Release form and composition
Escapel is available in the form of tablets: round, flat, beveled, white or almost white, engraved "G00" on one side (1 pc. In a blister, 1 blister in a cardboard box).
1 tablet contains:
- active substance: levonorgestrel - 1.5 mg;
- auxiliary substances: corn starch, potato starch, lactose monohydrate, magnesium stearate, colloidal anhydrous silicon dioxide, talc.
Pharmacological properties
Pharmacodynamics
Escapel is a contraceptive drug with a pronounced gestagenic and antiestrogenic effect.
Taking the drug provides suppression of ovulation and fertilization after unprotected intercourse during the period of the highest probability of fertilization - the preovulatory phase of the menstrual cycle. The mechanism of action of the drug is due to the ability of levonorgestrel, to induce regression of endometrial proliferation, preventing implantation of a fertilized egg. If implantation has already taken place, levonorgestrel is ineffective.
The recommended dosage regimen of the drug does not cause a significant change in blood coagulation factors, fat and carbohydrate metabolism.
The effectiveness of the drug depends on the period of time between unprotected intercourse and taking the pill. If taken within the first 24 hours, the contraceptive protection guarantee is 95%, in the period from 24 to 48 hours - 85%, from 48 to 72 hours - 58%. Therefore, it is recommended to take Escapel as soon as possible after sexual intercourse, unless other contraceptive measures have been used.
Pharmacokinetics
After oral administration of the Escapel tablet, levonorgestrel is rapidly and completely absorbed from the gastrointestinal tract (GIT). Its Cmax (maximum concentration) in plasma reaches 18.5 ng / ml after 2 hours, then begins to gradually decrease. The absolute bioavailability is 100%.
Plasma binding occurs with sex hormone binding globulin (SHBG) and albumin, while up to 65% of the total amount of levonorgestrel is specifically associated with SHBG and only 1.5% is in free form.
Biotransformation of levonorgestrel occurs in the liver, after hydroxylation in the form of inactive metabolites, the substance is excreted from the body.
T 1/2 (half-life) is 26 hours. The drug is excreted through the kidneys and intestines in the form of metabolites in approximately equal proportions.
In diseases of the gastrointestinal tract that cause malabsorption, the absorption of levonorgestrel and its effectiveness are reduced.
Indications for use
The use of Escapel is indicated as an emergency or postcoital contraception in cases where intercourse was unprotected, or the effectiveness of the contraceptives used is in doubt.
Contraindications

Instructions for use of Escapel: method and dosage
Escapel tablets are taken orally.
One tablet should be taken immediately after unprotected intercourse, but no later than 72 hours later. The effectiveness of emergency contraception is significantly reduced with delayed administration of the drug. If during the first three hours after taking the pill the woman has vomiting, then it is necessary to take the drug again in the original dose.
The use of Escapel is indicated in any phase of the menstrual cycle. With irregular monthly bleeding, it is necessary to exclude the presence of pregnancy.
It is not recommended to re-use the drug during one menstrual cycle. This can cause an increase in the frequency of acyclic spotting or bleeding.
Side effects

- cyclosporine: levonorgestrel may inhibit the metabolism of cyclosporine, increasing the risk of toxicity;
- GCS (glucocorticosteroids): the action of levonorgestrel increases the level of their concentration in plasma.
