Espa-Bastin is an antiallergic drug, blocker of H 1 -histamine receptors.

Release form and composition

Dosage form - film-coated tablets: round, white or almost white, flat-cylindrical (for a dosage of 10 mg) or slightly biconvex (for a dosage of 20 mg), with a risk on one side (10 tablets in blisters, in a cardboard box 1 blister and instructions for the use of Espa-Bastin).

Composition of 1 tablet:

  • active ingredient: ebastine - 10/20 mg;
  • additional ingredients: MCC (microcrystalline cellulose) - 85/171 mg; colloidal anhydrous silicon dioxide - 1/2 mg; sodium carboxymethyl starch (type A) - 3/6 mg; magnesium stearate - 0.5 / 1 mg;
  • film shell: Opadray Y-1-7000 white (hypromellose-5cP - 4.69 / 9.38 mg; macrogol 400 - 0.47 / 0.94 mg; titanium dioxide - 2.34 / 4.68 mg).

Pharmacological properties

Pharmacodynamics

Ebastine belongs to long-acting H 1 -histamine receptor blockers.

The antihistamine effect of Espa-Bastin is manifested 60 minutes after a single oral administration and is observed for two days. With long-term therapy, a high level of blockade of peripheral H 1 - histamine receptors remains, which is not accompanied by the development of tachyphylaxis. After a course of treatment lasting 5 days, antihistamine activity persists for three days due to the action of the active metabolite of ebastine. There are no pronounced anticholinergic and sedative effects of the drug; it does not penetrate the blood-brain barrier. Taking ebastine at a dose of 100 mg [5 times the recommended daily dose (20 mg)] does not affect the QT interval on the ECG (electrocardiogram).

Pharmacokinetics

After oral administration, ebastine is rapidly absorbed and almost completely metabolized in the liver. In the course of metabolism, karebastine is formed, a metabolite with pharmacological activity. After taking 20 mg of the drug once, the maximum concentration of carbastine in the blood plasma is 157 ng / ml and is reached after 60–180 minutes.

Daily intake of the drug in doses of 10–40 mg allows reaching the equilibrium concentration (Css) in 3–5 days. Css is 130–160 ng / ml and does not depend on the administered dose. The connection with blood plasma proteins of ebastine and its active metabolite (carabastine) exceeds 95%.

The half-life of carabastine varies within 15–19 hours, 66% of the drug is excreted through the kidneys in the form of conjugates.

Food intake does not affect the clinical effects of the drug.

In elderly patients, no significant changes in the pharmacokinetic parameters of Espa-Bastin are observed.

In case of impaired renal function, the half-life increases to 23–26 hours, and in case of impaired liver function - up to 27 hours, while the concentration of the drug does not exceed therapeutic values.

Indications for use

Espa-Bastin is prescribed for allergic rhinitis of various etiologies (seasonal and / or perennial), as well as urticaria of various etiologies, including chronic idiopathic.

Contraindications

Absolute:

  • severe hepatic dysfunction (class C on the Child-Pugh scale);
  • age up to 12 years;
  • pregnancy;
  • breast-feeding;
  • hypersensitivity to the active or auxiliary components of the drug.

Relative (taking Espa-Bastin must be taken with precautions):

  • mild to moderate hepatic impairment (classes A and B on the Child-Pugh scale);
  • renal failure;
  • lengthening the QT interval on the ECG;
  • combined use with erythromycin, itraconazole or ketoconazole (due to the possible increase in the risk of prolongation of the QT interval on the ECG).

Espa-Bastin, instructions for use: method and dosage

Espa-Bastin tablets are taken orally, regardless of the meal.

Adults and children over 12 years old at the beginning of therapy take 10 mg of ebastine 1 time per day (it is recommended to use Espa-Bastin tablets with a dosage of 10 mg).

In the absence of a sufficient effect, the dose must be doubled, that is, take the drug 20 mg once a day (it is recommended to use Espa-Bastin tablets with a dosage of 20 mg).

The course of treatment is determined by the disappearance of the symptoms of the disease.

Recommendations for use in special categories of patients:

  • impaired renal function: no dose adjustment is required;
  • hepatic impairment: severe (class C according to Child-Pugh classification) - the daily dose should not exceed 10 mg, and therefore it is recommended to use Espa-Bastin tablets with a dosage of 10 mg; mild to moderate (Child-Pugh classes A and B) - no dose adjustment is required;
  • advanced age: dose adjustment is not required.

Side effects

The classification of adverse events is made in accordance with the frequency of their development: very often - more than 1/10; often - from 1/100 to 1/10; infrequently - from 1/1000 to 1/100; rarely - from 1/10 000 to 1/1000; extremely rare - less than 1/10 000; with an unknown frequency - there is no possibility to determine the incidence of side effects based on the available data.

Possible violations from the systems and organs when taking the antihistamine Espa-Bastin:

  • central nervous system (CNS): often - headache, drowsiness; extremely rare - dysesthesia, paresthesia, dizziness, nervousness, insomnia;
  • cardiovascular system: extremely rare - palpitation, tachycardia;
  • digestive system: often - dryness of the oral mucosa; rarely - dyspepsia, abdominal pain, nausea; extremely rarely - vomiting;
  • liver and biliary tract: extremely rarely - changes in laboratory parameters (increased activity of gamma-glutamyltransferase, hepatic transaminases, bilirubin, alkaline phosphatase), cholestasis, hepatitis;
  • skin and subcutaneous tissues: extremely rare - dermatitis, rash, urticaria;
  • immune system: with an unknown frequency - hypersensitivity reactions (for example, angioedema, anaphylaxis);
  • reproductive system: extremely rare - menstrual disorders.

Overdose

Taking a dose of ebastine in the amount of 100 mg per day does not cause clinically significant symptoms of overdose.

If an overdose of ebastine is suspected, gastric lavage is prescribed, followed by symptomatic therapy. During treatment, it is necessary to control the vital functions of the body, including performing ECG monitoring. There is no specific antidote.

special instructions

Taking ebastine can affect the results of allergy skin tests. To avoid data distortion, such tests should be carried out no earlier than 5-7 days after the cancellation of Espa-Bastin.

Patients with hypokalemia, prolongation of the QT interval on the ECG, and also taking azole antimycotics and antibiotics of the macrolide group simultaneously with ebastine should be careful during the period of drug therapy.

In addition, caution when taking Espa-Bastin should be observed in patients with severe hepatic impairment.

Influence on the ability to drive vehicles and complex mechanisms

Espa-Bastin, taken in recommended therapeutic doses, has no effect on the ability to drive vehicles and control complex mechanisms.

In the event of the appearance of undesirable phenomena from the central nervous system, for example, drowsiness, care must be taken when engaging in potentially hazardous activities that require the speed of psychomotor reactions and increased concentration of attention (driving, the work of an operator, dispatcher, etc.).

Application during pregnancy and lactation

The drug is not prescribed to pregnant women, since there have been no studies on the safe use of Espa-Bastin in patients of this category.

The period of breastfeeding is an absolute contraindication to taking Espa-Bastin.

Pediatric use

In pediatrics, antiallergic agent is not prescribed to patients under 12 years of age.

With impaired renal function

  • theophylline, indirect anticoagulants, cimetidine, diazepam, ethanol and ethanol-containing drugs: no clinically significant interaction was found;
  • other antihistamines: their effect may be enhanced.
