Release form and composition
Dosage forms of Pirantel:
- suspension for oral administration: light yellow, with a characteristic smell of fruit (in 15 ml vials with a measuring cup included, in a cardboard box 1 set);
- tablets: flat, round, yellow with a gray-green tinge, beveled, scored, faint orange smell, slight marbling is possible; under the influence of light, the tablets darken, changing color to yellow-brown (in strips of 3 pcs., in a cardboard box 1 strip).
Composition of 15 ml suspension:
- active substance: pyrantel - 0.75 g (corresponds to the content of pyrantel embonate - 2.162 5 g);
- auxiliary components: citric acid monohydrate - 0.004 92 g; simethicone emulsion - 0.007 5 g; povidone - 0.009 g; polysorbate 80 - 0.03 g; magnesium aluminosilicate - 0.045 g; glycerol - 0.910 5 g; sorbitol 70% (non-crystallizing) - 10.813 5 g; carmellose sodium - 0.006 25 g; sodium benzoate - 0.045 g; apricot flavoring - 0.02 g; sodium hydroxide - qs; purified water - up to 15 ml.
Composition of 1 tablet:
- active substance: pyrantel - 250 mg (corresponds to the content of pyrantel embonate - 720 mg);
- auxiliary components: potato starch, orange flavor, talc, magnesium stearate, povidone, ethyl cellulose.
Pharmacological properties
Pharmacodynamics
Pyrantel is an anthelmintic drug that affects nematodes. In helminths sensitive to the agent, it causes a neuromuscular blockade, thereby facilitating their removal from the digestive tract without stimulating and stimulating the migration of the affected worms.
The drug acts on parasites in the early phase of development and during their maturity. Pirantel does not act on larvae during their migration in tissues.
The drug is highly effective against invasions caused by ascariasis, pathogens of trichocephalosis, hookworms and pinworms.
Pharmacokinetics
Characteristics of pyrantel:
- absorption: very poorly absorbed from the digestive tract; after oral administration in a dose of 10 mg per 1 kg of weight, its concentration in blood plasma varies from 0.005 to 0.13 μg per 1 ml for 1-3 hours;
- distribution: theoretically, the resorption of pyrantel is minimal; there are no data on its distribution in tissues and body fluids, penetration through the placenta and into breast milk;
- metabolism: the minimum amount of pyrantel resorbed from the digestive tract is partially metabolized in the liver to N-methyl-1,3-propanediamine;
- excretion: approximately 93% of the dose taken orally is excreted unchanged through the intestines, 7% (no more) through the kidneys in the form of a metabolite or unchanged.
Indications for use
- trichocephalosis;
- ascariasis;
- ankylostomiasis;
- non-kotorosis;
- enterobiasis.
Contraindications
Oral suspension
Absolute:
- liver failure;
- myasthenia gravis;
- age less than 6 months;
- combination therapy with levamisole and piperazine;
- rare congenital fructose intolerance;
- lactation period;
- individual intolerance to the components contained in the preparation.
Relative (diseases / conditions in the presence of which the appointment of Pirantel requires caution):
- anemia;
- depletion of the body;
- age from 6 months to 6 years and weight less than 10 kg;
- pregnancy.
Pills
Absolute:
- myasthenia gravis (period of therapy);
- age under 3 years old;
- individual intolerance to the components contained in the preparation.
Relative (diseases / conditions in the presence of which the appointment of Pirantel requires caution):
- liver failure;
- pregnancy;
- period of breastfeeding.
Instructions for the use of Pirantel: method and dosage
Oral suspension
The suspension is taken orally during or after meals. Immediately before use, the vial with the drug is shaken until a homogeneous suspension is obtained.
Recommended doses:
- children aged 6 months to 2 years: 125 mg each (2.5 ml, or ½ measuring cup);
- children aged 2 to 6 years: 250 mg each (5 ml, or 1 measuring cup);
- children aged 6 to 12 years: 500 mg (10 ml, or 2 measuring cups);
- children over the age of 12 and adults weighing less than 75 kg: 750 mg each (15 ml, or 3 measuring cups);
- adults weighing more than 75 kg: 1000 mg each (20 ml, 4 measuring cups).
In order to prevent re-infection, the drug is taken again after 21 days after the first dose.
