Platifillin with papaverine is a combined antispasmodic drug that has myotropic and m-anticholinergic effects.

Release form and composition

The drug is produced in the form of tablets: round, flat-cylindrical, white, with a chamfer and a dividing line (10 pcs. In blisters, 1 package in a cardboard box and instructions for the use of Platyphyllin with papaverine).

1 tablet contains:

  • active ingredients: platifillin hydrotartrate - 5 mg; papaverine hydrochloride - 20 mg;
  • auxiliary components: potato starch, refined sugar (sucrose), calcium stearate, talc.

Pharmacological properties

Pharmacodynamics

Platifillin with papaverine is a combined antispasmodic drug, the action of which is due to the following properties of the active ingredients:

  • papaverine: a myotropic antispasmodic, by reducing the tone of the smooth muscles of internal organs, including blood vessels, the gastrointestinal tract (GIT), the respiratory and genitourinary system, helps to relax them. Has a hypotensive effect, inhibits PDE (phosphodiesterase);
  • Platyphylline: an m-cholinergic blocker that blocks acetylcholine muscarinic receptors (m-cholinergic receptors) by disrupting the transmission of impulses from postganglionic cholinergic nerves to effector organs and tissues (smooth muscle organs, heart, glands of external secretion) innervated by them. Penetrates the BBB (blood-brain barrier). As a result of a decrease in the influence of the vagus nerve (nervus vagus), atrioventricular conduction improves, myocardial excitability increases, heart rate (HR) and minute blood volume increase. The direct myotropic antispasmodic effect of platifillin promotes the expansion of small vessels of the skin, a decrease in smooth muscle tone, the amplitude and frequency of peristaltic contractions of the muscles of the stomach, small and large intestines, and duodenum. With hyperkinesia of the biliary tract, it causes a moderate decrease in the tone of the gallbladder, with hypokinesia, it increases the tone of the gallbladder to a normal level. By relaxing the smooth muscles of the bladder, urinary tract and uterus, it helps to relieve pain against the background of their spasm. The relaxing effect on the smooth muscles of the bronchi increases the volume of respiration and inhibits the secretion of the bronchial glands (including bronchoconstriction caused by anticholinergics or an increase in the tone of the vagus nerve). The relaxing effect on the smooth muscles of the bronchi increases the volume of respiration and inhibits the secretion of the bronchial glands (including bronchoconstriction caused by anticholinergics or an increase in the tone of the vagus nerve). The relaxing effect on the smooth muscles of the bronchi increases the volume of respiration and inhibits the secretion of the bronchial glands (including bronchoconstriction caused by anticholinergics or an increase in the tone of the vagus nerve).

Pharmacokinetics

Data on the pharmacokinetics of Platyphyllinum with papaverine are not provided by the manufacturer.

Indications for use

The use of Platifillin with papaverine is indicated for the following diseases:

  • pathologies accompanied by spasm of smooth muscles of the abdominal organs - intestinal colic, cholecystitis, spastic colitis, pylorospasm, cholelithiasis, biliary colic, renal colic;
  • peptic ulcer of the stomach and duodenum;
  • spasm of smooth muscles of the walls of blood vessels of the brain;
  • angina pectoris - as part of complex therapy;
  • endarteritis;
  • bronchospasm.

Contraindications

An absolute contraindication to the use of Platifillin with papaverine is an individual intolerance to the components that make up it.

It is recommended to use the combined drug with caution in the following concomitant conditions and diseases:

  • cardiovascular pathologies for which an increase in heart rate is undesirable: arterial hypertension, chronic heart failure, tachycardia, atrial fibrillation, ischemic heart disease, mitral stenosis, acute bleeding;
  • reflux esophagitis and / or combined with reflux esophagitis hiatal hernia: dysfunction of the sphincter can reduce the motility of the esophagus and stomach, and relax the lower esophageal sphincter, possibly slowing down the evacuation of gastric contents and increased gastroesophageal reflux;
  • hyperthermia: Platyphyllin with papaverine suppresses the activity of the sweat glands, therefore, therapy at elevated body temperature can contribute to its increase;
  • achalasia, paralytic intestinal obstruction, pyloric stenosis, intestinal atony, especially in elderly or debilitated patients: in diseases of the gastrointestinal tract, accompanied by obstruction, a decrease in motility and muscle tone of the digestive tract is likely, which can cause obstruction and retention of stomach contents;
  • thyrotoxicosis: promotes increased tachycardia;
  • ulcerative colitis (ulcerative colitis): the use of high doses of Platifillin with papaverine in patients with ulcerative colitis causes inhibition of intestinal motility, increases the risk of paralytic intestinal obstruction or toxic megacolon;
  • increased intraocular pressure: with closed-angle glaucoma, it can contribute to the development of the mydriatic effect with a further increase in fluid pressure inside the eye, up to an acute attack of the disease; with open-angle glaucoma, the mydriatic effect contributes to a slight increase in intraocular pressure, at which correction of therapy will be necessary;
  • xerostomia: long-term treatment may cause a further increase in the severity of dryness of the oral mucosa;
  • hepatic and / or renal failure;
  • chronic lung diseases (especially in debilitated patients or children): taking Platifillin with papaverine can cause a decrease in bronchial secretion, thickening of secretions and the formation of congestion in the bronchi;
  • myasthenia gravis: worsening of the condition is possible due to inhibition of the effect of acetylcholine;
  • autonomic (autonomic) neuropathy: the likelihood of urinary retention and accommodation paralysis increases;
  • hypertrophy of the prostate gland without obstruction of the urinary tract, urinary retention or the presence of factors predisposing to it, other diseases accompanied by obstruction of the urinary tract (including obstruction of the bladder neck as a result of prostatic hypertrophy);
  • gestosis: therapy for this complication of pregnancy can increase arterial hypertension;
  • Down's disease: while taking pills, there is an unusual dilation of the pupils and an increase in heart rate;
  • brain damage in pediatric patients: reactions from the central nervous system may increase;
  • central paralysis in pediatric patients: an increase in the severity of the reaction of children to anticholinergic drugs is likely;
  • age over 40: at this age, the risk of manifestation of undiagnosed glaucoma increases;
  • the period of pregnancy and lactation.

Platyphyllin with papaverine, instructions for use: method and dosage

Platyphyllin tablets with papaverine are taken orally.

Recommended dosage: 1 pc. 2-3 times a day.

The duration of the course of treatment is determined by the doctor, taking into account clinical indications.

Side effects

Against the background of the use of Platifillin with papaverine, the following undesirable phenomena may appear: the development of allergic reactions, a decrease in blood pressure (BP), drowsiness, dizziness, headache, dryness of the oral mucosa, thirst, intestinal atony, mydriasis, accommodation paralysis, photophobia, tachycardia, delay urine, lung atelectasis, ventricular extrasystole, atrioventricular block. Changes in laboratory results - eosinophilia, increased activity of hepatic transaminases.

Overdose

Symptoms: weakness, drowsiness, diplopia, hyperthermia, dryness of the mucous membranes of the mouth, nose and throat, difficulty in swallowing and speaking, agitation, suppression of the activity of the respiratory and vasomotor centers, paralytic intestinal obstruction, paralysis of accommodation, glaucoma, mydriasis, tremor, convulsions, depression nervous system, in patients with prostatic hyperplasia - acute urinary retention.

Treatment: immediate gastric lavage. Parenteral administration of cholinesterase inhibitors and cholinostimulants is shown. Appointment of symptomatic therapy: with hyperthermia - taking antipyretic drugs and wet rubdown; when excited - sodium thiopental (intravenous) or chloral hydrate (rectally); with mydriasis - physostigmine, pilocarpine (in the form of eye drops). In case of an attack of glaucoma, it is recommended to use 1% pilocarpine solution (2 drops in the conjunctival sac with an interval of 1 hour) and 0.05% neostigmine methyl sulfate solution (1 ml subcutaneously 3-4 times a day).

special instructions

Before taking Platyphyllin with papaverine, a doctor's consultation is required, especially for patients with concomitant pathologies.

Influence on the ability to drive vehicles and complex mechanisms

During the period of application of Platyphyllinum with papaverine, patients should avoid all potentially hazardous activities, the implementation of which requires an increased concentration of attention and speed of psychomotor reactions, including driving vehicles and complex mechanisms.

Application during pregnancy and lactation

It is recommended to be careful if you need to take Platyphyllin with papaverine during pregnancy or lactation.

In women with gestosis against the background of the use of the drug, an increase in arterial hypertension is possible.

Pediatric use

With caution, it is recommended to use Platyphyllin tablets with papaverine for the treatment of young children with chronic lung diseases, brain damage, central paralysis.
