Release form and composition

Proflosin is available in the form of enteric capsules of prolonged action: size No. 2, gelatinous, opaque, the capsule body is light orange, the cap is gray-green, the cap has a black line and the inscription "TSL 0.4", on the body there is only a black line; inside capsules - pellets from almost white to white (10 pcs. in a blister, in a cardboard box of 1, 3 or 10 blisters).

1 capsule contains:

  • active substance: tamsulosin hydrochloride - 0.4 mg;
  • auxiliary components: copolymer of ethyl acrylate and methacrylic acid (1: 1), microcrystalline cellulose, talc, triethyl citrate;
  • the composition of the pellet shell: copolymer of ethyl acrylate and methacrylic acid (1: 1), talc, triethyl citrate;
  • composition of the capsule cap: gelatin, titanium dioxide, indigo carmine, iron dye yellow oxide, iron dye black oxide;

Pharmacological properties

Pharmacodynamics

Proflosin - alpha1-blocker, is used for urination disorders caused by benign prostatic hyperplasia.

The mechanism of action of tamsulosin is due to the competitive and selective blocking of postsynaptic alpha 1A adrenergic receptors of the smooth muscles of the prostate gland, the bladder neck, the prostatic urethra, which leads to a decrease in their tone, and an improvement in urine outflow.

The effect of alpha-adrenergic receptor blockers contributes to a decrease in peripheral vascular resistance and can cause a slight decrease in blood pressure (BP). This effect of tamsulosin does not affect the clinical parameters of patients with normal baseline blood pressure.

Pharmacokinetics

After oral administration, the absorption of tamsulosin from the gastrointestinal tract occurs almost completely and quickly. The rate of its absorption decreases with the simultaneous intake of food.

The maximum concentration in blood plasma after taking a single dose of the drug is reached after about 6 hours.

About 99% of the dose taken is bound to plasma proteins.

The volume of distribution of tamsulosin is 0.2 l / kg.

The metabolism of the active substance occurs slowly in the liver, with the formation of metabolites that are less active than tamsulosin. In the blood, most of tamsulosin is present unchanged. Its ability to induce the activity of liver microsomal enzymes to an insignificant extent has been experimentally confirmed.

It is excreted mainly through the kidneys, including 9% of the drug - unchanged. The half-life with a single dose of 0.4 mg of tamsulosin after meals is approximately 10 hours, with a daily intake - 13 hours.

Indications for use

According to the instructions, Proflosin is indicated only for men for the treatment of dysuric disorders associated with benign prostatic hyperplasia.

Contraindications

  • severe liver failure;
  • orthostatic hypotension (including history);
  • individual intolerance to the components of the drug.

It is recommended to prescribe Proflosin with caution for arterial hypotension, chronic renal failure with creatinine clearance (CC) less than 10 ml / min.

Instructions for the use of Proflosin: method and dosage

Proflosin capsules are taken orally, swallowing whole (the integrity of the capsule shell must not be violated, this will increase the rate of release of the active substance), drinking plenty of water after the first meal.

Recommended dosage: 1 pc. 1 time per day. The duration of the course is not limited.

Side effects

  • on the part of the cardiovascular system: infrequently - orthostatic hypotension, tachycardia;
  • from the nervous system: often - dizziness; infrequently - headache; rarely - fainting;
  • dermatological reactions: infrequently - urticaria, pruritus, rash; rarely - angioedema;
  • from the reproductive system: infrequently - violation of ejaculation; very rarely - priapism;
  • from the gastrointestinal tract: infrequently - nausea, vomiting, constipation, diarrhea;
  • others: in some cases - asthenia, rhinitis, during cataract surgery - the development of a narrow pupil syndrome (atonic iris syndrome).

Overdose

Symptoms: possibly a sharp drop in blood pressure, tachycardia.

Treatment: the patient needs to take a horizontal position, this will ensure recovery to normal blood pressure and heart rate. In order to prevent further absorption of tamsulosin from the gastrointestinal tract, it is advisable to rinse the stomach, take activated charcoal or sodium sulfate (osmotic laxative).

Hemodialysis is not advisable.

Drug interactions

With the simultaneous use of Proflosin:

  • nifedipine, atenolol, enalapril, theophylline do not cause changes in their pharmacodynamic and pharmacokinetic parameters and tamsulosin parameters;
  • other blockers of alpha1-adrenergic receptors can cause a decrease in blood pressure;
  • furosemide causes a slight decrease in the concentration of tamsulosin in the blood plasma (dose adjustment is not required);
  • cimetidine contributes to a slight increase in the level of concentration of tamsulosin in blood plasma (dose adjustment is not required);
  • diclofenac, warfarin increase the rate of drug elimination.
