Elset is an anti-allergic drug, blocker of H 1 -histamine receptors.

Release form and composition

Elset is available in the form of film-coated tablets: almost white or white, oblong, biconvex, with rounded ends and a dividing line, the core is white or almost white (7, 10, 14, 28 or 30 pieces in contour cell packs, in a cardboard box 1, 2 or 3 packs).

1 Elcet tablet contains:

  • active substance: levocetirizine dihydrochloride - 5 mg;
  • auxiliary components: calcium hydrogen phosphate dihydrate, lactose monohydrate, microcrystalline cellulose, povidone, sodium starch glycolate (sodium carboxymethyl starch), magnesium stearate;
  • shell composition: hydroxypropyl methylcellulose (hypromellose), titanium dioxide, macrogol 6000 (polyethylene glycol 6000).

Pharmacological properties

Pharmacodynamics

Elset is a drug that is intended to prevent the development and alleviate the manifestations of allergic reactions, has an anti-exudative, antipruritic effect. Its activity is due to the ability of levocetirizine to block H 1 -histamine receptors. Levocetirizine is a competitive histamine antagonist, is the R-enantiomer of cetirizine and has a 2-fold higher affinity for H 1 -histamine receptors than it. Its effect on the histamine-dependent stage of allergic reactions helps to reduce the movement of eosinophils, vascular permeability, and to limit the release of inflammatory mediators and cytokines. Antiserotonin and anticholinergic effects are practically absent, therapeutic doses of levocetirizine have practically no sedative effect.

Pharmacokinetics

Levocetirizine is characterized by a linear change in pharmacokinetic parameters.

When taken orally, it is rapidly and completely absorbed from the gastrointestinal tract. Simultaneous food intake reduces the rate of absorption, but does not affect the completeness of absorption. The maximum concentration (Cmax) of levocetirizine in blood plasma of 207 ng / ml is reached 0.9 hours after a single dose of 5 mg of the drug, after repeated administration in the same dose it is 308 ng / ml.

The binding of levocetirizine to blood plasma proteins is 90%, bioavailability is almost 100%, the volume of distribution is 0.4 l / kg.

Less than 14% of the levocetirizine dose undergoes metabolism; after N- and O-dealkylation, a pharmacologically inactive metabolite is formed. A low metabolic rate and lack of metabolic potential suggests that the drug interacts with other drugs is unlikely.

The half-life (T1 / 2) in adults is approximately 7.9 hours, in young children it is shorter. In adults, the total clearance of the drug is 0.63 ml / min / kg. 85.4% of the taken dose of the drug is excreted unchanged through the kidneys, 12.9% through the intestines.

Levocetirizine is not cumulated, the complete elimination of the drug from the body occurs in 96 hours.

In renal failure with creatinine clearance (CC) less than 40 ml / min, the clearance of the drug decreases, and T1 / 2 is lengthened. During a standard 4-hour hemodialysis procedure, the total clearance is reduced by 80%, less than 10% of the dose of levocetirizine is removed.

Indications for use

  • seasonal allergic rhinoconjunctivitis (hay fever);
  • persistent (year-round) and intermittent (seasonal) allergic rhinitis and allergic conjunctivitis, accompanied by symptoms such as nasal congestion, rhinorrhea, itching, sneezing, lacrimation, conjunctival hyperemia;
  • urticaria (including chronic idiopathic urticaria);
  • itching and rash with allergic skin diseases.

Contraindications

  • severe renal failure (CC less than 10 ml / min), simultaneous use of hemodialysis;
  • lactase deficiency, glucose-galactose malabsorption syndrome, lactose intolerance;
  • period of pregnancy;
  • breast-feeding;
  • age up to 6 years;
  • hypersensitivity to piperazine derivatives;
  • individual intolerance to the components of the drug.

Care should be taken when using Elcet for the treatment of patients with chronic renal failure (CC from 10 to 50 ml / min), in the elderly, in combination with alcohol.

