Release form and composition

Escape's dosage form - film-coated tablets: biconvex, round, white or almost white on the outside and at the break, have a faint smell of ammonia (10 or 14 pieces in a blister, 4 or 8 blisters in a cardboard box).

Composition of 1 tablet:

• active substance: bismuth tripotassium dicitrate - 300 mg (equivalent to 120 mg of bismuth oxide);
• auxiliary components: corn starch, povidone, polacrilin, magnesium stearate;
• film shell: hydroxypropyl methylcellulose, hydroxypropyl cellulose, stearic acid, talc, polyethylene glycol, titanium dioxide.

Pharmacological properties

Pharmacodynamics

Escape - gastroprotector; antiulcer drug with bactericidal activity against Helicobacter pylori, as well as astringent and anti-inflammatory effects.

Insoluble bismuth oxychloride and citrate are deposited in the stomach, forming chelating compounds with a protein substrate, thereby creating a protective film on the surface of erosions and ulcers. This film serves as a protective layer against the influence of aggressive factors and remains on the damaged surface for a long time. Escape increases the synthesis of prostaglandin E, the secretion of bicarbonate, the formation of mucus, stimulates the activity of cytoprotective mechanisms, increases the resistance of the mucous membrane of the gastrointestinal tract (GIT) to the effects of hydrochloric acid, pepsin, enzymes and bile salts. Leads to the accumulation of epidermal growth factor in the defect area. Reduces the activity of pepsin and pepsinogen.

Pharmacokinetics

Escape has a very low absorption capacity. It is excreted mainly in the feces. A small amount of it, which has entered the plasma, is excreted by the kidneys.

Indications for use

• functional dyspepsia (in the absence of organic gastrointestinal diseases);
• chronic gastritis and gastroduodenitis at the stage of exacerbation, including those associated with Helicobacter pylori;
• peptic ulcer of the stomach and duodenum at the stage of exacerbation, including associated with Helicobacter pylori;
• irritable bowel syndrome, proceeding mainly with symptoms of diarrhea.

Contraindications

• joint administration of preparations containing bismuth;
• chronic renal failure;
• age up to 4 years;
• pregnancy and lactation;
• increased sensitivity to Escape components.

Instructions for the use of Escape: method and dosage

Escape tablets are taken orally, whole, with a sufficient amount of water.

Recommended dosage:

• adults and children over 12 years old: 1 tablet 4 times a day, or 2 tablets 2 times a day, half an hour before meals and at night;
• children from 8 to 12 years old: 1 tablet 2 times a day, half an hour before meals;
• children from 4 to 8 years old: the drug is prescribed at the rate of 8 mg / kg / day, 1-2 tablets per day (for 1 or 2 doses), half an hour before meals.

The duration of treatment is usually 4–8 weeks. Within 2 months after the end of the course, it is not recommended to take medicines containing bismuth.

For the purpose of eradication of Helicobacter pylori, it is advisable to use Escape as part of a combination therapy with antibacterial drugs with anti-Helicobacter activity.

Side effects

• Gastrointestinal tract: stool staining black, nausea, vomiting, constipation, diarrhea;
• allergic reactions: itching, rash, anaphylactic reactions (very rare);
• nervous system: encephalopathy (with prolonged use in high doses).

After the Escape is canceled, the side effects disappear within a short period of time.

Overdose

Long-term use of the drug, as well as exceeding doses dozens of times, can lead to bismuth poisoning. In this case, the following symptoms are possible: dyspepsia, rash, inflammation of the oral mucosa and its characteristic darkening in the form of blue lines on the gums, impaired renal function (with prolonged excess doses). After the Escape is canceled, these symptoms completely disappear.

The specific antidote is unknown. Gastric lavage, intake of enterosorbents, as well as symptomatic therapy aimed at maintaining renal function are recommended. In case of impaired renal function with a high level of bismuth in the blood plasma, it is possible to introduce complexing agents - dimercaptosuccinic and dimercaptopropanesulfonic acids. In case of impaired renal function in a severe stage, hemodialysis is indicated.

Do not exceed the recommended doses, and take Escape tablets for longer than 8 weeks.

During the period of drug therapy, feces may become dark in color due to the formation of bismuth sulfide. Darkening of the tongue is sometimes noted.

Application during pregnancy and lactation

If necessary, taking the drug during lactation, breastfeeding should be discontinued.

Drug interactions

It is not recommended to take medications 30 minutes before and 30 minutes after taking Escape, you should also refrain from eating and drinking for this period of time (for example, antacids, milk, fruits and juices).

When taken together with Escape, the absorption of tetracyclines slows down.

Joint reception of Escape with other drugs containing bismuth is contraindicated, as this can lead to the development of side effects, including encephalopathy.
