Release form and composition
- film-coated tablets: round, biconvex, from yellow to orange; the cross section shows two layers - a film shell and an orange core, possibly the presence of inclusions of white and orange colors of different shades (6, 7, 10, 14, 20, 25 or 30 pcs. in blister strip packaging, in a cardboard box 1, 2, 3, 4, 5 or 10 packs; 10, 20, 30, 40, 50, 100 or 500 pieces each in PET / polypropylene cans, in a cardboard box 1 can);
- capsules: size No. 1, hard gelatinous, orange; content - granulate containing orange powder and granules, possibly the presence of orange blotches of different shades (6 or 10 pcs. in blisters, in a cardboard box 1 package).
Composition of one tablet:
- active substance: tilorone dihydrochloride (tilorone) - 60 or 125 mg;
- auxiliary components (in tablets 60/125 mg): lactose monohydrate (milk sugar) - 84.4 / 103 mg, microcrystalline cellulose - 28.5 / 45 mg, croscarmellose sodium - 7.6 / 12 mg, povidone-K25 - 7, 6/12 mg, magnesium stearate - 1.9 / 3 mg;
- the composition of the film shell (in tablets 60/125 mg): macrogol-4000 - 0.84 / 1.4 mg, titanium dioxide - 1.68 / 2.8 mg, hypromellose - 3.42 / 5.7 mg, tropeolin dye -0 - 0.06 / 0.1 mg.
Composition of one capsule:
- active substance: tilorone - 125 mg (in terms of 100% substance);
- auxiliary components: calcium stearate, copovidone (Plasdon S-630), microcrystalline cellulose (Vivapur type 102), colloidal silicon dioxide (Aerosil);
- composition of the capsule shell: gelatin, titanium dioxide (E171), sunset yellow dye (E110).
Pharmacological properties
Pharmacodynamics
Tilorone has an immunomodulatory and antiviral effect. It is a low molecular weight synthetic inducer of interferon. Stimulates the formation of beta, gamma, alpha, lambda interferons in the body. After administration of the drug, interferon is produced mainly by intestinal epithelial cells, T-lymphocytes, granulocytes, hepatocytes and neutrophils. After oral administration of tilorone, the maximum production of interferon is determined in the following sequence: intestines - liver - blood (after 4-24 hours).
After a single oral intake of the maximum daily dose of tilorone for humans, the maximum concentration in the lung tissue of interferon lambda is determined after 24 hours, interferon alpha - after 48 hours.
Interferon lambda in the lung tissue increases the antiviral protection of the respiratory tract against influenza and other respiratory viral infections.
Tilorone induces the synthesis of interferon in human leukocytes, stimulates bone marrow stem cells. Depending on the dose, the drug is able to enhance the formation of antibodies, reduce the degree of immunosuppression, and restore the ratio of T-suppressors and T-helpers.
The mechanism of the antiviral action of tilorone is due to inhibition of the translation of virus-specific proteins in infected cells, as a result of which the reproduction of viruses is suppressed.
The drug is effective for various viral infections, including herpes viruses, hepatitis viruses, influenza viruses and other acute respiratory viral infections (ARVI).
Pharmacokinetics
After entering the gastrointestinal tract, tilorone is rapidly absorbed. Its bioavailability is about 60%. 80% binds to plasma proteins.
The half-life is 48 hours. It is excreted mainly unchanged: through the intestines - 70%, by the kidneys - 9%.
Tilorone does not undergo biotransformation. Does not accumulate in the body.
Indications for use
In adults:
- flu and other acute respiratory viral infections (treatment and prevention);
- viral hepatitis A, B and C;
- cytomegalovirus infection;
- herpes infection;
- pulmonary tuberculosis (in combination therapy);
- urogenital and respiratory chlamydia (in combination therapy);
- viral and infectious-allergic encephalomyelitis, including leukoencephalitis, uveoencephalitis and multiple sclerosis (in combination therapy).
In children from 7 years old, Tiloron is used to treat ARVI, including influenza.
Contraindications
- hereditary lactose intolerance, lactase deficiency, glucose-galactose malabsorption;
- pregnancy;
- lactation period;
- children under 7 years of age;
- the presence of hypersensitivity to any component of the drug.
According to the instructions, Tiloron must be used under the close supervision of a physician as part of the combined treatment of viral and infectious-allergic encephalomyelitis.
Instructions for use of Tilorone: method and dosage
Tilorone is indicated for oral administration. The tablets must be taken after meals.
Recommended dosing regimens for adults, depending on the indication:
- influenza and other acute respiratory viral infections: treatment - 125 mg / day in the first two days, then - 125 mg at intervals of 48 hours, total course dose - 750 mg; prevention - 125 mg once a week for 6 weeks, the total course dose is 750 mg;
- viral hepatitis A: 125 mg twice a day on the first day, then 125 mg at intervals of 48 hours, the total course dose is 1250 mg;
- acute viral hepatitis B: 125 mg / day for the first two days, then 125 mg at intervals of 48 hours, the total course dose is 2000 mg. In the case of a protracted course of the disease - 125 mg twice a day on the first day, then 125 mg at intervals of 48 hours, the total course dose is 2500 mg;
- chronic viral hepatitis B: the initial phase of treatment (total dose - 2500 mg) - 125 mg twice a day in the first two days, then - 125 mg at intervals of 48 hours; continuation phase (total dose - 1250–2500 mg) - 125 mg once a week. The course dose of the drug can vary within 3750–5000 mg, the duration of treatment is 3.5–6 months, which depends on the results of morphological, immunological and biochemical studies of process activity markers;
- acute hepatitis C: 125 mg / day in the first two days, then - 125 mg at intervals of 48 hours, the total course dose is 2500 mg;
- chronic hepatitis C: the initial phase of treatment (total dose - 2500 mg) - 125 mg twice a day in the first two days, then - 125 mg at intervals of 48 hours; continuation phase (total dose - 2500 mg) - 125 mg once a week. The course dose of the drug is 5000 mg, the duration of treatment is approximately 6 months and depends on the results of morphological, immunological and biochemical studies reflecting the degree of activity of the process;
- cytomegalovirus, herpes infection: 125 mg / day in the first two days, then - 125 mg at intervals of 48 hours, the total course dose is 1250-2500 mg;
- pulmonary tuberculosis: 250 mg / day in the first two days, then 125 mg at intervals of 48 hours, the total course dose is 2500 mg;
- urogenital and respiratory chlamydia: 125 mg / day in the first two days, then 125 mg at intervals of 48 hours, the total course dose is 1250 mg;
- viral and infectious-allergic encephalomyelitis: 125–250 mg / day in the first two days, then - 125 mg at intervals of 48 hours. The dose is set individually, the duration of therapy is 3-4 weeks.
Doses for children from 7 years old:
- uncomplicated forms of influenza and other acute respiratory viral infections: 60 mg once a day on the first, second and fourth days of treatment. The total course dose is 180 mg;
- the occurrence of complications of influenza or other acute respiratory viral infections: 60 mg once a day on the first, second, fourth and sixth days of treatment. The total course dose is 240 mg.
Side effects
Tilorone can cause short-term chills, dyspeptic disorders and the development of allergic reactions.
Overdose
To date, no cases of overdose are known.

There was no negative effect of Tilorone on the cognitive and psychomotor functions of a person.
Application during pregnancy and lactation
Tilorone is contraindicated for pregnant and breastfeeding women.
Breastfeeding should be discontinued if the need for treatment during lactation is justified.
Pediatric use
Tiloron is used in children aged 7 years and older to treat influenza and ARVI.
