Dicloran plus is a non-steroidal anti-inflammatory drug (NSAID) of a combined composition for external use.

Release form and composition

Dicloran plus dosage form is a gel for external use (10, 30 and 50 g each in aluminum / plastic tubes, 1 tube in a cardboard box).

Active ingredients in 1 g of gel:

  • methyl salicylate - 100 mg;
  • levomenthol - 50 mg;
  • flax seed oil - 30 mg;
  • diclofenac diethylamine - 11.61 mg, which corresponds to 10 mg of diclofenac sodium.

Auxiliary components in the gel: propylene glycol, benzyl alcohol, citric acid monohydrate, carbomer 980, butylhydroxytoluene, macrogol glyceryl hydroxystearate (Cremophor RH-40), disodium edetate, butylhydroxyanisole, diethylamine.

Pharmacological properties

Dicloran plus has anti-inflammatory, anti-exudative and analgesic effects.

Pharmacodynamics

Methyl salicylate and sodium diclofenac are NSAIDs, the mechanism of action of which is explained by the ability to suppress the synthesis of prostaglandins. When applied topically, these substances reduce swelling and morning stiffness in joints, inflammation and pain in joints, and increase the number of movements.

Flax seed oil contains alpha-linolenic acid, which has anti-inflammatory properties.

Levomenthol has a mild analgesic and local distracting effect, irritates the nerve endings, a feeling of coolness at the site of application of the gel.

Pharmacokinetics

When applied topically, the active substances of Dicloran plus are absorbed and penetrate into the subcutaneous tissue, muscle tissue and the joint capsule.

Indications for use

  • diseases of the musculoskeletal system, including radiculitis, psoriatic arthritis, osteoarthritis of the peripheral joints and spine, ankylosing spondylitis, rheumatoid arthritis, osteochondrosis with radicular syndrome;
  • muscle pains of rheumatic and non-rheumatic origin, including those caused by excessive physical exertion;
  • rheumatic soft tissue lesions such as bursitis and tendovaginitis;
  • post-traumatic inflammation of soft tissues and joints (due to bruises, sprains and overstrain).

Contraindications

  • violation of the integrity of the skin;
  • complete / incomplete combination of bronchial asthma, recurrent polyposis of the nasal mucosa and paranasal sinuses with intolerance to acetylsalicylic acid (ASA) or other NSAIDs at the present time or in history;
  • children under 6 years old;
  • III trimester of pregnancy;
  • lactation period;
  • increased sensitivity to any component of the drug.

Dicloran plus should be used with caution in pregnant women in the I and II trimesters, elderly people, patients with bronchial asthma, severe renal and liver dysfunction.

Instructions for the use of Dicloran plus: method and dosage

Dicloran plus gel is applied externally. The product should be applied to the affected area and rubbed lightly.

A single dose for adults and adolescents from 12 years old depends on the size of the painful zone and is 2–4 g, which corresponds to a strip of gel 4–8 cm long (respectively) in the case of squeezing out the drug with the tube neck completely open. The frequency of application is 3-4 times a day.

Children 6–12 years old can use the drug 1–2 times a day in a single dose of no more than 2 g.

Unless otherwise prescribed by a doctor, the duration of treatment is no more than 10 days. The need for a longer treatment is decided individually.

Side effects

Dicloran Plus can cause the following side effects:

  • local: photosensitization, eczema, contact dermatitis (swelling of the treated skin area, peeling, papules, hyperemia, vesicles, itching);
  • systemic: allergic reactions (angioedema, urticaria, bronchospastic reactions), generalized skin rash.

Overdose

Subject to the rules of application and dosage of the drug, the systemic absorption of the active ingredients of the gel is extremely low, and therefore an overdose is practically impossible.

Systemic side effects may occur after accidental ingestion of the drug. In this case, it is necessary to artificially induce vomiting, rinse the stomach and take activated charcoal. If required, forced diuresis is performed and symptomatic therapy is prescribed to eliminate the undesirable effects that have arisen. Dialysis is ineffective, since diclofenac is characterized by a high degree of binding to blood plasma proteins (about 99%).

The gel can only be applied to the skin without damage, avoiding contact with open wounds, since in this case (due to the resorptive effect) there is a high risk of developing systemic side effects.

Do not allow the drug to come into contact with mucous membranes and eyes. Do not apply under an occlusive dressing.
