Escapel tablets
Escapel tablets

Escapel is a progestogen, postcoital contraceptive drug.

Release form and composition

Escapel is available in the form of tablets: round, flat, beveled, white or almost white, engraved "G00" on one side (1 pc. In a blister, 1 blister in a cardboard box).

1 tablet contains:

  • active substance: levonorgestrel - 1.5 mg;
  • auxiliary substances: corn starch, potato starch, lactose monohydrate, magnesium stearate, colloidal anhydrous silicon dioxide, talc.

Pharmacological properties

Pharmacodynamics

Escapel is a contraceptive drug with a pronounced gestagenic and antiestrogenic effect.

Taking the drug provides suppression of ovulation and fertilization after unprotected intercourse during the period of the highest probability of fertilization - the preovulatory phase of the menstrual cycle. The mechanism of action of the drug is due to the ability of levonorgestrel, to induce regression of endometrial proliferation, preventing implantation of a fertilized egg. If implantation has already taken place, levonorgestrel is ineffective.

The recommended dosage regimen of the drug does not cause a significant change in blood coagulation factors, fat and carbohydrate metabolism.

The effectiveness of the drug depends on the period of time between unprotected intercourse and taking the pill. If taken within the first 24 hours, the contraceptive protection guarantee is 95%, in the period from 24 to 48 hours - 85%, from 48 to 72 hours - 58%. Therefore, it is recommended to take Escapel as soon as possible after sexual intercourse, unless other contraceptive measures have been used.

Pharmacokinetics

After oral administration of the Escapel tablet, levonorgestrel is rapidly and completely absorbed from the gastrointestinal tract (GIT). Its Cmax (maximum concentration) in plasma reaches 18.5 ng / ml after 2 hours, then begins to gradually decrease. The absolute bioavailability is 100%.

Plasma binding occurs with sex hormone binding globulin (SHBG) and albumin, while up to 65% of the total amount of levonorgestrel is specifically associated with SHBG and only 1.5% is in free form.

Biotransformation of levonorgestrel occurs in the liver, after hydroxylation in the form of inactive metabolites, the substance is excreted from the body.

T 1/2 (half-life) is 26 hours. The drug is excreted through the kidneys and intestines in the form of metabolites in approximately equal proportions.

In diseases of the gastrointestinal tract that cause malabsorption, the absorption of levonorgestrel and its effectiveness are reduced.

Indications for use

The use of Escapel is indicated as an emergency or postcoital contraception in cases where intercourse was unprotected, or the effectiveness of the contraceptives used is in doubt.

Contraindications

  • severe liver failure;
  • lactase deficiency, glucose-galactose malabsorption syndrome, lactose intolerance;
  • period of pregnancy;
  • age under 16;
  • hypersensitivity to the components of the drug.

According to the instructions, Escapel should be taken with caution in case of liver diseases, biliary tract, jaundice (including history), Crohn's disease, breastfeeding.

Instructions for use of Escapel: method and dosage

Escapel tablets are taken orally.

One tablet should be taken immediately after unprotected intercourse, but no later than 72 hours later. The effectiveness of emergency contraception is significantly reduced with delayed administration of the drug. If during the first three hours after taking the pill the woman has vomiting, then it is necessary to take the drug again in the original dose.

The use of Escapel is indicated in any phase of the menstrual cycle. With irregular monthly bleeding, it is necessary to exclude the presence of pregnancy.

It is not recommended to re-use the drug during one menstrual cycle. This can cause an increase in the frequency of acyclic spotting or bleeding.

Side effects

  • from the central nervous system: very often - fatigue; often - dizziness, headache;
  • from the digestive system: very often - nausea; often - diarrhea, vomiting;
  • from the reproductive system: very often - acyclic bleeding (spotting), pain in the lower abdomen; often - menstrual irregularities (delayed menstruation no more than 5-7 days), breast tenderness.

Overdose

Symptoms: increased severity of side effects.

Treatment: there is no specific antidote, symptomatic therapy is performed if necessary.

special instructions

In case of development of allergic reactions, the use of tablets should be discontinued. Side effects are transient and do not require special treatment.

Escapel is an emergency contraceptive only and is only recommended for a single dose during the menstrual cycle. If necessary, barrier methods of contraception should be used before the next menstrual period.

If, while taking the drug, menstruation is delayed for more than 7 days, conception must be excluded.

If pain in the lower abdomen or fainting occurs, it should be borne in mind that they may be caused by an ectopic (ectopic) pregnancy.

The tablets do not protect against sexually transmitted diseases.

The contraceptive effect of Escapel may be reduced in women with impaired gastrointestinal function, including Crohn's disease.

Girls under 16 years old, in exceptional cases (including possible rape), prior to taking the drug, require a consultation with a gynecologist to confirm pregnancy. After taking emergency contraception measures, a second consultation is necessary to select a more acceptable method for permanent contraception.

Studies on the effect of Escapel on the ability to drive vehicles and complex mechanisms have not been conducted.

For violations of liver function

Reviews about Escapelle

Reviews about Escapel by gynecologists are positive. They indicate that the drug is one of the most effective and safe means of emergency postcoital contraception. Doctors argue their conclusions by the complete elimination of the drug from the body within 24 hours, the lack of influence on blood coagulation factors, carbohydrate and lipid metabolism, as well as a cumulative effect.

Reviews of women regarding the effectiveness of Escapel are often positive. Timely intake of the drug after intercourse prevents unwanted conception in unforeseen situations. There are frequent reports of side effects of varying severity, including nausea, headache, and menstrual irregularities. In some cases, the pills caused hormonal disruption.
