Hydroxyzin Canon is a tranquilizer (anxiolytic).

Release form and composition

Dosage form - film-coated tablets: white, round, convex on both sides, with a risk; on a cross section almost white (10 and 25 pcs. in blisters, in a cardboard box 1 or 2 packs of 25 tablets or 2, 3 or 5 packs of 10 tablets).

Active ingredient: hydroxyzine hydrochloride, 1 tablet - 25 mg.

Additional components:

  • excipients: mannitol, magnesium stearate, pregelatinized corn starch, microcrystalline cellulose, colloidal silicon dioxide;
  • sheath: white opadry, including hypromellose (hydroxypropyl cellulose), hypromellose (hydroxypropyl methylcellulose), titanium dioxide.

Indications for use

Hydroxyzin Canon is used in the following cases:

  • symptomatic treatment of anxiety in adults;
  • symptomatic therapy of itching of allergic origin;
  • premedication (as a sedative).

Contraindications

  • porphyria;
  • pregnancy and childbirth;
  • lactation;
  • children under 3 years old;
  • hypersensitivity to any of the components of the drug, aminophylline, ethylenediamine, cetirizine and other piperazine derivatives.

Carefully:

  • seizure disorders, including epilepsy;
  • dementia;
  • glaucoma;
  • hyperplasia of the prostate gland with clinical manifestations, accompanied by difficulty urinating and constipation;
  • hyperthyroidism;
  • tendency to arrhythmias, including electrolyte imbalance (hypomagnesemia, hypokalemia);
  • a history of heart disease or concomitant use of drugs that can cause arrhythmias.

Method of administration and dosage

The method of taking Hydroxyzin Canon is oral.

Recommended doses for children, depending on age and indications:

  • symptomatic treatment of allergic itching: 3–6 years - 1–2.5 mg / kg / day in several doses, from 6 years - 1–2 mg / kg / day in several doses;
  • premedication: 1 mg / kg at night on the day before anesthesia.

Recommended doses for adults:

  • symptomatic treatment of anxiety: 50 mg / day - 12.5 mg (½ tab.) in the morning and afternoon, 25 mg in the evening; in severe cases - 50-100 mg 4 times a day;
  • symptomatic therapy of allergic itching: 25 mg at bedtime, if necessary, the frequency of administration is increased up to 3-4 times a day;
  • premedication: 50-200 mg at night on the day before anesthesia.

The maximum single dose for adults is 200 mg (8 tablets), the daily dose is 300 mg (12 tablets).

For elderly people, the doctor selects the dose individually, depending on the presence / absence of concomitant diseases.

In severe and moderate renal failure, the excretion of cetirizine (the main metabolite of hydroxyzine) decreases, therefore, such patients are reduced the daily dose by 2 times.

Patients with hepatic impairment are advised to reduce the daily dose by 33%.

Side effects

Classification of side effects (according to the World Health Organization) depending on the frequency of their development: very often -> 10%, often - from> 1% to 0.1% to 0.01% to <0.1%.

Possible side effects:

  • from the nervous system: infrequently - dizziness, tremors, insomnia; rarely - dyskinesia, convulsions;
  • on the part of the psyche: infrequently - confusion, agitation; rarely - disorientation, hallucinations;
  • from the immune system: rarely - hypersensitivity reactions; very rarely - anaphylactic shock;
  • on the part of the cardiovascular system: rarely - lowering blood pressure, tachycardia; frequency unknown - pirouette-type ventricular tachycardia, prolongation of the QT interval on the electrocardiogram;
  • from the liver and biliary tract: rarely - violation of liver function tests; frequency unknown - hepatitis;
  • from the gastrointestinal tract: infrequently - nausea; rarely - vomiting, constipation;
  • from the respiratory system: very rarely - bronchospasm;
  • on the part of the kidneys and urinary tract: rarely - urinary retention;
  • on the part of the organ of vision: rarely - visual impairment, impaired accommodation;
  • on the part of the skin and subcutaneous tissues: rarely - rash (erythematous, maculopapular), dermatitis, itching, urticaria; very rarely - increased sweating, angioedema, erythema multiforme, Stevens-Johnson syndrome, acute generalized exanthematous pustular rash;
  • others: rarely - malaise, hyperthermia.

