Urofoscin is an antibacterial drug for the treatment of urinary tract infections, has a wide spectrum of bactericidal action.

Release form and composition

The drug is produced in the form of granules for the preparation of a solution for oral administration: a granular powder of almost white or white color without large agglomerates of stuck together particles, has a faint citrus smell (5.334 or 8 g in bags, in a cardboard box 1 or 2 bags and instructions for use Urofoscin).

1 package contains:

  • active substance: fosfomycin trometamol - 3.754 or 5.631 g, which corresponds to 2 or 3 g of fosfomycin, respectively;
  • auxiliary components: sodium saccharinate, sucrose, orange flavor, tangerine flavor.

Pharmacological properties

Pharmacodynamics

Urofoscin is a broad spectrum antibacterial drug that has a bactericidal effect. Its active ingredient, fosfomycin trometamol, is a phosphonic acid derivative, a structural analogue of phosphoenol pyruvate.

The mechanism of action of Urofoscin is due to the ability of fosfomycin to suppress the first stage of the synthesis of the bacterial cell wall. As a result of its competitive interaction with the enzyme N-acetyl-glucosamino-3-o-enolpyruvyl transferase, specific, irreversible and selective inhibition of this enzyme occurs. This eliminates cross-resistance with other classes of antibiotics and provides the possibility of synergy with antibiotics such as amoxicillin, cephalexin, pipemidic acid.

The in vitro spectrum of antibacterial activity of fosfomycin trometamol includes the following pathogens:

  • gram-positive microorganisms: Enterococcus faecalis, Enterococcus speciales (spp.), Staphylococcus aureus, Staphylococcus saprophyticus, Staphylococcus spp.;
  • gram-negative microorganisms: Escherichia coli, Proteus mirabilis, Enterobacter spp., Citrobacter spp., Klebsiella spp., Morganella morganii, Klebsiella pneumoniae, Serratia spp., Pseudomonas spp.

Under in vitro conditions, it was found that the action of fosfomycin trometamol helps to reduce the adhesion of a number of bacteria to the epithelium of the urinary tract.

Pharmacokinetics

After oral administration, fosfomycin is rapidly absorbed from the gastrointestinal tract (GIT). Simultaneous food intake slows down its absorption. The bioavailability of the drug is 34–65%. With a single oral administration in a dose of 3 g, the maximum plasma concentration is reached after 2–2.5 hours and is 22–32 mg / l. The concentration level of fosfomycin in urine is from 1053 to 4415 mg / l, it is 99% bactericidal for most common causative agents of urinary tract infections, for which the minimum inhibitory concentration is 128 mg / l. In urine, it is maintained for 24–48 hours. For Escherichia coli, the level of fosfomycin in urine above the minimum inhibitory concentration is maintained for 80 hours or more.

Fosfomycin does not bind to plasma proteins. It accumulates mainly in the urine, with the creation of high concentrations.

In the body, the active substance is not metabolized, but dissociates into fosfomycin and trometamol, which does not possess antibacterial properties.

The half-life (T 1/2) from plasma is 4 hours. Fosfomycin is excreted unchanged: through the kidneys - 90%, through the intestines - about 10% of the dose taken.

With a moderate decrease in renal function with creatinine clearance (CC) less than 80 ml / min and in the elderly, T 1/2 is slightly lengthened, but this does not affect the concentration of fosfomycin in the urine, it corresponds to the therapeutic level.

Indications for use

The use of Urofoscin is indicated in the treatment of infectious and inflammatory diseases of the urinary tract of various localization, caused by pathogens sensitive to fosfomycin, including acute uncomplicated urinary tract infections and asymptomatic bacteriuria.

In addition, the drug is used to prevent urinary tract infections after transurethral diagnostic tests or surgery.

Contraindications

Absolute:

  • severe renal failure (CC less than 10 ml / min);
  • intolerance to fructose, sucrase or isomaltase deficiency, glucose-galactose malabsorption syndrome;
  • breast-feeding;
  • age up to 5 years;
  • hypersensitivity to the components of the drug.

Caution should be given to Urofoscin during pregnancy.

Urofoscin, instructions for use: method and dosage

The solution prepared from granules is taken orally, having previously emptied the bladder, 2-3 hours before or after a meal, preferably before bedtime.

To prepare the solution, the contents of the package should be dissolved in 100 ml of water.

The recommended dosage has the following age restrictions:

  • adults and adolescents over the age of 12 years: 3 g once a day;
  • children from 5 to 12 years old: 2 g once a day.

The course of treatment is 1 day.

In order to prevent urinary tract infections, the above dose of Urofoscin should be taken 2 times. The first time is 3 hours before surgery or diagnostic research, and the second time - a day after their completion.

Side effects

  • from the digestive system: dyspepsia, heartburn, diarrhea, nausea, vomiting, abdominal pain, pseudomembranous colitis, increased activity of hepatic transaminases;
  • from the immune system: allergic reactions, skin rash, itching, urticaria, angioedema, anaphylactic shock;
  • from the nervous system: headache, dizziness, paresthesia (feeling of numbness of the skin or "creeping"), asthenia, optic neuritis;
  • on the part of the cardiovascular system: lowering blood pressure, tachycardia;
  • others: vulvovaginitis, bronchial asthma, thrombocytosis, leukopenia.

Overdose

Symptoms: General taste disorders, including metallic taste, diarrhea, hearing impairment, vestibular syndrome.

Treatment: in order to increase urine output, it is recommended to drink plenty of fluids. If necessary, symptomatic therapy is prescribed.

special instructions

During the period of treatment with Urofoscin or after its termination, the occurrence of acute and persistent diarrhea may be a symptom of colitis (including pseudomembranous colitis) caused by taking an antibiotic. In this case, the use of drugs that slow down peristalsis is contraindicated. When pseudomembranous colitis is diagnosed, immediate action is required.

Patients with diabetes mellitus should take into account that the sucrose content in 1 package with a dosage of 2 g is 1.475 g, with a dosage of 3 g - 2.213 g.

Influence on the ability to drive vehicles and complex mechanisms

Due to the fact that while taking Urofoscin, the development of undesirable phenomena from the nervous system is possible, patients are advised to be careful and refrain from driving vehicles and complex mechanisms when dizziness appears.

Application during pregnancy and lactation

The use of Urofoscin granules during breastfeeding is contraindicated.

If necessary, taking fosfomycin during lactation for the period of treatment should stop breastfeeding.

During pregnancy, the drug should be prescribed with caution, only in cases where the expected clinical effect for the mother outweighs the potential threat to the fetus.

Pediatric use

The use of Urofoscin is contraindicated for the treatment of children under the age of 5 years.

With impaired renal function

Drug interactions

With the simultaneous administration of Urofoscin with antacids, metoclopramide, calcium salts containing drugs or drugs that enhance gastrointestinal motility, a decrease in the concentration of fosfomycin in serum and urine is possible, so such combinations should be avoided.
