Zyrtec is a blocker of histamine H 1 receptors, an anti-allergic drug with antipruritic and anti-exudative action.

Release form and composition

  • Film-coated tablets: oblong, biconvex, white, on one side there is a risk, on both sides of which there is an engraving "Y" (7 pcs. In blisters, in a cardboard box 1 blister; 10 pcs. In blisters, in a pack of cardboard 1 or 2 blisters);
  • Drops for oral administration: transparent, colorless, with the smell of acetic acid (10 or 20 ml each in dark glass dropper bottles, in a cardboard box 1 bottle).

The active ingredient is cetirizine dihydrochloride:

  • 1 tablet - 10 mg;
  • 1 ml drops - 10 mg.

Excipients of tablets: colloidal silicon dioxide, lactose monohydrate, magnesium stearate, microcrystalline cellulose, Opadray Y-1-7000 (titanium dioxide (E171), hypromellose (E464), macrogol 400).

Excipients of drops: methylparabenzene, propylparabenzene, propylene glycol, sodium acetate, glycerol, glacial acetic acid, sodium saccharinate, purified water.

Indications for use

  • Urticaria, including chronic idiopathic
  • Pollinosis (hay fever);
  • Quincke's edema;
  • Seasonal and / or perennial rhinitis and conjunctivitis of allergic genesis (symptomatic treatment of symptoms such as itching, conjunctival hyperemia, lacrimation, sneezing, rhinorrhea);
  • Other allergic dermatoses, accompanied by itching and rashes, including atopic dermatitis.

Contraindications

Absolute:

  • End-stage renal failure (creatinine clearance less than 10 ml / minute);
  • Lactase deficiency, hereditary galactose intolerance, glucose-galactose malabsorption syndrome;
  • Children up to 6 months old - for drops, up to 6 years old - for tablets;
  • Pregnancy and lactation;
  • Increased individual sensitivity to the drug or hydroxyzine.

Relative:

  • Chronic renal failure
  • Chronic liver disease;
  • Elderly age.

Method of administration and dosage

Zyrtec is taken orally.

Recommended doses:

  • Adults: 10 mg (1 tablet or 20 drops) 1 time per day;
  • Children over 6 years of age: 10 mg once a day or 5 mg (1/2 tablet or 10 drops) 2 times a day;
  • Children 2-6 years old: 2.5 mg (5 drops) 2 times a day or 5 mg (10 drops) 1 time a day;
  • Children 1-2 years old: 2.5 mg (5 drops) 1-2 times a day;
  • Children 6-12 months: 2.5 mg (5 drops) 1 time per day.

In some cases, 5 mg per day is sufficient for adults and children over 6 years of age to achieve a therapeutic effect.

For elderly people and patients with renal insufficiency, the dose is adjusted depending on the creatinine clearance (CC), which is calculated as follows:

  • For men: CC (ml / minute) = [140 - age (in years)] x body weight (in kilograms) / 72 x serum creatinine (mg / dl);
  • For women: CC (ml / minute) = [140 - age (years)] x body weight (in kilograms) / 72 x serum creatinine (mg / dl) x 0.85.

Recommended doses of Zyrtek for patients with hepatic and renal impairment:

  • CC 50-79 ml / minute (mild renal failure) - 10 mg / day;
  • CC 30-49 ml / minute (average renal failure) - 5 mg / day;
  • CC <30 ml / minute (severe renal failure) - 5 mg every other day.

In end-stage renal failure, the drug is contraindicated.

Patients diagnosed with only liver dysfunction do not need dose adjustment.

Side effects

Zyrtec is generally well tolerated, in rare cases the following side effects occur:

  • Allergic reactions: rash, urticaria, itching, angioedema, hypersensitivity reactions, up to the development of anaphylactic shock;
  • From the side of the organ of vision: blurred vision, violation of accommodation, nystagmus;
  • From the side of metabolism: an increase in body weight;
  • From the respiratory system: pharyngitis, rhinitis;
  • From the side of the cardiovascular system: tachycardia;
  • From the hematopoietic system: thrombocytopenia;
  • From the urinary system: urinary disorder and enuresis;
  • From the digestive system: nausea, dry mouth, diarrhea, abdominal pain, abnormal liver function (increased levels of hepatic transaminases, alkaline phosphatase, gamma-glutamyltransferase, bilirubin);
  • From the nervous system: dizziness, drowsiness, headache, agitation, aggression, confusion, hallucinations, depression, insomnia, convulsions, tic, dyskinesia, paresthesia, dystonia, tremor, fainting;
  • Others: fatigue, malaise, asthenia, edema.

special instructions

When taking Zyrtek at the recommended dose, no negative effect on the speed of reactions and the ability to concentrate was found. However, during treatment it is recommended to refrain from driving vehicles and performing potentially hazardous activities.

Drug interactions

No unwanted drug interactions of cetirizine with glipizide, erythromycin, ketoconazole, azithromycin, diazepam, cimetidine, pseudoephedrine were detected.

With the simultaneous use of theophylline (in a daily dose of 400 mg), the total clearance of cetirizine decreases by 16%, the kinetics of theophylline does not change.

Data on the interaction of Zirtek with alcohol have not been obtained, however, patients are advised to refrain from drinking alcoholic beverages during the period of drug treatment in order to avoid depression of the central nervous system.