The following side effects were observed in patients receiving cetirizine, which is the main metabolite of hydroxyzine: weight gain, enuresis, dysuria, oculogyric crisis, dystonia, asthenia, depression, aggression, paresthesia, tic, thrombocytopenia, edema, diarrhea. Their development is also possible when taking Hydroxyzine Canon.

Overdose symptoms: drowsiness, hyperthermia, nausea, vomiting, impaired pupillary reflex, tachycardia, hallucinations, confusion, tremor, arrhythmia, decreased blood pressure, convulsions, respiratory depression and consciousness, cardiopulmonary collapse, aggravation of coma.

The doctor calculates the dose for children individually, taking into account body weight and in accordance with the dosage recommendations. It should be borne in mind that the minimum dose after dividing the tablet is 12.5 mg.

If you need a methacholine test or allergological tests, you should stop taking the drug for 5 days. Otherwise, you may receive false results.

The dose of Hydroxyzin Canon should be reduced while prescribing drugs that depress the central nervous system (CNS), or drugs with m-anticholinergic properties.

The drug can lengthen the QT interval on an electrocardiogram (ECG). When used in combination with other drugs that can disrupt cardiac activity, the risk of arrhythmias increases. It is assumed that other drugs that cause changes in the ECG (for example, tricyclic antidepressants, antiparkinsonian drugs, phenothiazines, lithium carbonate, atropine, thioridazine, procainamide, quinidine) can enhance and exacerbate the changes caused by hydroxyzine, thereby increasing the risk of sudden death. For this reason, it is recommended to avoid the simultaneous use of two or more drugs that lengthen the QT interval, since additive effects can be noted that can lead to the development of severe and potentially life-threatening arrhythmias.

During treatment, one should refrain from drinking alcoholic beverages, engaging in potentially hazardous activities that require speed of reactions and increased attention.

Drug interactions

Hydroxyzine interferes with the action of cholinesterase and betahistine inhibitors, the anticonvulsant activity of phenytoin and the pressor effect of epinephrine.

When using cimetidine in a daily dose of 1200 mg (600 mg 2 times a day), the concentration of hydroxyzine in the serum increases by 36%, the maximum concentration of the metabolite cetirizine decreases by 20%.

Hydroxyzine can mask the symptoms of ototoxicity caused by drugs with an ototoxic effect (eg, gentamicin).

The potentiating effect of hydroxyzine should be taken into account with the simultaneous use of drugs that depress the central nervous system (for example, tranquilizers, barbiturates, hypnotics, analgesics, ethanol). In these cases, the dose of each drug should be selected individually by the doctor.

Combined use with drugs that have the potential to cause arrhythmias can lead to an increased risk of prolongation of the QT interval and the development of pirouette-type ventricular arrhythmias.

Avoid the combined use of Hydroxyzin Canon with monoamine oxidase inhibitors and anticholinergics.

As an inhibitor of the isoenzyme CYP2D6, hydroxyzine in high doses can interact with CYP2D6 substrates.

Hydroxyzine is metabolized in the liver, therefore, its concentration in blood plasma may increase with the simultaneous use of inhibitors of microsomal liver enzymes.

The concentration of hydroxyzine in blood plasma may increase in the case of the combined use of drugs that potentially inhibit the isoenzyme CYP3A4 / 5, such as delavirdine, ketoconazole, voriconazole, clarithromycin, itraconazole, posaconazole, styripentol, telithromycin, some inhibitors of HIV proteases, atlazinavirinavir, including nelazinavir saquinarin, ritonavir, saquinarine / ritonavir, lopinavir / ritonavir, tipranavir / ritonavir. However, inhibition of one metabolic pathway can be partially compensated by the work of another.