In cases of enterobiasis, ascariasis, mixed infestations by these parasites, the dose intended for one course (at the rate of 10 mg per 1 kg of body weight) is taken once.
Against the background of ankylostomiasis, a combination of noncatorosis with ascariasis or other combined helminthic lesions, the drug is taken for 3 days at 10 mg per 1 kg of body weight per day.
The recommended dose for massive invasions of Necator americanus (necator americanus, or hookworm of the New World) is 20 mg per 1 kg of body weight per day for 2 days.
Pills
The tablets are thoroughly chewed and taken orally, during or after meals with water (1 glass).
Recommended doses:
- children aged 3 to 6 years: 1 pc.;
- children aged 6 to 12 years: 2 pcs.;
- children over 12 years old and adults weighing less than 75 kg: 3 pcs.;
- adults weighing more than 75 kg: 4 pcs.
To avoid self-invasion, a second course of therapy is allowed after 21 days after the first.
In cases of isolated ascariasis, 5 mg of the drug per 1 kg of body weight (once), ascariasis and enterobiasis, as well as mixed infestations by these parasites are taken - the dose intended for the course based on the calculation of 10 mg per 1 kg of body weight (once).
In the treatment of ankylostomiasis, a combination of noncatorosis with ascariasis or other combined helminthic lesions, take 10 mg per 1 kg of body weight per day for 3 days, in the treatment of massive invasions of Necator americanus - 20 mg per 1 kg of body weight per day for 2 days.
Side effects
Oral suspension
Possible adverse reactions (> 10% - very common;> 1% and 0.1% and 0.01% and <0.1% - rarely; <0.01% - very rare):
- nervous system: rarely - insomnia, drowsiness, dizziness, headache; frequency unknown - paresthesia, hallucinations with confusion;
- organ of hearing and labyrinth disorders: frequency unknown - hearing impairment;
- gastrointestinal tract: rarely - decreased appetite, diarrhea, pain and stomach cramps, vomiting, nausea;
- liver and biliary tract: rarely - transient increase in serum aspartate aminotransferase activity;
- skin and subcutaneous tissue: rarely - urticaria, rash;
- general disorders: rarely - weakness; frequency unknown - increase in body temperature.
Pills
While taking the drug, the development of pain and stomach cramps, diarrhea, vomiting, nausea, and lack of appetite is observed. In rare cases, there is a transient increase in the concentration of aspartate aminotransferase in the blood serum, an increase in body temperature, rash, weakness, insomnia / drowsiness, dizziness, headache.
In isolated cases, paresthesias, hallucinations with confusion, and hearing impairment were recorded.
Overdose
Oral suspension
The main symptoms are: hypoxia, tremors, muscle cramps, irregular heartbeat, weakness, sweating, orthostatic fainting, dizziness, confusion, visual impairment.
Therapy: symptomatic treatment, including gastric lavage and measures to maintain blood pressure and breathing.
Pills
Overdose cases of Pirantel in tablet form have not been described. In case of overdose, gastric lavage should be performed, symptomatic treatment should be prescribed. There is no specific antidote.

Before prescribing the drug, it is important to warn the attending physician about the presence of liver diseases.
Do not use laxatives after taking Pirantel.
In cases of enterobiasis, simultaneous therapy of all persons living together is necessary.
To prevent reinfection, careful hygiene is necessary, including cleaning of living quarters and cleaning clothes in order to destroy helminth eggs.
After 2 weeks after treatment with the drug, control parasitological studies are performed.
Due to the fact that the suspension contains sorbitol, it cannot be taken by patients with rare hereditary pathologies (fructose intolerance). Sorbitol may have a laxative effect.

Due to the possible development of dizziness, drowsiness and other undesirable reactions from the nervous system, during the period of taking Pirantel suspension, it is recommended to refrain from driving vehicles and conducting potentially hazardous activities.

Application during pregnancy and lactation
Oral suspension
The use of Pirantel is contraindicated during breastfeeding. In pregnancy, it is prescribed with caution after correlating the predicted benefits to the mother with the potential risks to the fetus.

Pediatric use
