Dioctahedral smectite is an antidiarrheal agent.
Release form and composition
The drug is produced in the form of a powder for preparation of a suspension for oral administration (with orange, vanilla or caramel flavor): from pale yellow / grayish white to grayish white / brownish white or brownish gray with a yellow tint, with a weak odor nonspecific to a low orange, vanilla or low caramel odor (depending on the filling) (3.76 g each in a sachet made of a combined multilayer material, in a cardboard box 1, 3, 5, 10, 15, 20, 30 or 250 sachets and instructions for use of dioctahedral Smectite; it is possible to apply the text of the instruction on the secondary packaging if it contains 30 sachets).
Contents of 1 sachet includes:
- active substance: dioctahedral smectite - 3 g;
- additional components: sodium saccharinate / sodium saccharinate dihydrate, dextrose monohydrate, vanillin / orange flavor / caramel flavor.
Pharmacological properties
Pharmacodynamics
The drug is of natural origin and has an adsorbing effect. The drug regulates the barrier function of the gastrointestinal tract (GIT) mucosa, creates polyvalent bonds with the glycoproteins of the cavity mucus, increases its amount, enhances the cytoprotective effect (against the negative effects of bile salts, hydrochloric acid, microorganisms and their toxins). Dioctahedral smectite is characterized by selective sorption properties, which are due to its discoid crystal structure; it absorbs viruses, bacteria and toxins localized in the lumen of the gastrointestinal tract.
When used in therapeutic doses, the active substance does not affect intestinal motility. It is X-ray transparent, does not stain the stool.
Pharmacokinetics
Dioctahedral smectite is not absorbed in the gastrointestinal tract, including in the presence of diseases of the latter, accompanied by manifestations of colitis and colonopathy. It is excreted unchanged.
Indications for use
- chronic and acute diarrhea of drug and allergic genesis, as well as against the background of violations of the qualitative composition of food and diet; diarrhea of infectious genesis (as part of complex treatment);
- heartburn, bloating, discomfort, feeling of heaviness in the abdomen, other symptoms of dyspepsia caused by diseases of the gastrointestinal tract.
Contraindications
Absolute:
- intestinal obstruction;
- glucose-galactose malabsorption, fructose intolerance, sucrase / isomaltase deficiency (depending on the manufacturer);
- hypersensitivity to any constituent of Smectite dioctahedral powder.
Relative contraindications for which the sorbent should be taken with caution are severe chronic constipation and diabetes mellitus (due to the dextrose present in the composition).

A suspension prepared from dioctahedral Smectite powder is taken orally.
In case of esophagitis, the drug should be taken after meals, for other indications - between meals. A single dose for children and adults is the contents of 1 sachet.
Recommended frequency of admission for the treatment of acute diarrhea:
- adults and adolescents over 12 years old: 6 times a day;
- children 1–12 years old: 4 times a day for 3 days, then 2 times a day;
- infants under 1 year old: 2 times a day for 3 days, then 1 time a day.
Frequency rate of reception of dioctahedral Smectite for other indications:
- adults and adolescents over 12 years old: 3 times a day:
- children 2-12 years old: 2-3 times a day;
- children 1–2 years old: 1–2 times a day;
- infants under 1 year: once a day.
The recommended duration of treatment is 3–7 days.
Adults and adolescents over 12 years old need to dissolve the contents of 1 sachet in 100 ml of water, gradually adding the powder and stirring gently. For children under 12 years old, the contents of 1 sachet should be dissolved in 50 ml of liquid or mixed with any semi-liquid product, such as puree, porridge, baby food. If necessary, you can distribute the daily dose over several doses throughout the day.
Unused prepared suspension should be placed in a closed container and stored in the refrigerator for no more than 16 hours at a temperature of 2-8 ° C. Shake it vigorously before each use.
Side effects
In the course of clinical studies of the drug, the following adverse events were recorded in both children and adults:
- often: constipation; usually eliminated by dose reduction, but rarely required discontinuation of treatment;
- infrequently: vomiting, flatulence.
These adverse reactions were generally minor and transient.
In the process of post-registration use of dioctahedral Smectite, cases of hypersensitivity reactions (at an unknown frequency) were noted, including urticaria, itching, rash and angioedema. There have been reports of episodes of exacerbation of constipation.
Overdose
When using the drug in excessively high doses, severe constipation or bezoar may develop.
