Texamen is a drug with anti-inflammatory, analgesic and antipyretic effects.

Release form and composition

Dosage forms of Texamena:

  • film-coated tablets: yellow with a brown tint, oval biconvex, on one side with a line; the core in the cross section is bright yellow (in a cardboard box, 1 blister with 10 tab.);
  • lyophilisate for the preparation of a solution for intravenous and intramuscular administration: yellow with a greenish tinge lyophilized powder (in a cardboard box 1 bottle with a lyophilisate and 1 ampoule with a solvent of 2 ml in a plastic cassette or without it).

Composition of 1 tablet Texamen:

  • active substance: tenoxicam - 20 mg;
  • additional components: iron oxide yellow - 0.6 mg; macrogol 400 (polyethylene glycol 400) - 0.3 mg; hypromellose - 2.9 mg; titanium dioxide - 1.8 mg; lactose monohydrate - 90 mg; corn starch - 70 mg; pregelatinized starch - 16 mg; talc - 3 mg; magnesium stearate - 1 mg; Opadry OY-S 22989 yellow - 5.6 mg.

Composition of 1 bottle of Texamen lyophilisate:

  • active substance: tenoxicam - 20 mg;
  • additional components: trometamol - 3 mg; mannitol - 57.33 mg; edetate disodium - 0.2 mg; sodium hydroxide - 3.28 mg; sodium metabisulfite - 2 mg.

Pharmacological properties

Pharmacodynamics

Texamen belongs to the number of NSAIDs (non-steroidal anti-inflammatory drugs).

Tenoxicam is a thienothiazine derivative of oxicam. In addition to anti-inflammatory, antipyretic and analgesic effects, Texamen also prevents platelet aggregation.

The anti-inflammatory effect of tenoxicam is provided by the suppression of the activity of cyclooxygenase isoenzymes, which is involved in the metabolism of arachidonic acid, due to which the synthesis of prostaglandins is suppressed.

Tenoxicam has no effect on the activity of lipoxygenases. In addition, the substance inhibits some of the functions of leukocytes, including phagocytosis, the release of histamine, and reduces the content of active radicals in the focus of inflammation.

Pharmacokinetics

Texamen in unchanged form is rapidly absorbed from the gastrointestinal tract. The maximum concentration of the substance is reached 2 hours after taking the drug. When Texamen is taken after a meal or when combined with antacids, the rate, but not the degree of its absorption, decreases.

The average half-life is 70 hours. Tenoxicam is completely absorbed, its bioavailability is 100%. In the blood, the substance binds to proteins by 99%.

Texamen penetrates well into the synovial fluid, is characterized by a long half-life and low systemic clearance, thanks to which the drug can be taken once a day.

It is excreted in the urine and feces (2/3 and 1/3 of the dose, respectively). Accumulation of tenoxicam is not observed with prolonged use; serum content of the drug - 0.01–0.015 mg / ml.

Indications for use

According to the instructions, Texamen is prescribed for the symptomatic treatment of the following diseases:

  • bursitis, ankylosing spondylitis, osteoarthritis, tendovaginitis, rheumatoid arthritis, articular syndrome with exacerbation of gout;
  • pain syndrome of weak / moderate intensity, which develops with arthralgia, myalgia, neuralgia, migraine, toothache and headache, algodismenorrhea;
  • pain with burns, injuries;
  • lumbago, sciatica, epicondylitis and other inflammatory and degenerative diseases of the musculoskeletal system, accompanied by pain.

Texamen reduces pain and inflammation at the time of use, does not affect the progression of the disease.

Contraindications

Absolute:

  • hearing loss;
  • blood diseases;
  • severe gastritis;
  • confirmed hyperkalemia;
  • erosive and ulcerative lesions of the gastrointestinal tract (including a burdened history);
  • gastrointestinal bleeding (including a burdened history);
  • pathology of the vestibular apparatus;
  • active liver diseases, conditions after coronary artery bypass grafting;
  • progressive kidney disease;
  • partial / complete combination of bronchial asthma, recurrent polyposis of the nose and paranasal sinuses, and intolerance to acetylsalicylic acid or other NSAIDs (including a burdened history);
  • hypocoagulation;
  • hemophilia;
  • inflammatory diseases of the gastrointestinal tract;
  • hepatic and renal failure (creatinine clearance less than 30 ml / min);
  • deficiency of glucose-6-phosphate dehydrogenase;
  • pregnancy and the period of breastfeeding;
  • individual intolerance to the components of the drug.

