Tiaprid is a neuroleptic (antipsychotic) with hypnotic, analgesic and sedative effects.

Release form and composition

  • tablets: flat-cylindrical, white or white color with a slight yellowish tinge (in blister packs of 10 pcs. or in dark glass jars of 30 pcs., in a cardboard box 3 contour packs or 1 glass jar);
  • solution for intramuscular and intravenous administration: transparent, colorless or slightly colored (in ampoules of 2 ml, in a cardboard box 10 ampoules or 5 ampoules in a blister strip, in a cardboard box 2 packs. The kit includes an ampoule knife or scarifier).

Each pack also contains instructions for the use of Tiaprid.

The active substance is tiapride (in the form of hydrochloride):

  • 1 tablet - 100 mg;
  • 1 ml of solution - 50 mg.

Inactive components:

  • tablets: povidone (low molecular weight medical PVP), potato starch, magnesium stearate, lactose monohydrate (milk sugar);
  • solution: sodium chloride, water for injection.

Pharmacological properties

Pharmacodynamics

Tiapride is an antipsychotic agent (neuroleptic) with sedative, hypnotic and analgesic effects.

The drug is characterized by a pronounced analgesic effect, both for interoceptive and exteroceptive pain. Eliminates dyskinesias of central origin.

The antipsychotic effect of tiapride is due to its ability to block the dopamine B2 receptors of the mesocortical and mesolimbic systems.

The sedative effect of the drug is explained by the blockade of adrenergic receptors of the reticular formation of the brain stem.

The hypothermic action develops as a result of blockade of dopamine receptors of the hypotolamus, antiemetic action - blockade of the dopamine D2-penenTopoB trigger zone of the vomiting center.

Pharmacokinetics

Tiapride is characterized by rapid absorption (however, it is somewhat slower in elderly patients). Absorption is reduced if the drug is taken orally after a meal.

Bioavailability is 75%. In the case of taking tablets immediately before meals, bioavailability increases by 20%, the maximum plasma concentration - by 40%.

After oral administration of a dose of 200 mg, the maximum concentration of the drug in the blood plasma (Cmax) is 1.3 μg / ml and is reached within 1 hour. After intramuscular administration of a dose of 200 mg, Cmax is 2.5 μg / ml and is reached within 0.5 h.

Plasma protein binding is low. Within an hour after application, tiapride is distributed to tissues and organs, penetrates the blood-brain and placental barriers, as well as into breast milk (in milk, the concentration is 50% of plasma). Does not cumulate.

It is slightly metabolized in the liver. Half-life: in women - 2.9 hours, in men - 3.6 hours. Renal clearance is 330 ml / minute. About 70% of the dose received is excreted by the kidneys in the urine unchanged. In chronic renal failure, the elimination of the drug depends on the amount of creatinine clearance.

Indications for use

Pills

Adults:

  • intense chronic pain syndrome;
  • psychomotor agitation and aggressive states, especially in the elderly and patients with chronic alcoholism.

Adults and children over 6 years old:

  • Gilles de la Tourette's syndrome;
  • different types of chorea.

Children over 6 years old are also prescribed for behavioral disorders accompanied by agitation and aggressiveness.

Solution for intravenous and intramuscular administration

  • headache of neurogenic origin;
  • chronic pain syndrome, including spastic pain in the limbs, arthralgia, senile motor instability, pain of neurotic origin, pain in cancer patients;
  • chorea (both acute and chronic);
  • psychomotor agitation;
  • nervous tic;
  • tremor;
  • dyskinetic and hyperkinetic syndromes, including with vascular parkinsonism, including tardive dyskinesias, as well as with the use of antipsychotics;
  • violation of behavior (alcoholism);
  • disorders of conduct in old age;
  • withdrawal symptoms (alcoholism, drug addiction);
  • alcoholic psychosis.

Contraindications

For both dosage forms:

  • hypertensive crisis;
  • pheochromocytoma or suspicion of its presence;
  • severe renal and / or hepatic impairment;
  • pregnancy, especially the first trimester;
  • lactation period;
  • individual sensitivity to any component of the drug.

