Ursosan is a hepatoprotective drug with hypocholesterolemic, cholelitholytic, hypolipidemic, choleretic, immunomodulatory effects.
Release form and composition
Dosage forms:
- capsules: size No. 0, white, gelatinous, with an opaque solid structure; the contents of the capsules are almost white or white powder, or almost white or white powder with pieces of mass, or almost white or white powder compressed in a column, disintegrating upon compression (10 pcs. in a blister, in a cardboard box 1, 5 or 10 blisters);
- film-coated tablets: almost white or white, oblong biconvex, notched on one side, and deep dividing risk on the other; the core of the tablet is white or almost white (10 pcs. in a blister strip, in a cardboard box of 1, 5 or 10 packs).
1 capsule contains:
- active substance: ursodeoxycholic acid - 250 mg;
- auxiliary components: magnesium stearate, pregelatinized corn starch, colloidal silicon dioxide, corn starch;
- capsule shell: titanium dioxide, gelatin.
1 tablet contains:
- active substance: ursodeoxycholic acid - 500 mg;
- auxiliary components: magnesium stearate, pregelatinized corn starch, sodium carboxymethyl starch (type A), corn starch, colloidal silicon dioxide;
- shell composition: Opadry white 03B28796 (macrogol 400, titanium dioxide, hypromellose).
Indications for use
- uncomplicated form of gallstone disease: treatment of biliary sludge, dissolution of cholesterol gallstones in the gallbladder, which cannot be removed surgically or endoscopically;
- prevention of recurrence of stone formation after removal of the gallbladder;
- cholestatic liver pathologies of various etiologies, including primary sclerosing cholangitis, cystic fibrosis (cystic fibrosis), in the absence of signs of decompensation - primary biliary cirrhosis;
- chronic hepatitis of various etiologies, including toxic, medicinal form of liver damage;
- non-alcoholic fatty liver disease, including non-alcoholic steatohepatitis;
- alcoholic liver disease;
- biliary reflux esophagitis and reflux gastritis;
- dyskinesia of the biliary tract;
- chronic viral hepatitis.
Contraindications
- high calcium gallstones (X-ray positive);
- non-functioning gallbladder;
- liver cirrhosis in the stage of decompensation;
- acute inflammatory diseases of the gallbladder, bile ducts, intestines;
- severe dysfunction of the liver and pancreas;
- breast-feeding;
- severe renal dysfunction;
- hypersensitivity to the components of the drug.
It is not recommended to prescribe Ursosan to children under the age of 3 years, although the use of the drug has no age restrictions.
During pregnancy, the use of the drug is possible provided that the expected benefit from therapy for the mother outweighs the potential threat to the fetus.
Method of administration and dosage
Capsules and tablets are taken orally, swallowed whole and washed down with the required amount of water.
When breaking the tablet into two parts according to the dividing risk, you can take only the whole half, if the tablet is crushed into small pieces, it cannot be taken.
Recommended dosage:
- gallstone disease (biliary sludge and cholesterol gallstones): at the rate of 10–15 mg per 1 kg of the patient's body weight per day, the dose received should be taken once before bedtime. The drug is taken regularly for 6-24 months. In the absence of a therapeutic effect after 12 months of treatment, the drug should be discontinued. After complete dissolution of stones, in order to prevent recurrence of stone formation, the use of the drug should be continued for another 3 months;
- prevention of recurrent cholelithiasis after cholecystectomy: 250 mg 2 times a day for 2-3 months;
- biliary reflux gastritis and reflux esophagitis: before bedtime - 250 mg per day, the course of treatment - from 0.5 to 6 months, to achieve the desired therapeutic effect, the course can be continued up to 24 months;
- chronic hepatitis of various etiologies, chronic viral hepatitis, non-alcoholic fatty liver disease (including non-alcoholic steatohepatitis), alcoholic liver disease: 10-15 mg per 1 kg of body weight, divided into 2-3 doses. Duration of treatment is 6–12 months and longer;
- cholestatic liver diseases of various etiologies, including primary biliary cirrhosis (without signs of decompensation), cystic fibrosis, primary sclerosing cholangitis: 12-15 mg per 1 kg of body weight per day (it is possible to increase the daily dose to 20-30 mg per 1 kg of body weight), divided into 2-3 receptions. If the biochemical parameters of the blood improve after three months of therapy, the patient is transferred to a single daily dose before bedtime. The course of therapy is from 6 to 24 months or more;
- biliary dyskinesia: 10 mg per 1 kg of body weight per day, divided into 2 doses. The course of treatment is 0.5–2 months; it is recommended to continue taking the drug in the absence of the desired clinical effect.
Side effects
- from the gastrointestinal tract: often - pasty stools or diarrhea; very rarely - intense pain in the upper abdomen in the treatment of primary biliary cirrhosis;
- on the part of the hepatobiliary system: very rarely - calcification of gallstones, decompensation of liver cirrhosis in patients in the late stages of primary biliary cirrhosis (after discontinuation of the drug it stops);
- dermatological reactions: very rarely - urticaria.
special instructions
In gallstone disease, the use of the drug is indicated only with preserved patency of the cystic and common duct of the functioning gallbladder. Gallstones in composition should be cholesterol (X-ray negative), be up to 15–20 mm in size, and their number should not exceed half of the gallbladder volume.
Long-term use of the drug for dissolving gallstones should be accompanied by regular biochemical blood tests to control the activity of transaminases and other microsomal liver enzymes, monthly during the first three months, then once every 3 months.
The effectiveness of treatment is assessed by the indications of an ultrasound examination of the gallbladder and biliary tract, which is carried out 1 time in 6 months.
Before using Ursosan, women of reproductive age should exclude the presence of pregnancy; during the period of treatment, it is necessary to use reliable contraceptives.
Drug interactions
With the simultaneous use of Ursosan:
- ion-exchange resins (colestipol, cholestyramine), antacids containing aluminum can reduce the absorption of ursodeoxycholic acid, so the interval between taking these drugs and the drug should be 2 hours or more;
- estrogens, progestogenic oral contraceptives, clofibrate, neomycin can increase cholelithiasis or have the opposite effect of the drug;
- cyclosporine increases its absorption from the intestines, therefore, monitoring of its concentration in blood plasma and correction of the dosage regimen if necessary;
- ciprofloxacin may decrease its absorption.
