Asterisk Flew is a combined drug for eliminating symptoms of acute respiratory infections (ARI) and colds.

Release form and composition

The preparation is produced in the form of a powder for the preparation of a solution for oral administration (orange / raspberry / lemon): granular, homogeneous, easily free-flowing; with orange flavor - granules from pale orange to white with a slight orange scent; with raspberry flavor - granules from pale pink to white with a slight raspberry smell; with lemon flavor - pale yellow to white granules with a slight lemon scent; the reconstituted solution is almost transparent or transparent, with orange flavor - pale orange, with raspberry flavor - pale raspberry color, with lemon flavor - pale yellow color (15 g of powder in a sachet made of combined material - aluminum foil, polyethylene; c cardboard box of 5 or 10 sachets and instructions for use of the Asterisk Fly).

Pharmacological properties

Pharmacodynamics

Asterisk Flew is a combined remedy used to eliminate the symptoms of acute respiratory infections and colds, which includes: H 1 -histamine receptor blocker + non-narcotic analgesic agent + α-adrenergic agonist + vitamin.

Thanks to the active ingredients, the drug demonstrates antipyretic, analgesic, mild anti-inflammatory, vasoconstrictor, anti-edema and anti-allergic effects.

Pharmacological properties of active substances that make up Asterisk Flu:

  • pheniramine: blocker of H 1 -histamine receptors, antiallergic agent, reduces lacrimation and rhinorrhea, relieves spastic phenomena; to a moderate degree shows a sedative effect and has an M-anticholinergic (antimuscarinic) effect;
  • paracetamol: a non-narcotic analgesic, by inhibiting the production of prostaglandins in the central nervous system (CNS), it provides anesthetic and antipyretic effect; does not affect hemostasis and platelet function;
  • phenylephrine: a sympathomimetic agent (adrenergic agonist), when used topically, demonstrates a moderate vasoconstrictor effect as a result of stimulation of α 1 -adrenergic receptors; reduces hyperemia and edema of the nasal mucosa;
  • ascorbic acid (vitamin C): helps to increase the body's resistance to infections, reduce vascular permeability; takes part in the regulation of carbohydrate metabolism, the course of redox reactions, blood clotting, tissue restoration, the production of steroid hormones; helps to improve the tolerance of paracetamol and lengthen its action, which is associated with an increase in the half-life (T 1/2).

Pharmacokinetics

The maximum concentration (C max) of pheniramine in plasma is observed on average after 1–2.5 hours after administration, T 1/2 is 16–19 hours; approximately 70–83% of the administered dose is eliminated from the body in the urine unchanged or in the form of metabolites.

Paracetamol is intensively and almost completely absorbed from the gastrointestinal tract (GIT); after oral administration, C max in blood plasma is observed after 10-60 minutes. The active substance is distributed in most body tissues, passes through the placenta, and is detected in breast milk. When used in therapeutic concentrations, the binding of the agent with plasma proteins is fixed to an insignificant extent, with increasing concentration it increases. Paracetamol is metabolized in the liver, eliminated mainly in the urine in the form of sulfate and glucuronide compounds, T 1/2 - 1-3 hours.

Phenylephrine is absorbed from the gastrointestinal tract and undergoes primary metabolic transformation in the intestine and liver. The active substance is excreted in the urine almost 100% in the form of sulfate compounds, C max in plasma after oral administration is observed in the range from 45 minutes to 2 hours, T 1/2 - 2-3 hours.

Ascorbic acid is characterized by rapid and complete absorption in the gastrointestinal tract. It binds to plasma proteins by 25%, in case of overdose it is excreted in the urine in the form of metabolites.

Indications for use

The Flew asterisk is recommended for the symptomatic treatment of infectious and inflammatory diseases, including acute respiratory viral infections (ARVI), including influenza, accompanied by such phenomena as fever, body aches, chills, headache and muscle pain, nasal congestion, runny nose, sneeze.

Contraindications

Absolute:

  • arterial hypertension;
  • severe cardiovascular lesions;
  • portal hypertension;
  • pheochromocytoma;
  • deficiency of sucrase / isomaltase, glucose-galactose malabsorption, fructose intolerance (the product contains sucrose);
  • angle-closure glaucoma;
  • diabetes;
  • hyperthyroidism;
  • alcoholism (due to increased hepatotoxicity of paracetamol);
  • phenylketonuria (Asterisk Flu includes aspartame);
  • concomitant therapy with monoamine oxidase (MAO) inhibitors or their use for two weeks before starting the intake of Zvezdochka Flu (the effect of MAO inhibitors increases, the threat of hypertensive crisis increases);
  • combined use with tricyclic antidepressants, β-blockers or other sympathomimetic drugs (the risk of adverse reactions from the cardiovascular system is aggravated);
  • age up to 12 years;
  • pregnancy and lactation;
  • hypersensitivity to any of the constituents of the drug.

