Etrivex is a topical glucocorticosteroid (GCS).
Release form and composition
The dosage form of Etrivex is 0.05% shampoo: a viscous translucent liquid from colorless to slightly yellow in color with the smell of alcohol (30, 60 and 125 ml in polyethylene bottles, each bottle is packed in a cardboard box).
Composition of 1 g of shampoo:
- active substance: clobetasol propionate - 500 mcg;
- additional components: sodium citrate, sodium lauryl sulfate, citric acid monohydrate, cocobetaine, ethanol 96%, polyquaternium, purified water.
Pharmacological properties
Pharmacodynamics
Clobetasol is a glucocorticosteroid that has anti-inflammatory, vasoconstrictor and antipruritic effects.
The mechanism of anti-inflammatory activity is not fully understood, but it is believed that GCS act by inducing lipocortin proteins, which inhibit phospholipase A 2. This process suppresses the release of arachidonic acid, which is a precursor to powerful inflammatory mediators such as leukotrienes and prostaglandins.
Thus, due to the induction of lipocortins, clobetasol reduces the biosynthesis of the main inflammatory mediators, and, therefore, has anti-inflammatory properties.
Pharmacokinetics
Clobetasol is characterized by low systemic absorption when applied externally, so the likelihood of developing systemic effects during treatment with Etrivex is extremely low. However, the risk of systemic actions is possible when the drug is applied to large areas of the body, especially with impaired skin integrity or skin inflammation.
Clobetasol is metabolized mainly in the liver, to a lesser extent in the kidneys.
It is excreted by the kidneys.
Indications for use
According to the instructions, Etrivex is intended for the treatment and prevention of recurrence of psoriasis of the scalp, with the exception of extensive plaque and pustular psoriasis.
Contraindications
- bacterial, viral, fungal and parasitic infections of the skin;
- violation of the integrity of the skin at the intended site of shampoo application (for example, skin ulceration, wounds, etc.);
- age up to 18 years;
- pregnancy and lactation;
- hypersensitivity to any component of Etrivex.
Instructions for the use of Etrivex: method and dosage
Etrivex shampoo is applied externally only on the scalp.
For medicinal purposes, the shampoo should be applied daily: applied to dry scalp, rubbing thoroughly into the affected areas. Regardless of the number of affected areas, approximately 7.5 ml of shampoo (½ tablespoon) is sufficient for one application. The applied product is left on for 15 minutes, and then washed off with water and washed with regular shampoo. Do not cover your head to enhance the effect of the drug.
The duration of treatment is determined individually, but does not exceed 4 weeks.
After the lesions have cleared, or a small amount of psoriatic scales remains after a 4-week course, shampoo is used 2 times a week to maintain the therapeutic effect and prevent relapses. Maintenance therapy is effective for 6 months.
To prevent exacerbations, repeated treatment courses are carried out using Etrivex shampoo daily. Therapy should be carried out under the supervision of a physician.
Side effects
Etrivex shampoo is generally well tolerated. In some cases (0.1-1%), the following side effects occur:
- from the nervous system: headache;
- on the part of the organ of vision (in case of accidental contact with the eyes): eye irritation;
- on the part of the skin and subcutaneous fat: acne (where the drug is applied), allergic contact dermatitis, skin tightening, folliculitis, discomfort, soreness and swelling of the skin, dryness and irritation of the skin, urticaria, itching, telangiectasia, rash, erythema, exacerbation of psoriasis, alopecia, skin atrophy.
With prolonged daily use of Etrivex, application of shampoo in large quantities and / or on large areas of the body, systemic absorption of clobetasol is possible and, as a consequence, the development of hypercortisolism (Cushing's syndrome) or suppression of the hypothalamic-pituitary-adrenal system, which wears a short-term (transient) and quickly reversible. However, if you follow the recommendations for the use of the drug (apply shampoo for 15 minutes), then systemic absorption is possible only in rare cases.
Intensive and / or prolonged treatment with powerful glucocorticosteroids can lead to local skin atrophy, contact dermatitis, purpura, erythema, telangiectasia, striae. In case of contact with the skin of the face, exacerbation of rosacea, development of perioral dermatitis or skin atrophy is possible.
There are separate reports of the occurrence of pustular rashes and acne, the development of hypertrichosis and changes in pigmentation with the external use of powerful glucocorticosteroids.
Overdose
The development of symptoms of an acute overdose is unlikely, however, if the recommended doses are constantly exceeded or if Etrivex is used incorrectly, signs of hypercortisolism may occur. In this case, the drug is gradually canceled. Due to the risk of suppression of adrenal function, the patient should be under close medical supervision.
special instructions
Like other GCS for external use, clobetasol should be used with caution, including because of the recurrence of the disease after the end of the course of treatment, due to the development of resistance, local and systemic toxicity.
In rare cases, with intensive or prolonged use of GCS, or after their cancellation, an outbreak of generalized pustular psoriasis occurs.
Hypersensitivity reactions to glucocorticosteroids are very rare. The likelihood of their occurrence should be assumed in patients refractory to therapy.
In general, long-term continuous treatment with daily use of GCS, as well as the use of occlusive dressings, is associated with the risk of developing systemic effects. In this case, careful medical supervision and regular examinations of the patient are required in order to identify in time possible suppression of the hypothalamic-pituitary system. Systemic effects usually subside after discontinuation of therapy. However, the withdrawal of the drug should be carried out gradually, since with a sharp withdrawal, the development of acute adrenal insufficiency is possible.
Etrivex Shampoo is intended for use on the scalp only. It should not be applied to other parts of the body, including the face and eyelids, eroded areas of the body, and areas of folded skin (armpits, genital and anal areas), as the risk of unwanted local reactions such as dermatitis increases., telangiectasia and atrophic skin changes.
If shampoo gets in your eyes, rinse immediately with plenty of clean water, as there is a risk of developing cataracts and glaucoma.
Etrivex is recommended for short-term treatment. Repeated short courses can be carried out if necessary.

When applied externally, clobetasol does not adversely affect the patient's ability to concentrate, the speed of his mental and physical reactions.

Etrivex is contraindicated during pregnancy and lactation.
Pediatric use
Etrivex should not be used to treat children and adolescents under 18 years of age.
