Release form and composition
The preparation is produced in the form of capsules: size No. 1, hard gelatinous, with a yellow body and lid; content - white / almost white powder (10 or 20 pcs. in blisters, a cardboard box may contain 1, 2, 3 or 4 packages and instructions for the use of Elmucin).
Composition of 1 capsule:
- active substance: erdosteine - 300 mg;
- auxiliary components: povidone K30, microcrystalline cellulose, magnesium stearate;
- composition of the capsule body and cap: gelatin, titanium dioxide, sunset yellow dye, quinoline yellow dye.
Pharmacological properties
Pharmacodynamics
The active substance of Elmucin is erdosteine, which determines the pharmacological properties of the drug.
The effectiveness of erdosteine as a mucolytic is due to the action of its active metabolites. The thiol groups of metabolites cause the rupture of disulfide bridges that bind the fibers of glycoproteins to one another, as a result of which the viscosity and elasticity of sputum decreases. As a result, the drug enhances the release of the respiratory tract from secretions and accelerates this process, improves the secretory function of the epithelium, and also increases the efficiency of mucociliary transport in the upper and lower parts of the respiratory tract.
Erdosteine carries free radicals, has antioxidant activity: it protects the respiratory tract from the damaging effects of cigarette smoke, inactivating alpha-1-antitrypsin. Reduces the suppressive effect of tobacco smoke on the function of granulocytes.
In chronic obstructive airway diseases, the drug increases the concentration of immunoglobulin A (IgA) in the mucous membrane of the respiratory tract.
The effect develops after 3-4 days of regular administration of Elmucin. The drug does not contain free SH-radicals, therefore it does not have a negative effect on the digestive system, and its side reactions from the gastrointestinal tract do not differ from those with placebo.
Pharmacokinetics
Once in the gastrointestinal tract, erdosteine is rapidly absorbed. The maximum plasma concentration (C max) is reached within approximately 1.48 hours and is 3.46 μg / ml. The area under the concentration-time curve (AUC 0-24h) is 12.09.
Pharmacokinetic parameters of erdosteine do not depend on food intake and duration of drug administration.
The connection with plasma proteins is about 64.5%.
Erdosteine is metabolized in the liver, as a result of which three active metabolites are formed, the main of which is N-thiodiglycolyl-homocysteine (Metabolite 1, or M1).
It is excreted through the kidneys and intestines in the form of inorganic sulfates. The half-life is over 5 hours and may increase in patients with severe liver dysfunction.
In renal failure, drug metabolites may accumulate.
If liver function is impaired, C max and AUC values increase.
Indications for use
Elmucin is intended for the treatment of respiratory diseases, accompanied by the formation of viscous sputum:
- smoker's cough;
- chronic obstructive pulmonary disease;
- acute and chronic bronchitis;
- bronchiectasis;
- bronchial asthma with difficulty in sputum discharge;
- pneumonia.
Also, the drug is used to prevent lung atelectasis and pneumonia after surgery.
Contraindications
- renal failure (creatinine clearance <25 ml / min);
- severe liver dysfunction;
- homocystenuria;
- pregnancy (I trimester);
- lactation period;
- age up to 18 years;
- hypersensitivity to any component of the drug.
Elmucin capsules should be used with caution in patients with functional disorders of the liver and renal failure (creatinine clearance> 25 ml / min), as well as in diseases of the bronchi, accompanied by excessive accumulation of secretions.
Elmucin, instructions for use: method and dosage
Elmucin capsules should be taken orally 1 pc. 2 or 3 times during the day.
The recommended duration of admission is 5 days. If during this time there is no improvement or worsening is noted, a doctor's consultation is required.
Side effects
- from the gastrointestinal tract: rarely (from ≥ 1/10 000 to <1/1000) - heartburn, diarrhea, nausea; very rarely (<1/10 000) - change or loss of taste sensitivity (noted at the beginning of therapy);
- allergic reactions: rarely - urticaria, skin redness.
Overdose
No cases of erdosteine overdose have been reported to date.
For patients who have taken an excessive dose and children who have taken the drug, gastric lavage and symptomatic treatment are recommended.

During the period of taking Elmucin, you need to drink enough liquid, since it enhances the expectorant effect of the drug.
In case of impaired motility of the bronchi and a significant amount of secreted sputum, the mucolytic should be used with caution, since there is a likelihood of a delay in the discharge in the airways, the risk of bronchospasm and infection increases.
Influence on the ability to drive vehicles and complex mechanisms
The components in Elmucin do not adversely affect the psychoemotional state of the patient and his physical abilities.
Application during pregnancy and lactation
In preclinical studies, the embryotoxic effect of erdosteine was not revealed. However, clinical experience with the use of the drug is limited, therefore Elmucin may be prescribed only in the II and III trimesters of pregnancy, provided that the intended benefits of therapy definitely outweigh the potential risks.
During lactation (breastfeeding) the drug is contraindicated.
Pediatric use
Elmucin is not used in pediatrics.
With impaired renal function
- contraindicated: renal failure with creatinine clearance <25 ml / min;
- caution is required: renal failure with creatinine clearance> 25 ml / min.
For violations of liver function
- contraindicated: severe liver dysfunction;
- caution is required: moderate hepatic impairment.
Drug interactions
Erdosteine, when combined with amoxicillin, increases its concentration in bronchial secretions, thereby increasing the effectiveness of the treatment in comparison with monotherapy with amoxicillin.
