restoring the water and electrolyte balance.

The preparation contains potassium bicarbonate, which should be taken into account when prescribing it to patients who follow a low-calorie diet.

Orange flavor contains lactose, so Pikoprep is contraindicated in people with lactase deficiency, lactose intolerance and glucose / galactose malabsorption syndrome.

With a significant decrease in renal function, an increase in plasma concentration of magnesium is possible. In this case, another laxative must be used.

In the case of insufficient consumption of electrolytes and water, the development of a clinically significant deficiency of these components (especially in debilitated patients) is possible.

The total duration of preparation for bowel cleansing should be no more than 24 hours, since with the lengthening of this period, the risk of water-electrolyte disturbances increases.

Other powdered laxatives reduce the effectiveness of Pikoprep.

Influence on the ability to drive vehicles and complex mechanisms

Since the drug can cause undesirable effects on the part of the nervous system, during the period of its use, care must be taken when driving vehicles and doing other work that requires a high concentration of attention and a quick reaction.

Application during pregnancy and lactation

Special studies on the use of Pikoprep during pregnancy have not been carried out, but since sodium picosulfate stimulates intestinal peristalsis, it should not be prescribed to pregnant women in the first trimester. In the second and third trimesters, the use of the drug is possible only after assessing the expected benefits and possible risks.

The laxative components are not secreted in breast milk. Despite this, during lactation, the drug Pikoprep is used with caution.

Pediatric use

Pikoprep is contraindicated in children under 9 years of age.

With impaired renal function

A laxative should not be administered to patients with severe renal impairment.

For mild or moderate renal impairment, Pikoprep is used with caution.

Drug interactions

Pikoprep can affect the degree of absorption of systemically taken oral medications (antibiotics, hypoglycemic and antiepileptic drugs, oral contraceptives). Cases of epileptic seizures have been reported in patients taking a laxative against the background of epilepsy, compensated by earlier intake of antiepileptic drugs. In such patients, Pikoprep is used with extreme caution.

Diuretics, cardiac glycosides, and corticosteroids taken with laxatives increase the risk of hypokalemia.

The likelihood of imbalance in electrolytes and water increases with simultaneous use with non-steroidal anti-inflammatory drugs and drugs that can cause cerebral salt-wasting syndrome and / or Parkhon's syndrome (syndrome of inappropriate antidiuretic hormone secretion). These drugs include antipsychotics, selective serotonin reuptake inhibitors, carbamazepine, tricyclic antidepressants, etc.

Analogs

Pikoprep's analogues are Bisacodyl, Sena, Castor oil, Senadexin, Senna extract, Guttalax, Laxigal-teva, Regulax picosulfate, Slabilen, Slabikap, etc.

Terms and conditions of storage

Keep out of reach of children at a temperature not exceeding 25 ° C.

The shelf life is 3 years.

The drug must be used immediately after opening. Dispose of any opened but unused powder bag.

Terms of dispensing from pharmacies

Reviews about Pikoprep

According to reviews, Pikoprep is a good bowel cleansing agent in order to prepare for colonoscopy, operations and other studies. The drug acts mildly, does not cause intestinal spasms. The solution tastes good, its volume is small, which is also very convenient.

Some users complain of laxative side effects (severe weakness, dizziness and nausea). Also, patients note that Pikoprep is difficult to find on sale (it is not present in all pharmacies), and its cost is quite high.