Relative (requires the use of Asterisk Flu powder with caution):

  • cardiovascular diseases;
  • severe atherosclerotic lesions of the coronary arteries;
  • severe liver and / or kidney damage;
  • acute hepatitis;
  • congenital hyperbilirubinemia (Dubin-Johnson, Gilbert and Rotor syndromes);
  • difficulty urinating due to prostatic hypertrophy, prostatic hyperplasia;
  • deficiency of glucose-6-phosphate dehydrogenase;
  • blood diseases, hemolytic anemia;
  • bronchial asthma;
  • stenosing ulcer of the stomach and / or 12 duodenal ulcer, pyloroduodenal obstruction;
  • epilepsy;
  • exhaustion, dehydration;
  • simultaneous use of drugs that can have a negative effect on the liver (inducers of liver microsomal enzymes).

Asterisk Flew, instructions for use: method and dosage

A solution prepared from Asterisk Fly powder is taken orally.

The contents of 1 sachet must be diluted in 1 glass of hot boiled water. The resulting solution should be taken hot, you can also add sugar to taste. Subsequent doses may be taken every 4 hours, but no more than 4 doses over a 24 hour period.

The Fleu asterisk can be used at any time of the day, but for maximum effect it is recommended to drink the solution before bedtime, at night.

If within 3 days after the start of the course there is no decrease in the severity of the symptoms of the disease, it is necessary to consult a doctor. The drug should not be treated for more than 5 days.

Side effects

  • immune system: rarely - hypersensitivity reactions (shortness of breath, skin rash, anaphylactic shock), angioedema; with an unknown frequency - anaphylactic reaction, toxic epidermal necrolysis, Stevens-Johnson syndrome;
  • nervous system: often - drowsiness; rarely - headache, dizziness;
  • mental disorders: rarely - sleep disturbance, increased excitability;
  • blood and lymphatic system: extremely rarely - leukopenia, pancytopenia, thrombocytopenia, agranulocytosis;
  • organ of vision: rarely - mydriasis, increased intraocular pressure, paresis of accommodation;
  • hepatobiliary system: rarely - increased activity of hepatic enzymes;
  • digestive tract: often - nausea, vomiting; rarely, abdominal pain, dry mouth, constipation / diarrhea;
  • cardiovascular system: rarely - palpitations, increased blood pressure (BP), tachycardia;
  • urinary system: rarely - difficulty urinating;
  • skin and subcutaneous tissues: rarely - skin rash, itching, urticaria, erythema;
  • general disorders: rarely - malaise.

Overdose

Paracetamol

Symptoms of intoxication caused by paracetamol occur, as a rule, after using the drug in a dose exceeding 10-15 g. In severe cases, hepatotoxic effects may occur, including the development of liver necrosis. Also, as a result of an overdose, nephropathy and irreversible liver damage may develop. The severity of symptoms depends on the dose, therefore, it is forbidden to use other paracetamol-containing agents simultaneously with the Asterisk Flu. The risk of intoxication is aggravated in the elderly, children, patients with liver damage, in malnourished patients or receiving inducers of microsomal liver enzymes, as well as in the presence of chronic alcoholism. An overdose of paracetamol can cause liver failure, encephalopathy, coma, and lead to death.

In the first 24 hours, symptoms of an overdose may be: nausea, vomiting, pallor of the skin, anorexia, convulsions. Pain in the abdomen is evidence of liver damage and, as a rule, does not occur for 24–48 hours, and in some cases is recorded much later, after 4–6 days. To the maximum extent, signs of liver damage usually appear within 72–96 hours after taking the drug. Disorders of glucose metabolism and metabolic acidosis are also possible. In the absence of liver damage, acute tubular necrosis and acute renal failure may nevertheless occur. Cases of cardiac arrhythmia and pancreatitis have been reported.

Within 48 hours after an overdose of paracetamol, oral administration or intravenous (iv) injections of acetylcysteine as an antidote, gastric lavage procedure, oral administration of methionine may have a positive effect. They also recommend taking activated charcoal, and monitoring respiration and blood circulation, with seizures, diazepam is prescribed.