For Tiaprid tablets additionally:

  • children under 6 years old;
  • prolactin-dependent tumor (for example, breast cancer or pituitary prolactinoma) or suspicion of its presence;
  • simultaneous use of sultopride;
  • combination with drugs that can cause cardiac arrhythmias, such as mizolastine, diphemanil, bepridil, erythromycin, levodopa, moxifloxacin, cisapride, sparfloxacin, vincamine for intravenous administration, some antipsychotics (pimozide, levomepoperapromazine, halipromulperidazine, triflulupridine, thioridazine, droperidol, cyamemazine), class Ia antiarrhythmic drugs (disopyramide, quinidine, hydroquinidine) and class III (sotalol, dofetilide, ibutilide, amiodarone);
  • simultaneous use of dopaminergic agonists (bromocriptine, lisuride, piribedil, cabergoline, pramipexole, entacapone, amantadine, quinagolide, pergolide, ropinirole, apomorphine), excluding patients with Parkinson's disease.

For Tiaprid solution, an additional contraindication is children under 7 years of age.

Both tablets and solution should be used with caution in the following cases:

  • Parkinson's disease;
  • epilepsy;
  • decompensated cardiovascular diseases;
  • chronic renal failure;
  • elderly age.

In addition, Tiaprid tablets should be used with caution in the following cases:

  • congenital lengthening of the QT interval;
  • bradycardia (less often 55 beats / minute);
  • hypokalemia;
  • the use of drugs that can cause hypokalemia, decreased cardiac conduction, severe bradycardia and prolongation of the QT interval;
  • the simultaneous use of medicines containing alcohol.

Tiaprid, instructions for use: method and dosage

Pills

Tiapride is indicated for oral administration.

Recommended doses:

  • intense chronic pain syndrome: 200-400 mg per day;
  • psychomotor agitation, aggressive states: adults - 200-300 mg per day for 1-2 months, the elderly - 50 mg 2 times a day. If necessary, the dose is increased by 50-100 mg every 2-3 days. The maximum permissible daily dose is 300 mg;
  • Gilles de la Tourette's syndrome and chorea: the initial daily dose for adults is 25 mg, then it is gradually increased to the minimum effective, depending on the clinical situation. The average therapeutic dose can vary from 300 to 800 mg per day. Doses for impaired renal function: creatinine clearance 11–20 ml / minute - ½ the standard dose, creatinine clearance 10 ml / minute - ¼ the standard dose. Children over 6 years of age are prescribed 3–6 mg / kg per day;
  • behavioral disorders with agitation and aggressiveness in children from 6 years of age: 100–150 mg per day.

Tiaprid is always recommended to be used in the lowest effective dose. If the patient's condition allows, the initial dose should be low, then it is gradually increased.

Solution for intravenous and intramuscular administration

Tiapride is administered intramuscularly or intravenously.

Recommended doses for adults:

  • intractable pain: 200-400 mg per day;
  • pre-relirious and delirious state: 400–1200 mg per day in portions every 4–6 hours. The maximum allowable daily dose is 1800 mg.

Doses for impaired renal function depending on creatinine clearance:

  • 11–20 ml / minute - ½ standard dose;
  • <10 ml / minute - ¼ standard dose.

Children over 7 years of age are prescribed 100-150 mg / kg per day. If necessary, it is possible to increase the daily dose, but not more than up to 300 mg.

Side effects

Pills

  • weight gain, chest pain, postural hypotension, atrial fibrillation, prolongation of the QT interval;
  • hyperprolactinemia and related disorders: swelling of the mammary glands, amenorrhea, impotence or impaired ejaculation, galactorrhea;
  • drowsiness / insomnia, apathy, agitation, fatigue, asthenia, headache, psychomotor agitation, increased blood pressure, dizziness, Parkinson's syndrome - tremor, hypersalivation, bradykinesia;
  • akathisia, early dyskinesia and dystonia, including oculogyric crises, spastic torticollis and trismus (these side effects are usually reversible after discontinuation of therapy or the use of anticholinergic antiparkinsonian drugs);
  • tardive dyskinesia (stereotypical involuntary movements of the limbs, face and tongue), which occurs during long-term treatment with Tiaprid;
  • neuroleptic malignant syndrome.

