Alprostan is a prostaglandin E 1 drug that has a vasodilating and angioprotective effect, which improves peripheral circulation.

Release form and composition

Dosage form - concentrate for the preparation of solution for infusion: transparent colorless solution (0.2 ml in ampoules, 5 ampoules in PVC containers, 2 containers in a cardboard box).

1 ampoule contains:

  • Active ingredient: alprostadil - 100 mcg;
  • Auxiliary component: ethanol.

Indications for use

Alprostan is a remedy for the treatment of chronic obliterating arterial diseases of the III-IV stage (in accordance with the Fontaine classification).

Contraindications

Absolute:

  • Unstable or severe angina;
  • Decompensated chronic heart failure, heart rhythm disturbances;
  • Acute and subacute myocardial infarction;
  • Severe liver dysfunction;
  • Pulmonary edema, severe broncho-obstructive syndrome with signs of respiratory failure, infiltrative changes in the lungs, confirmed by clinical and X-ray examination;
  • Hyperhydration, oliguria, cerebral edema;
  • Diseases accompanied by the risk of bleeding (severe vascular damage to the brain, stomach or duodenal ulcer, extensive trauma, proliferative retinopathy with a tendency to bleeding, etc.);
  • Pregnancy and lactation period;
  • Age under 18 and over 75;
  • The simultaneous use of vasodilators and / or anticoagulants;
  • Hypersensitivity to Alprostan's components.

Relative (use with caution):

  • Cardiovascular insufficiency;
  • Arterial hypotension;
  • Type 1 diabetes mellitus, diabetic angiopathy;
  • Hemodialysis.

Method of administration and dosage

Alprostan is administered intravenously. Immediately before administration, an infusion solution is prepared from the lyophilisate.

Recommended dilution: dissolve the contents of 1 ampoule with lyophilisate (0.2 ml) in 9.8 ml of solvent (5% or 10% glucose solution, physiological solution). After dilution, the concentration of alprostadil in 10 ml of solution is 100 μg. The resulting solution, if necessary, can be diluted in a larger volume of solvent.

Adult patients are prescribed 50-200 mcg 1 time per day. In severe cases, the dose is increased to 50-100 mcg 2 times a day. The lyophilisate is usually dissolved in 200-500 ml. The duration of the infusion is at least 2 hours.

The average duration of treatment is 14 days. In case of a positive effect, therapy is continued for another 7-14 days. The total duration of the drug should not exceed 28 days.

If there is no effect during the first 14 days of treatment, Alprostan is canceled.

For patients with impaired renal function (serum creatinine concentration greater than 1.5 ml / dl), the initial dose is 20 μg. After 2-3 days, if necessary, the dose is increased to 40-60 mcg.

With heart and renal failure, the maximum volume of injected fluid is 50-100 ml per day.

The duration of treatment for these categories of patients is 28 days.

Side effects

  • From the side of the cardiovascular system: cardiac arrhythmias, tachycardia, decreased blood pressure, angina attacks;
  • From the side of the musculoskeletal system: with prolonged therapy (from 4 weeks) - hyperostosis of long bones (reversible);
  • From the digestive system: nausea, epigastric discomfort, diarrhea, vomiting;
  • From the urinary system: the presence of blood in the urine (hematuria);
  • From the nervous system: dizziness, headache;
  • Allergic reactions: itching, skin rash;
  • Laboratory indicators: leukocytosis, hyperbilirubinemia, leukopenia, transient increase in the activity of hepatic transaminases in the blood serum; in individual cases - an increase in the titer of C-reactive protein;
  • Others: malaise, increased fatigue, hyperthermia, hyperhydration, swelling of the limb, into the vein of which the drug is infused;
  • Local reactions: signs of phlebitis proximal to the injection site (disappear a few hours after the end of the infusion or change the injection site, no specific treatment is required; the frequency of this side effect can be reduced by catheterization of the central vein).

The number of adverse reactions depends on the volume, concentration and rate of infusion.

Alprostan should be used only by medical specialists with experience in angiology, familiar with modern methods of continuous monitoring of the state of the cardiovascular system and having the appropriate equipment for this.

During the period of treatment, it is necessary to monitor the hemodynamic parameters of the acid-base balance, the biochemical parameters of the blood and the blood coagulation system (in patients with disorders of the blood coagulation system or receiving simultaneous therapy with drugs that affect the hemocoagulation system).

In case of side effects, it is recommended to reduce the dose of the drug.

Patients with renal failure and coronary heart disease during treatment and within 1 day after its completion should be in the hospital under the supervision of a doctor. In order to avoid the development of overhydration, it is desirable that the volume of injected fluid in such patients does not exceed 50-100 ml per day. Patients should be carefully monitored: blood pressure and heart rate, if necessary, body weight, fluid balance, central venous pressure, or echocardiography. In newborns, Alprostan should be used under constant monitoring of blood pressure, if conditions are available for mechanical ventilation.

Treatment of patients on hemodialysis should be carried out in the post-dialysis period.

In patients with chronic obliterating diseases of the arteries of the lower extremities, the use of Alprostan is part of a complex therapy, the clinical effect of the treatment is long-lasting and may appear with a certain delay after the end of the therapeutic course.

Alprostane can cause systemic effects in the form of dizziness, weakness or hypotension, which usually disappear soon after the cessation of infusions, so patients should be careful when driving and performing activities that require increased speed of psychomotor reactions and concentration.

Drug interactions

Alprostadil enhances the effect of vasodilators and antihypertensive agents.

With the simultaneous use of antiplatelet and anticoagulant drugs, thrombolytics, cefamandol, cefotetan, cefoperazone, the risk of bleeding increases.

Adrenomimetics (epinephrine, norepinephrine) reduce the vasodilating effect of the drug.
