Release form and composition

The drug is produced in the form of enteric-coated tablets: brown or dark brown, round, biconvex, with a specific odor (20, 30 or 60 pcs. 1 tablet contains:

• active substance: pancreatin with the following enzyme activity: amylase - not less than 4620 U (unit of action), lipase - not less than 3850 U, protease - 275-500 U;
• auxiliary components: sodium carboxymethyl starch (primogel), microcrystalline cellulose, pregelatinized starch (starch 1500), povidone (low molecular weight medical polyvinylpyrrolidone), colloidal silicon dioxide (aerosil), magnesium stearate;
• shell composition: tween-80 (polysorbate), cellacephate (acetylphthalylcellulose), vaseline oil (liquid paraffin), brown iron oxide (ferronate E172 or sikovit 70 E172).

Pancreatin forte is a digestive enzyme preparation with proteolytic, amylolytic and lipolytic effects. Compensating for the insufficiency of the secretory function of the pancreas, it helps to improve the digestion of food. Pancreatic enzymes (lipase, trypsin, alpha-amylase, chymotrypsin) are involved in the biotransformation of fats to glycerol and fatty acids, proteins to amino acids, starch to dextrins and monosaccharides.

As a result of improving the breakdown of plant fiber, the hemicellulase enzyme helps to reduce the formation of gases in the intestine.

The analgesic effect of the drug is due to the suppression of stimulated secretion of the pancreas by trypsin.

Pharmacokinetics

After oral administration, the maximum activity of pancreatin is reached after 30–45 minutes.

The enteric coating protects pancreatic enzymes from the acidic environment of gastric juice, their release occurs in the alkaline environment of the small intestine.

Indications for use

• exocrine pancreatic insufficiency: dyspepsia, chronic pancreatitis, pancreatectomy, condition after radiation, cystic fibrosis - as replacement therapy;
• diarrhea of non-infectious genesis, flatulence;
• indigestion due to the condition after surgery for resection of the stomach and small intestine;
• gastrocardiac syndrome (Rehmeld syndrome);
• disorders of chewing function, sedentary lifestyle, prolonged immobilization, inaccuracies in nutrition (irregular nutrition, consumption of large quantities or fatty foods) in persons with normal function of the gastrointestinal tract (GIT) - in order to normalize food digestion;

Contraindications

Absolute:

• acute pancreatitis;

Pancreatin forte tablets are taken orally, swallowing whole (without violating the integrity of the shell), during or immediately after a meal, washed down with a large amount of liquid, preferably alkaline (water, fruit juices).

The doctor sets the dose of the drug in terms of lipase individually, taking into account the degree of pancreatic insufficiency and the patient's age.

The duration of the course of treatment depends on the clinical indication. If the digestive process is disturbed due to errors in the diet, you can limit yourself to a single dose of the drug or a course of several days. With constant substitution therapy, reception can last from several months or more.

For adults, the average daily dose is 150,000 lipase units, with complete insufficiency of the exocrine function of the pancreas, it should correspond to the daily requirement of an adult for lipase - 400,000 units.

Recommended dosage of Pancreatin forte:

• adult patients: 3-4 tablets 3 times a day; during preparation for X-ray or ultrasound examination of the abdominal organs, take 2 tablets 2-3 times a day for 2-3 days before the study;
• children over three years old: 100,000 lipase units per day, divided into 3-4 doses.

The maximum daily dose of enzymes should not exceed 15,000–20,000 IU of lipase per 1 kg of patient weight, especially in cystic fibrosis.

Side effects

• from the digestive system: rarely - epigastric discomfort, nausea, abdominal pain, diarrhea, constipation, decreased endogenous synthesis of bile acids; when used in high doses for a long period - hyperuricosuria; while taking high doses with cystic fibrosis - strictures in the ileocecal and ascending colon, which are manifested by symptoms of intestinal obstruction;
• from the immune system: allergic reactions.

Overdose

Symptoms: hyperuricemia, with prolonged use of high doses of the drug - hyperuricosuria. In patients with cystic fibrosis, high doses can cause strictures in the ileocecal and ascending colon. Overdose in children is accompanied by the development of constipation.