Relative (Texamen is prescribed under medical supervision):

  • chronic renal failure (creatinine clearance 30-60 ml / min);
  • chronic heart failure;
  • cardiac ischemia;
  • swelling;
  • severe somatic diseases;
  • diabetes;
  • peripheral arterial disease;
  • arterial hypertension;
  • cerebrovascular diseases;
  • hyperlipidemia / dyslipidemia;
  • smoking;
  • the presence of infection Helicobacter pylori (H. pylori);
  • alcoholism;
  • long-term use of NSAIDs;
  • combined use with the following drugs: oral glucocorticosteroids, anticoagulants, antiplatelet agents, selective serotonin reuptake inhibitors;
  • elderly age.

Instructions for the use of Texamen: method and dosage

Texamen tablets are taken orally, preferably at the same time after meals.

The injection solution is administered intramuscularly, intravenously.

Frequency rate of application - 1 time per day.

The average single dose, including elderly patients, is 20 mg; with prolonged use, the dose is reduced by 2 times.

In case of acute attacks of gout, Texamen should take 40 mg for the first two days, then switch to the standard dosage regimen for a course of 3-5 days.

Side effects

During therapy with Texamen, the following disorders may develop (> 10% - very often;> 1% and 0.1% and 0.01% and <0.1% - rarely; <0.01% - very rarely):

  • hematopoietic system: decreased hemoglobin level, granulocytopenia;
  • nervous system: rarely - headache, blurred vision, eye irritation, depression, dizziness, agitation, hearing loss, tinnitus, drowsiness;
  • cardiovascular system: infrequently - tachycardia, heart failure, increased blood pressure;
  • digestive system: infrequently - pain / burning sensation in the stomach, vomiting, nausea, diarrhea / constipation, gastropathy, flatulence, perforation of the intestinal walls, abdominal pain, anorexia, stomatitis, hepatic dysfunction; with long-term use of large doses of Texamene - ulceration of the mucous membrane of the gastrointestinal tract, bleeding (uterine, gastrointestinal, hemorrhoidal, gingival);
  • hematopoietic organs: rarely - leukopenia, agranulocytosis, anemia, thrombocytopenia;
  • genitourinary system: rarely - an increase in the content of urea nitrogen and creatinine in the blood;
  • hepatobiliary system: rarely - increased activity of alanine transaminase, aspartate aminotransferase and gamma-glutamyltransferase and serum bilirubin level;
  • allergic reactions: rarely - photodermatitis, rash, urticaria, pruritus, erythema; very rarely - Lyell and Stevens-Johnson syndromes;
  • laboratory parameters: rarely - prolongation of bleeding time, increased concentration of urea nitrogen and hepatic transaminase activity, hypercreatininemia, hyperbilirubinemia;
  • others: metabolic disorders, mental disorders.

Overdose

In cases of overdose, symptomatic therapy is indicated.

During the period of therapy with Texamen, monitoring of the functional state of the liver / kidneys and the picture of peripheral blood, prothrombin index (against the background of the use of indirect anticoagulants), the concentration of glucose in the blood (against the background of the use of oral hypoglycemic drugs) is required.

If it is necessary to determine 17-ketosteroids, Texamen should be canceled 48 hours before the study.

A few days before surgery, the drug is canceled. When prescribed with diuretics in patients with arterial hypertension and chronic heart failure, the likelihood of sodium and water retention in the body should be taken into account.

In order to reduce the risk of adverse events from the gastrointestinal tract, Texamen should be taken in the lowest effective dose for a minimally short course.

Influence on the ability to drive vehicles and complex mechanisms

Since dizziness may develop during the use of Texamen, patients should be careful when driving vehicles.

Application during pregnancy and lactation

Texamen is not prescribed for women during pregnancy / lactation.

With impaired renal function

  • progressive kidney disease, renal failure (creatinine clearance less than 30 ml / min): therapy is contraindicated;
  • chronic renal failure (creatinine clearance 30-60 ml / min): Texamen is prescribed under medical supervision.

For violations of liver function

Texamen is not prescribed for liver failure.

Use in the elderly

For elderly patients, Texamen should be used under medical supervision.

Drug interactions

  • myelotoxic drugs: increased manifestation of drug hepatotoxicity;
  • anticoagulants, antiplatelet agents, fibrinolytics: enhancing their action;
  • other NSAIDs: an increase in the risk of adverse reactions, especially from the gastrointestinal tract;
  • medicines with hypotensive action, diuretics: decrease in their effectiveness;
  • adrenal cortex hormones, glucocorticosteroids, estrogens: increasing their side effects;
  • uricosuric drugs: decrease in their effectiveness;
  • sulfonylurea derivatives: increased hypoglycemic effect;
  • Li + preparations, methotrexate: an increase in their concentration in the blood;
  • cholestyramine, antacids: decreased absorption of Texamena;
  • phenytoin, tricyclic antidepressants, barbiturates, ethanol, phenylbutazone, rifampicin and other inducers of microsomal oxidation in the liver: an increase in the production of hydroxylated active metabolites.