Pheniramine and phenylephrine

Overdose symptoms for these two active components are combined due to the threat of mutual enhancement of the sympathomimetic action of phenylephrine and the parasympatholytic action of pheniramine in case of an overdose of Asterisk Flew.

These symptoms may include the following effects: headache, drowsiness, with the further appearance of anxiety (mainly in children), dizziness, nausea, vomiting, skin rash, visual disturbances, insomnia, hyperexcitability, circulatory disorders, behavior changes, increase / decrease in blood pressure, bradycardia, convulsions, coma. Against the background of an overdose of pheniramine, cases of atropine-like psychosis were recorded.

There is no specific antidote. It is recommended to take activated charcoal, saline laxatives, the implementation of measures aimed at maintaining cardiac and respiratory functions. Due to the increased risk of seizures, psychostimulants (methylphenidate) are not prescribed.

When arterial hypotension occurs, it is possible to use vasopressor drugs. With an increase in blood pressure, intravenous administration of α-adrenergic blockers can be performed, since phenylephrine belongs to the selective agonists of α 1 -adrenergic receptors. In case of phenylephrine overdose, the hypertensive effect must be eliminated by blocking α 1 -adrenergic receptors. If seizures develop, diazepam is used.

special instructions

Paracetamol can modify the results of a laboratory determination of uric acid using a precipitating agent.

During the period of therapy, it is necessary to abandon the use of ethanol-containing drinks due to the increased threat of their hepatotoxic action.

Due to the presence of mannitol in Zvezdochka Flew, a slight laxative effect is possible.

Do not use powder if damaged in sachets.

In case of development of bronchial asthma (attacks of shortness of breath), emphysema or chronic bronchitis during therapy with the drug, as well as in the absence of improvement in the condition within 5 days of admission, or when observing severe fever for more than 3 days, skin rash or persistent headache, you need to see a doctor. These reactions may indicate the development of more serious diseases.

Influence on the ability to drive vehicles and complex mechanisms

Since Asterisk Flew may cause drowsiness during the treatment period, you should refrain from driving vehicles and operating any other complex or moving machinery.

Application during pregnancy and lactation

Reception of Zvezdochka Flu in pregnant and lactating women is contraindicated due to the lack of data confirming the safety of treatment with the drug in this group of patients.

Pediatric use

For children under 12 years of age, the appointment of Asterisk Flu is contraindicated.

With impaired renal function

In the presence of severe renal failure (creatinine clearance below 10 ml / min), the interval between doses of Asterisk Flew should be at least 8 hours.

For violations of liver function

Patients with impaired liver function or Gilbert's syndrome should reduce the dose or increase the period between doses of Asterisk Flew.

Drug interactions

  • carbamazepine, phenytoin, rifampicin, barbiturates (phenobarbital), zidovudine, isoniazid, other inducers of liver microsomal enzymes: aggravate the threat of the hepatotoxic effect of paracetamol;
  • sedatives, MAO inhibitors, ethanol: Asterisk Flu enhances the effects of these agents;
  • warfarin and other coumarin derivatives: the anticoagulant effect of these drugs increases and the threat of bleeding increases with prolonged use of paracetamol;
  • domperidone, metoclopramide: there is an increase in the rate of absorption of paracetamol and a faster achievement of its C max in plasma;
  • lamotrigine: the bioavailability of this substance decreases (by inducing hepatic metabolism) and its effect is weakened;
  • chloramphenicol: its T 1/2 increases under the influence of paracetamol;
  • cholestyramine: reduces the absorption of paracetamol, therefore, it is recommended to take Asterisk Flu 1 hour before using cholestyramine;
  • barbiturates, tricyclic antidepressants, MAO inhibitors, ethanol, antiparkinsonian drugs, narcotic drugs, tranquilizers: the effect of these drugs on the central nervous system is enhanced by the action of pheniramine; ethanol increases the sedative effect of pheniramine;
  • zidovudine: possible development of neutropenia and an increase in the threat of liver damage against the background of regular treatment with paracetamol;
  • probenecid: enhances the metabolism of paracetamol, with this combination it is recommended to reduce the dose of paracetamol;
  • anticoagulants: their effect is suppressed by pheniramine;
  • guanethidine, reserpine, methyldopa: the risk of undesirable effects from the cardiovascular system, including increased blood pressure, is aggravated;
  • digoxin and other cardiac glycosides: the risk of arrhythmia or myocardial infarction increases;
  • ergot alkaloids (metizergide, ergotamine): the threat of ergotism may increase.
