Thioridazine is a neuroleptic with antipsychotic, antidepressant, tranquilizing, antiemetic and antipruritic effects.

Release form and composition

Dosage form - coated tablets: round, convex on both sides, from light pink to pink (10 mg tablets) or from light yellow to yellow with a greenish tinge (25 mg tablets); the cross section shows 2 layers (60 pcs. in polymer cans, 1 can in a cardboard box).

Active ingredient: thioridazine hydrochloride, in 1 tablet - 10 or 25 mg.

Auxiliary components: gelatin, calcium stearate, basic magnesium carbonate, low molecular weight medical polyvinylpyrrolidone (povidone), lactose monohydrate, colloidal silicon dioxide (aerosil), apiary wax, titanium dioxide pigment, sugar, potato starch, talc in tablets, as well as dye (mg - acid red 2C, 20 mg tablets - yellow water-soluble KF-6001).

Indications for use

neuroses, accompanied by sleep disturbances, depressed mood, tension, anxiety, obsessive states, excitement, fear;
psychomotor agitation of various origins;
psychotic disorders, accompanied by hyperreactivity and agitation;
affective insanity;
tics, Huntington's disease;
severe behavioral disorders, manifested by aggressiveness, inability to long-term concentration of attention;
schizophrenia;
withdrawal symptoms (substance abuse, alcoholism);
skin diseases occurring with severe and painful itching;
moderate to severe depression in adults;
behavioral disorders with increased psychomotor activity in children.

Contraindications

Absolute:

traumatic brain injury;
acute depressive conditions;
progressive systemic diseases of the brain and spinal cord;
porphyria;
pheochromocytoma;
blood diseases (hypo- and aplastic processes);
severe arterial hypotension;
severe cardiovascular insufficiency;
coma of any etiology;
children under 4 years of age;
pregnancy and lactation;
hypersensitivity to any component of the drug.

Method of administration and dosage

Thioridazine should be taken orally.

Average therapeutic doses for adults:

acute psychosis, mania, schizophrenia, agitated states: the initial daily dose is 25–75 mg, within 7 days it is gradually increased to the optimal therapeutic dose, which is 150–400 mg on an outpatient basis, and 250–800 mg in the hospital. Improvement is noted in 10-14 days, but treatment lasts several weeks. Maintenance daily dose - 75-200 mg 1 time per day (before bedtime). Elderly people are prescribed lower doses - 30–100 mg per day;
emotional disorders: mild - 30–75 mg per day, moderate - 50–200 mg per day. Start therapy with a low dose, gradually increasing it to an effective therapeutic dose;
psychosomatic disorders: 10–75 mg per day;
severe itching: up to 200 mg per day.

The specific dose is determined only by the doctor individually for each patient, depending on the clinical situation.

Recommended doses for children:

15-18 years - 30-50 mg 3 times a day;
8-14 years old - 20-30 mg 3 times a day;
4-7 years - 10-20 mg 2-3 times a day.

Thioridazine should be discontinued gradually.

Side effects

allergic reactions: nasal congestion, rash, erythema, exfoliative dermatitis, bronchospastic syndrome, angioedema;
from the digestive system: nausea, increased appetite, hyposalivation, hypertrophy of the papillae of the tongue, diarrhea, anorexia, increased body weight, intestinal obstruction, vomiting; rarely - cholestatic hepatitis;
on the part of the reproductive system: false positive pregnancy tests, dysmenorrhea, gynecomastia, hyperprolactinemia, impaired ejaculation, priapism, decreased libido;
from the urinary system: dysuria, paradoxical ischuria;
on the part of the cardiovascular system: tachycardia, hypotension;

Drug interactions

general anesthetics, barbiturates, ethanol, opioids, atropine: synergistic action is noted;
anticoagulants: their effect decreases;
sympathomimetics: the likelihood of developing an arrhythmogenic effect increases;
levodopa: its antiparkinsonian effect decreases;
cimetidine, anticonvulsants: the effectiveness of thioridazine decreases;
prolactin: its concentration in plasma increases;
antihypertensive drugs: their effect increases, the risk of orthostatic hypotension increases;
bromocriptine: its effect decreases;
quinidine: the cardiodepressant effect of thioridazine is potentiated;
antithyroid drugs: the likelihood of developing agranulocytosis increases;
guanethidine: its antihypertensive effect decreases;
maprotiline, monoamine oxidase inhibitors, tricyclic antidepressants, antihistamines: anticholinergic and sedative effects may increase;
amphetamine: antagonistic effect is noted;
beta-blockers: the hypotensive effect increases, the risk of arrhythmia, tardive dyskinesia and irreversible retinopathy increases;
appetite suppressants (except fenfluramine): their effectiveness decreases;
disopyramide, astemizole, pimozide, erythromycin, quinidine, procainamide, cisapride, probucol: additional lengthening of the QT interval is noted;
antidiabetic drugs: their hepatotoxic effect is enhanced;
epinephrine (adrenaline), ephedrine: a sudden and pronounced decrease in blood pressure is possible;
apomorphine: its emetic effect decreases, the inhibitory effect on the central nervous system increases;
thiazide diuretics: hyponatremia may increase;
lithium preparations: their absorption from the gastrointestinal tract decreases, the rate of excretion of lithium ions by the kidneys increases, the severity of extrapyramidal disorders increases (while such early signs of lithium intoxication as nausea and vomiting can be masked by the antiemetic effect of thioridazine).