Instructions for the use of Elcet: method and dosage

Elcet tablets are taken orally, swallowed whole and washed down with a sufficient amount of water, before meals or during meals.

Recommended dosage for adults and children over 6 years old: 1 pc. 1 per day.

With normal renal function in elderly patients, a decrease in the daily dose is not required.

In renal insufficiency, the dose of Elcet is prescribed depending on the degree of impairment. The recommended daily dosage for patients with renal and hepatic insufficiency, taking into account the CC value:

  • CC above 50 ml / min: 1 pc.;
  • CC 30–49 ml / min: 1 pc. 1 time in 2 days;
  • QC less than 30 ml / min: 1 pc. 1 time in 3 days;
  • CC less than 10 ml / min (terminal stage, including patients on hemodialysis): Elcet is contraindicated.

If only the liver function is impaired, the general dosage regimen of Elcet is not changed.

For men, CC can be calculated using the serum creatinine concentration. To do this, subtract the patient's age in years from 140, then multiply the difference by his weight in kilograms. The result must be divided by the serum creatinine value (mg / dL) multiplied by 72.

For women, CC is determined in the same way, only the resulting value is additionally multiplied by a coefficient of 0.85.

The duration of the course of treatment depends on the clinical indications; with pollinosis, it can be from 7 to 42 days.

Side effects

  • from the immune system: very rarely - skin rash, urticaria, itching, anaphylaxis, angioedema; frequency not established - persistent drug erythema, hypersensitivity reactions;
  • from the nervous system: infrequently - drowsiness, fatigue, headache; rarely - asthenia; very rarely - agitation, aggression, depression, hallucinations, convulsions; frequency not established - taste disturbance, dizziness, sleep disturbances, insomnia, paresthesia, tremor, fainting, suicidal thoughts;
  • on the part of the cardiovascular system: very rarely - tachycardia; frequency not established - palpitations;
  • from the digestive system: infrequently - dry mouth; rarely - abdominal pain; very rarely - diarrhea, nausea, hepatitis; frequency not established - increased appetite, vomiting, constipation;
  • on the part of the organ of hearing and labyrinthine disorders: frequency not established - vertigo;
  • on the part of the organ of vision: very rarely - visual impairment; frequency not established - diplopia;
  • from the side of metabolism: very rarely - an increase in body weight;
  • from the respiratory system: very rarely - shortness of breath;
  • from the urinary system: the frequency has not been established - dysuria, urinary retention;
  • on the part of the musculoskeletal system: very rarely - myalgia;
  • laboratory indicators: very rarely - violation of liver function tests;
  • other: frequency not established - peripheral edema.

Overdose

Symptoms: in adults - drowsiness, in children - anxiety, agitation, alternating with drowsiness.

Treatment: there is no specific antidote, so you should immediately rinse the stomach, take activated charcoal, and, if necessary, seek symptomatic and supportive therapy. Hemodialysis is ineffective.

During the period of Elcet's use, it is recommended to avoid potentially hazardous activities, the performance of which depends on an increased concentration of attention and the speed of psychomotor reactions, including driving.

Application during pregnancy and lactation

According to the instructions, Elset is contraindicated to use during gestation and breastfeeding.

If necessary, the appointment of levocetirizine during lactation should stop breastfeeding.

Pediatric use

Elcet is contraindicated for the treatment of children under 6 years of age.

With impaired renal function

Elcet is contraindicated in patients with severe renal insufficiency (CC less than 10 ml / min) and patients on hemodialysis.

Caution should be exercised in case of renal failure and the dose of the drug should be determined taking into account the CC value. For patients with renal and hepatic impairment, the following daily dosage is recommended:

  • CC above 50 ml / min: 1 pc.;

Drug interactions

The combination of Elcet with theophylline in a daily dose of 400 mg causes a decrease in the total clearance of racemic cetirizine by 16%, while the kinetic properties of theophylline do not change.

With the simultaneous use of alcoholic beverages (ethanol) or taking medications with a depressing effect on the central nervous system (CNS), their effect on the central nervous system may increase.
