Presartan is an antihypertensive drug.

Release form and composition

Dosage form of Presartan - film-coated tablets: in a dosage of 25 and 50 mg - round, biconvex, pink, 25 mg tablets with a dividing line on one side; in a dosage of 100 mg - drop-shaped, biconvex, white or almost white with an engraving "100" on one side and "BL" on the other (10 pcs. in a blister, in a cardboard box 3 blisters; 14 pcs. in a blister, in a cardboard box box 2 blisters).

Composition of 1 tablet 25/50 mg:

  • active substance: losartan potassium - 25/50 mg;
  • auxiliary components: dried starch, microcrystalline cellulose, purified talc, colloidal silicon dioxide, sodium starch glycolate, magnesium stearate, isopropyl alcohol, methylene chloride, opad OY-55030, crimson red dye.

Pharmacological properties

Pharmacodynamics

Presartan is a specific angiotensin receptor blocker (type AT1). Does not inhibit kinase II, an enzyme that breaks down bradykinin. Losartan lowers the total peripheral vascular resistance, blood pressure (BP), afterload and pressure in the pulmonary circulation. Reduces the concentration of aldosterone and norepinephrine in the blood. It has a diuretic effect. Has an inhibitory effect on myocardial hypertrophy. Increases resistance to physical activity in patients with chronic heart failure.

The hypotensive effect reaches its maximum in the case of a single dose of the drug after 6 hours, and gradually decreases during the day. After the start of the course of taking Presartan, its maximum effect is achieved on average in a month.

Pharmacokinetics

Presartan is rapidly absorbed from the gastrointestinal tract (GIT). Metabolized by first passage through the liver. The degree of binding to blood plasma proteins of losartan and its metabolites is 92–99%. Bioavailability - 33% (food intake has no effect). The drug practically does not penetrate the blood-brain barrier. It does not accumulate in the body; it is excreted in urine and bile. The half-life of losartan is 2 hours.

Indications for use

  • arterial hypertension and left ventricular hypertrophy (to reduce the risk of developing cardiovascular pathologies and mortality);
  • type II diabetes mellitus with proteinuria (to reduce the risk of developing proteinuria and hypercreatininemia);
  • chronic heart failure [used as part of combination therapy when it is impossible to use angiotensin-converting enzyme (ACE) inhibitors].

Contraindications

Absolute contraindications:

  • severe liver failure ˃ 9 points on the Child-Pugh scale (for 100 mg tablets);
  • pregnancy and lactation;
  • age up to 18 years;
  • increased sensitivity to the components of Presartan.

Relative contraindications (for 100 mg tablets):

  • gout;
  • hyperuricemia;
  • allergic reactions during previous therapy with ACE inhibitors or other drugs;
  • bronchial asthma;
  • systemic blood diseases;
  • reduced volume of circulating blood (BCC);
  • arterial hypotension;
  • co-administration with non-steroidal anti-inflammatory drugs (NSAIDs);
  • cardiac ischemia;
  • elderly age.

Instructions for the use of Presartan: method and dosage

Presartan tablets are taken orally 1 time per day, regardless of the meal.

Dosing according to indications:

  • arterial hypertension: the recommended initial dose is 25 mg / day, the average dose is 50 mg / day, if necessary, it can be increased to 100 mg / day, while taking the drug 2 times a day;
  • heart failure: the recommended initial dose is 12.5 mg / day, the dose is titrated at weekly intervals. The average maintenance dose is 50 mg / day;
  • prevention of cardiovascular pathologies and mortality in patients with arterial hypertension and left ventricular hypertrophy: the recommended initial dose is 50 mg / day, then it is increased to 100 mg / day, or a joint administration of hydrochlorothiazide is prescribed;
  • type II diabetes mellitus with proteinuria: the recommended initial dose is 50 mg / day, then it is increased to 100 mg / day.

Special patient groups:

  • liver failure (˂ 9 points on the Child-Pugh scale), taking high doses of diuretics, hemodialysis, over 75 years of age: the initial dose of the drug should not exceed 25 mg / day;
  • liver dysfunction: lower doses of the drug should be used.

Side effects

Presartan 25 mg and 50 mg are generally well tolerated. Side reactions may occur in the form of diarrhea, dyspepsia, muscle pain, swelling, headache, dizziness, sleep disturbances, hyperkalemia (potassium concentration> 5.5 meq / l); in rare cases - cough, respiratory failure, tachycardia, angioedema (lips, face, pharynx and / or tongue), urticaria, increased activity of liver enzymes, serum bilirubin.

Possible side effects when taking Presartan 100 mg tablets:

  • cardiovascular system: tachycardia, palpitations, epistaxis, dose-dependent orthostatic hypotension, arrhythmias, bradycardia, vasculitis, angina pectoris, myocardial infarction;

  • respiratory system: cough, pharyngitis, bronchitis, nasal congestion, sinusitis, upper respiratory tract infections;
  • genitourinary system: urinary tract infections, urge to urinate, impaired renal function, decreased libido, impotence;
  • other: asthenia, chest pain, increased fatigue, peripheral edema, exacerbation of gout;
  • laboratory parameters: hyperuricemia, increased concentration of urea, residual nitrogen and creatinine in the blood serum, increased activity of hepatic transaminases (moderate), hyperbilirubinemia.

Overdose

Symptoms of an overdose of Presartan: tachycardia and a marked decrease in blood pressure, parasympathetic stimulation can cause bradycardia.

For treatment, forced diuresis and symptomatic therapy are indicated. Hemodialysis is ineffective.
