Release form and composition
Dosage form Essliala forte - capsules: size No. 0, hard gelatinous, light brown body, dark brown cap; the capsules contain a brown or yellow-brown homogeneous mass with a characteristic odor and consistency from buttery to denser (in a cardboard box 1-6, 9 or 10 blisters of 5, 6 or 10 capsules or 1 can of 10, 20, 30, 40, 50, 60, 90 or 100 capsules).

Essential phospholipids are the main structural elements of cell organelles and the membrane of liver cells. In liver diseases, damage to the membranes of the liver cells and their organelles always occurs, which is the cause of disturbances in the activity of enzymes and receptor systems associated with them, a decrease in the ability to regenerate and a deterioration in the functional activity of liver cells.
The phospholipids included in Esslial forte in their chemical structure correspond to endogenous phospholipids, however, they surpass them in activity, which is associated with a higher content of essential (polyunsaturated) fatty acids. When these high-energy molecules are inserted into the damaged areas of the cell membranes of hepatocytes, the integrity of the liver cells is restored and the ability to regenerate is improved.
Due to the cis-double bonds of polyunsaturated fatty acids, parallel arrangement of hydrocarbon chains in the phospholipids of cell membranes is prevented, and the density of the phospholipid structure of the cell membranes of hepatocytes decreases. This leads to an increase in their elasticity and fluidity, improves metabolism. Due to the formed functional blocks, the activity of enzymes fixed on the membranes increases, which contributes to the normal, physiological pathway of the most important metabolic processes.
Phospholipids, which are part of the drug, transfer neutral fats and cholesterol to oxidation sites, as a result of which the metabolism of lipoproteins is regulated. These processes occur mainly by increasing the ability of high density lipoproteins to form bonds with cholesterol.
When phospholipids are released into bile, its stabilization and a decrease in the lithogenic index are observed.
Esslial forte generally has a normalizing effect: on the detoxification function of the liver and phospholipid-dependent enzyme systems; on the metabolism of proteins and lipids. Ultimately, the drug prevents the formation of connective tissue in the liver and promotes the physiological recovery of liver cells.
Pharmacokinetics
Absorption of taken orally phospholipids (more than 90%) occurs in the small intestine. Most cleaved by phospholipase A 1-acyl lysophosphatidylcholine, 1 / 2 which is immediately converted into polyunsaturated phosphatidylcholine (acetylation by reverse, process occurs even during suction time in the intestinal mucosa). With the flow of lymph, this polyunsaturated phosphatidylcholine enters the bloodstream, then enters the liver, mainly in the form associated with high density lipoproteins.
Studies of pharmacokinetic processes were carried out using dilinoleoyl-phosphatidylcholine. The maximum concentration of the choline component (radioactive label 3 N) is 19.9% of the dose received, it is reached in 6-24 hours. The maximum concentration of the linoleic acid residue (radioactive label 14 C) is up to 27.9%, the time to reach it is from 4 to 12 hours. The half-life is 66 and 32 hours, respectively.

Esslial forte is taken orally, preferably with meals. The capsules must be swallowed whole, without chewing or chewing, with sufficient water (about 200 ml).
Adults and children over 12 years old with a weight of 43 kg are recommended to take 2 capsules 3 times a day.
There are no restrictions on the duration of the course.
