Release form and composition
Dosage form - coated tablets:
- 12.5 mg: round, convex on both sides, white, marked "1L" on one side;
- 25 mg: round, convex on both sides, white, marked "2L" on one side;
- 50 mg: round, convex on both sides, white, with side marks and marks on both sides, with markings "3" and "L" on both sides of the marks;
- 100 mg: oval, convex on both sides, white, with a notch on one side and the marking "4L" on the other, with lateral notches.

Pharmacological properties
Pharmacodynamics
Losartan is a specific angiotensin II receptor antagonist, belongs to the AT 1 subtype. Kinase II (enzyme degrading bradykinin) does not suppress.
The main effects of losartan:
- a decrease in the total peripheral vascular resistance, the concentration of aldosterone and adrenaline in the blood, blood pressure, pressure in the pulmonary circulation;
- reduced afterload;
- diuretic effect;
- preventing the development of myocardial hypertrophy;
- increased exercise tolerance against the background of heart failure.
Vasotenz has a hypotensive effect after a single dose (manifests itself as a decrease in systolic and diastolic pressure), reaching a maximum after 6 hours, then over 24 hours the effect gradually decreases.
The maximum hypotensive effect of Vasotenza develops 3–6 weeks after the start of administration.
Pharmacokinetics
Losartan is rapidly absorbed from the gastrointestinal tract. Bioavailability is approximately 33%. T max (time to reach the maximum concentration of the substance) - 60 minutes.
Losartan undergoes the effect of the first passage through the liver, metabolism occurs by carboxylation with the participation of the CYP2C9 isoenzyme, and an active metabolite is formed. T max of the active metabolite is 3-4 hours, the degree of its binding to blood plasma proteins is 99%.
T 1/2 (half-life) of a substance is in the range from 1.5 to 2 hours, its main metabolite is 6-9 hours. Approximately 35% of the dose is excreted in the urine, through the intestines - about 60%.
With cirrhosis of the liver, the plasma concentration of losartan increases significantly.
Indications for use
- monotherapy: arterial hypertension;
- combination therapy: chronic heart failure [in case of ineffectiveness or intolerance to angiotensin-converting enzyme (ACE) inhibitors].
Contraindications
- dehydration;
- hyperkalemia;
- arterial hypotension;
- age up to 18 years;
- pregnancy and lactation;
- hypersensitivity to the components of Vasotenza.

Vasotenz, instructions for use: method and dosage
Vasotenz tablets should be taken orally 1 time per day (regardless of the prescribed dose). Meal time does not matter.
Standard dosage regimens of Vasotenza:
- arterial hypertension: the average therapeutic dose is 50 mg, to achieve a greater effect, it is possible to increase the dose to 100 mg, if necessary, the daily dose can be divided into 2 doses. The initial dose for patients receiving high doses of diuretics is 25 mg;
- heart failure: the initial dose is 12.5 mg, then it is increased at intervals of 1 week, first to 25 mg, then to 50 mg. The average maintenance dose is 50 mg.
At lower doses, Vasotenz is prescribed for patients with functional liver disorders, including cirrhosis.
Side effects
In most cases, Vasotens is well tolerated, adverse reactions are transient and do not require discontinuation of therapy.
Possible side effects:
- on the part of the cardiovascular system: orthostatic hypotension (dose-dependent), palpitations, arrhythmias, bradycardia, tachycardia, angina pectoris;
- from the nervous system: often (≥ 1%) - dizziness, fatigue, asthenia, insomnia, headache; rarely (<1%) - memory disorders, ataxia, paresthesia, drowsiness, sleep disturbance, migraine, depression, peripheral neuropathy, syncope, hyposthesia, anxiety, tremor;
- from the respiratory system: often - cough, nasal congestion, upper respiratory tract infections (pharyngitis, sinusitis, sinusopathy, sore throat, fever); rarely - rhinitis, bronchitis, dyspnea;
- from the digestive system: often - dyspeptic symptoms, diarrhea, abdominal pain, nausea; rarely - toothache, dry mouth, flatulence, constipation, anorexia, gastritis, vomiting;
- from the genitourinary system: often - impaired renal function, urinary tract infections, urge to urinate, weakening of libido, impotence;
- from the musculoskeletal system: often - pain in the legs, chest and back, myalgia, cramps; rarely - fibromyalgia, arthralgia, knee and shoulder pain, arthritis;
- from the senses: taste disturbance, ringing in the ears, conjunctivitis, changes in vision;
- dermatological reactions: often - photosensitivity, hot flushes, increased sweating, erythema, dry skin, alopecia;
- allergic reactions: often - itching, skin rashes, urticaria, angioedema (including swelling of the lips, pharynx, face and / or tongue);
- others: anemia, hyperkalemia (serum potassium more than 5.5 mmol / l).
The relationship of side effects, developing rarely (with a frequency of <1%), with the use of losartan has not been reliably established.
Overdose
The main symptoms: a marked decrease in blood pressure, tachycardia, bradycardia (associated with parasympathetic stimulation).
Therapy: forced diuresis, symptomatic treatment; hemodialysis is ineffective.

Before prescribing Vasotenza, dehydration should be corrected or treatment should be started with a lower dose.
During the use of the drug, regular monitoring of the concentration of potassium in the blood is necessary, especially in the elderly and patients with impaired renal function.
If necessary, Vasotens can be used in combination with other antihypertensive drugs.
In patients with bilateral renal stenosis or stenosis of an artery of a solitary kidney, losartan may increase the concentration of serum creatinine and the level of urea in the blood.
Drugs that act directly on the renin-angiotensin system, when used in the II and III trimesters of pregnancy, can cause fetal defects and even intrauterine death. In this regard, in case of pregnancy, Vasotens should be immediately canceled.
Application during pregnancy and lactation
Vasotenz tablets are not prescribed during pregnancy / lactation.
Pediatric use
For patients under 18 years of age, Vasotens is not prescribed.

Drug interactions
In the case of the simultaneous use of potassium preparations or potassium-sparing diuretics, the risk of developing hyperkalemia increases.
When conducting combination therapy with other antihypertensive drugs (diuretics, sympatholytics, beta-blockers), there is a mutual enhancement of the action.
There was no clinically significant interaction with the simultaneous use of digoxin, cimetidine, indirect anticoagulants, hydrochlorothiazide, phenobarbital.
