Pharmacological action Hartil
Due to its active components, Hartil has cardioprotective and antihypertensive effects.
Release form
Hartil is available in the form of tablets with different contents of the active active ingredient - ramipril. There are three types of tablets: 2.5 mg (yellow tablets), 5 mg (orange-pink) and 10 mg (white). The rest of the tablets are similar and have an oval shape with a score, a chamfer and engravings "R2", "R3" and "R4", respectively.
Indications for Hartil's use
According to the instructions for Hartil, the drug should be taken as a medicine for the following types of diseases:
- with arterial hypertension;
- with chronic heart failure;
- with heart failure after acute myocardial infarction in patients with stable hemodynamics;
- with diabetic nephropathy, chronic diffuse kidney disease.
Medical reviews about Hartil recommend the drug as a means to reduce the risk of myocardial infarction, "coronary death" or stroke in patients with coronary artery disease, including those who have undergone coronary artery bypass grafting, myocardial infarction, percutaneous transluminal coronary angioplasty.
Contraindications
Hartil and drug analogues are not recommended for use in the cases listed below:
- a history of angioedema, including associated with previous therapy with ACE inhibitors;
- high sensitivity to the components of the drug, including ramipril;
- stenosis of an artery of a single kidney, significant from the point of view of hemodynamics, bilateral stenosis of the renal arteries;
- unstable hemodynamics and / or arterial hypotension;
- any trimester of pregnancy, as well as the period of breastfeeding;
- primary hyperaldosteronism;
- renal failure.
Instructions for the use of Hartil
According to the instructions for Hartil, the drug is administered orally. There is no link to meal time. The tablets should not be chewed, however, it is necessary to drink at least 200 ml of liquid. The dosage of Hartil for each patient is set individually by the attending physician, while there are recommended doses of the drug that depend on the specific disease.
With arterial hypertension, you should start with a single dose of 2.5 mg of Hartil per day. If necessary, the dose is increased every 2-3 weeks, doubling it. In this case, the maximum dosage should not be higher than 10 mg of the drug per day.
In chronic heart failure, it is recommended to start taking Hartil with 1.25 mg daily. The dose can be doubled every 2-3 weeks. The maximum is also 10 mg per day.
When treating after a myocardial infarction, it is recommended to start taking Hartil a few days later (from 2 to 9) after the acute stage of the disease. The initial dosage depends on the patient's condition and the time elapsed since the acute phase and, as a rule, is 2 tablets of 2.5 mg twice a day (or an equivalent dosage of tablets of 1.25 mg). If necessary, the daily dose can be doubled. The maximum allowable daily dosage is 10 mg.
For nephropathies (diabetic and non-diabetic), the instruction to Hartil prescribes to take the medicine at 1.25 mg once a day. The dose can be increased by doubling every 2-3 weeks. It is recommended to take no more than 5 mg of the drug per day.
In the prevention of stroke, myocardial infarction or death from cardiovascular disorders, the initial dose of Hartil is 2.5 mg. If the drug is well tolerated, the dose is doubled after a week of administration, after three weeks it can be doubled again. Maximum per day - 10 mg.
Hartil's side effects
Medical reviews about Hartil indicate the likelihood of side effects. Their list is very extensive and requires detailed study. It is recommended that you familiarize yourself with the most common side effects before starting the medication:
- orthostatic hypotension, lowering blood pressure. In rare cases - arrhythmia, the appearance of circulatory disorders of organs, ischemia of the myocardium and brain;
- increased proteinuria, renal failure, decreased libido, decreased urine volume;
- weakness, headache, drowsiness, nervous irritability, tremors, mood disorders, anxiety, muscle spasm;
- disturbances in perception (smell, hearing, taste, vision) and vestibular disturbances;
- decreased appetite, nausea, constipation / diarrhea, pancreatitis, dry mouth, vomiting;
- shortness of breath, bronchospasm, rhinorrhea, sinusitis, rhinitis, bronchitis, dry cough;
- urticaria, itching, skin rash, conjunctivitis;
- thrombocytopenia, anemia, decreased hematocrit and hemoglobin concentration, thrombocytopenia, neutropenia, leukocytopenia, agranulocytosis, hemolytic anemia, pancytopenia;
- convulsions, hyperthermia, alopecia, sweating;
- hyperkalemia, hypercreatininemia, increased hepatic transaminase and urea nitrogen levels, hyponatremia.
The use of Hartil during pregnancy can lead to side effects on the fetus: various disorders of the development and function of the kidneys, a decrease in the child's blood pressure, hypoplasia of the lungs and skull, contracture of the limbs, deformation of the skull.
Precautions
During the use of Hartil and analogues, constant medical supervision is urgently needed. This is especially true when taking the drug for the first time and increasing its dosage. Multiple blood pressure measurements are recommended within 8 hours from the moment of taking the drug.
Before starting the medication, it is necessary to correct hypovolemia and dehydration.
Patients with lesions of the renal vessels, impaired renal function and after kidney transplantation require particularly careful observation while taking Hartil.
There is insufficient data regarding the intake of Hartil by children and patients during dialysis.
In the case of a decrease in blood pressure, patients taking Hartil are advised to refuse to perform those activities that require increased concentration.
