Elzhina is an antimicrobial combined preparation for intravaginal use.

Release form and composition

Elzhina is produced in the form of vaginal tablets: oblong, flat, beveled, white with a yellow tinge or white, marbling is possible (3 pieces in a blister strip, 1, 2 or 3 packages in a cardboard box).

Composition of 1 vaginal tablet:

  • active ingredients: neomycin (in the form of sulfate) - 65,000 international units (IU), ornidazole - 500 mg, econazole (in the form of nitrate) - 100 mg, prednisolone (in the form of sodium phosphate) - 3 mg;
  • additional components: potato starch, lactose monohydrate, crospovidone, povidone (low molecular weight polyvinylpyrrolidone), talc, colloidal silicon dioxide, calcium stearate, polysorbate 20.

Pharmacological properties

Pharmacodynamics

Elzhina is a combined agent of antibacterial, antifungal, anti-inflammatory, antiprotozoal and antipruritic action for local use in gynecology. The effect of the drug is due to the properties of its active ingredients:

  • neomycin: a broad-spectrum antibiotic belonging to the aminoglycoside group; is active against a number of gram-positive and gram-negative aerobic microorganisms, the resistance of which to neomycin is formed slowly and to an insignificant extent; almost not absorbed through the mucous membranes;
  • Ornidazole: antiprotozoal drug, antimicrobial action; effective against Giardia lamblia (Giardia intestinalis), Entamoeba histolytica, Trichomonas vaginalis, some anaerobic bacteria, including Fusobacterium spp., Clostridium spp., Bacteroides spp., as well as anaerobic cocci - Peptostreptococcus spp.; the mechanism of action is associated with the suppression of synthesis and damage to the structure of the DNA of pathogens;
  • prednisolone: a glucocorticosteroid, a dehydrated analog of hydrocortisone; has antiallergic, antipruritic and anti-inflammatory effect; quickly relieves burning and itching;
  • econazole: demonstrates antifungal action against yeast and yeast-like fungi of the genus Candida, dermatophytes Epidermophyton spp., Microsporum spp. and Trichophyton spp. combined with an antibacterial effect against gram-positive bacteria Streptococcus spp. and Staphylococcus spp.; the mechanism of action is due to the suppression of the synthesis of ergosterol, which is a component of the fungal cell membrane; possesses bactericidal and fungicidal properties.

Elzhina prevents the formation, release and manifestation of the activity of inflammatory mediators, such as prostaglandins, kinin, histamine, lysosomal enzymes. The drug blocks the migration of cells to the site of inflammation, reduces vasodilation and increased vascular permeability in the lesion. Due to its vasoconstrictor effect, it reduces exudation.

Pharmacokinetics

With intravaginal administration, there is a slight systemic absorption of the active substances of the drug. The agent is almost not absorbed from the surface of the vaginal mucosa.

Indications for use

  • bacterial vaginitis, excited by the banal pyogenic flora;
  • vulvovaginitis caused by Candida fungi;
  • vaginitis caused by mixed infection.

Contraindications

Absolute:

  • age up to 18 years;
  • disorders of hematopoiesis;
  • organic diseases of the central nervous system (CNS);
  • pregnancy and lactation;
  • hypersensitivity to any of the components that make up the product.

Relative (use the drug with caution):

  • alcoholism;
  • functional disorders of the liver.

Instructions for the use of Elzhina: method and dosage

Elzhin's tablets are used intravaginally.

It is recommended to take 1 tablet daily at bedtime. Lying on your back, the drug should be injected deep into the vagina after hygienic treatment of the external genitalia. Before administration, the tablet should be held for 20-30 seconds in cold boiled water. After the procedure, it is required to be in the supine position for another 10-15 minutes.

The course can be on average 6-9 days. During menstrual bleeding, Elzhina should be used as usual.

If, after a course of drug therapy, there is no improvement in the condition or new symptoms of the disease appear, you should consult your doctor.

Side effects

  • skin and subcutaneous tissues: skin peeling, contact dermatitis, erythema, urticaria, angioedema;
  • general disorders and reactions at the injection site: vaginal dryness (mainly at the beginning of the course), irritation, pain, burning and edema at the injection site.

Overdose

Against the background of intravaginal administration of the drug, an overdose is unlikely.

In case of unintentional oral administration of the drug, symptoms of intoxication can be: diarrhea, vomiting, nausea, tremors, dizziness, convulsions, headache, loss of consciousness. Treatment is symptomatic.

Despite the fact that in the course of clinical studies no undesirable effects on the part of the hematopoietic system were found, for existing diseases of the hematopoietic system, including disorders of hematopoiesis and leukemia (including a history), the drug is contraindicated.

In order to reduce the threat of recurrence of the disease, it is recommended to carry out simultaneous therapy of sexual partners.

In the presence of diseases of the central nervous system, the drug should be used with caution.

During the course of treatment, it is required to refrain from sexual intercourse.

During therapy, cross-allergization and resistance to aminoglycosides may occur.

It is not recommended to use tablets for more than 9 days.

Application during pregnancy and lactation

Elzhina is contraindicated for use during pregnancy and lactation.

Elzhina's use is contraindicated in patients under 18 years of age.

The interaction of the active ingredients of Elzhina during intravaginal use has not been studied. Although the active components of vaginal tablets are characterized by a low degree of systemic absorption, patients receiving acenocoumarol, warfarin and other indirect anticoagulants during the period of therapy should be careful and monitor blood coagulation parameters.