Solution for intravenous and intramuscular administration

  • extrapyramidal symptoms, decreased muscle tone, increased fatigue, agitation, drowsiness, neuroleptic malignant syndrome;
  • early and late dyskinesias;
  • hyperstimulation phenomena (exacerbation of hallucinatory-delusional symptoms or increased anxiety in patients with schizophrenia);
  • orthostatic hypotension;
  • weight gain, amenorrhea, galactorrhea, decreased potency, frigidity;
  • gynecomastia, hyperprolactinemia;
  • skin allergic reactions;
  • hyperthermia.

Overdose

Overdose symptoms: drowsiness, excessive sedation, depression of consciousness (up to coma), extrapyramidal disorders, arterial hypotension.

If signs of an overdose develop, Tiaprid should be canceled, gastric lavage should be performed (if the drug was used in the form of tablets), and detoxification and symptomatic therapy should be carried out. It is necessary to monitor vital body functions, especially cardiac activity (due to the risk of lengthening the QT interval). When severe extrapyramidal disorders appear, anticholinergic therapy is prescribed. Tiapride is poorly dialyzed. The specific antidote has not been established.

If hypokalemia is present, it should be corrected prior to the appointment of Tiaprid. During treatment, it is necessary to monitor the clinical picture of the disease, electrolyte balance and electrocardiogram.

If extrapyramidal syndrome develops during treatment, anticholinergics should be prescribed instead of dopaminergic agonists.

During the period of treatment, you should refrain from drinking alcohol.

Influence on the ability to drive vehicles and complex mechanisms

While taking Tiaprid, care must be taken when engaging in potentially hazardous activities, including when working with mechanisms and driving a car.

Application during pregnancy and lactation

Tiaprid is strictly contraindicated for use in the first trimester of pregnancy. The effect of antipsychotics on the course of pregnancy and the fetal brain has not been established, therefore it is not recommended to prescribe the drug also in the II-III trimesters. If the treatment is clinically justified, it is necessary, if possible, to use the antipsychotic in the minimum dose and limit the duration of treatment. With prolonged use and / or use of high doses of the drug and / or in late pregnancy, careful monitoring of the neurological functions of the newborn is required.

It is not known whether tiapride penetrates into mother's milk. In this regard, it is recommended to stop breastfeeding during treatment.

Pediatric use

Tiaprid tablets are contraindicated in children under 6 years old, injection solution - up to 7 years old.

With impaired renal function

In chronic renal failure, Tiapride should be used with caution, in severe renal failure, it is contraindicated.

For violations of liver function

Severe hepatic impairment is a contraindication to the appointment of Tiaprid.

Use in the elderly

In old age, Tiaprid should be used with caution.

Drug interactions

The solution for intravenous and intramuscular administration is incompatible with ethanol and levodopa. Enhances the action of angiotensin-converting enzyme inhibitors (ACE) and other antihypertensive drugs, as well as drugs that depress the central nervous system (benzodiazepines, barbiturates, narcotic analgesics, antihistamines).

When using Tiaprid in tablets simultaneously with other drugs, the following interaction reactions may occur:

  • ethanol: the sedative effect of tiapride is enhanced;
  • dopaminergic agonists: psychotic disorders may develop or worsen;
  • levodopa: mutual antagonism is noted (patients with Parkinson's disease should use the minimum effective dose of both drugs);
  • drugs that depress the central nervous system: the depressing effect is enhanced;
  • antihypertensive drugs: the likelihood of developing postural hypotension increases;
  • antiarrhythmics, drugs that cause hypokalemia and bradycardia: atrial fibrillation and ventricular arrhythmia may develop.
