Pharmacological properties

Pharmacodynamics

Portalac is a laxative with a hyperosmotic effect. Its mechanism of action is due to the influence of lactulose on the potentiation of the reproduction process in the intestine of Lactobacillus acidophilus and Lactobacillus bifidus, which causes a decrease in acidity in the lumen of the large intestine and the activation of its peristalsis. Along with the stimulation of intestinal motility, lactulose improves the absorption of phosphates and calcium salts, excretion of ammonium ions, and slows down the growth of Salmonella in the large intestine. Against the background of taking Portalak, the volume of feces increases and their softening occurs. The syrup does not directly affect the mucous membrane and smooth muscles of the large intestine.

The effect of lactulose helps to reduce the formation of nitrogen-containing toxic substances in the proximal colon and their absorption into the systemic circulation. The level of ammonium ions in the blood decreases by 25-50%, the severity of hepatic encephalopathy decreases, the patient's mental state improves and electroencephalography is normalized.

After taking the syrup inside, the therapeutic effect occurs as the drug passes through the gastrointestinal tract (GIT) for 24-48 hours.

Pharmacokinetics

After oral administration, lactulose is practically not absorbed from the gastrointestinal tract, no more than 3% of the dose taken is absorbed.

While taking vitamins, their absorption is not reduced.

Portalak is not addictive.

Due to the insignificant systemic effect of lactulose for elderly patients, with renal or hepatic insufficiency, dosage adjustment is not required.

Indications for use

  • constipation, including chronic constipation;
  • intestinal dysbiosis;
  • hemorrhoids, pain syndrome after removal of hemorrhoids, preparation for surgery on the colon and / or anus, postoperative period - in order to soften the stool for medical purposes;
  • hyperammonemia;
  • hepatic encephalopathy;
  • treatment and prevention of hepatic precoma and coma;
  • enteritis due to salmonella or shigella;
  • salmonella carriage;
  • putrid dyspepsia syndrome in young children due to acute food poisoning.

Contraindications

Absolute:

  • intestinal obstruction;
  • galactosemia;
  • rectal bleeding not associated with hemorrhoids;
  • colostomy, ileostomy;
  • suspicion of appendicitis;
  • individual intolerance to lactulose.

Patients with lactose intolerance, diabetes mellitus, gastrocardiac syndrome should be used with caution.

Portalak, instructions for use: method and dosage

Portalac syrup is taken orally, preferably during or after a meal, with water or other liquid.

The dose of the drug should be selected individually. The dose or time of administration should be changed as needed.

After the start of taking the laxative effect develops during the first two days of therapy.

Recommended dosage of Portalak:

  • treatment of constipation and softening of stools: infants under 1 year old - 5 ml (1 teaspoon) per day; children aged 1-6 years - 5-10 ml per day; children from 7 to 14 years old, initial dose (3 days) - 15 ml (1 tablespoon) per day, maintenance dose - 10 ml per day; adults, the initial dose is 15–45 ml per day for three days, then 15–30 ml per day. The daily dose is taken once in the morning or divided into two doses. In the treatment of constipation, the duration of the course can be from 28 to 120 days;
  • treatment of hepatic (portal) encephalopathy, hepatic coma and precoma: 30-50 ml 3 times a day. In order to achieve a quick effect, the initial dose can be 30–45 ml with an interval of 1–2 hours. After the first emptying of the intestine, the patient is transferred to a supporting individual dose that provides soft stools 2–3 times a day;
  • treatment of dysbiosis, normalization of intestinal microflora: a single dose for children under 1 year old - 1.5-3 ml, from 1 to 3 years old - 3 ml, from 3 to 7 years old - 5 ml, for adults and children over 7 years old - 5– 10 ml. It is divided into 2-4 doses per day during or after meals. The duration of the course of treatment is 10-14 days. Repeated courses are prescribed with an interval of 7 days.

Side effects

Adverse events against the background of the use of Portalak arise, as a rule, due to an excess of the dose and are mildly reversible. Reducing the dose may be sufficient to relieve abdominal discomfort or pain, cramps and diarrhea.

The flatulence that appears at the beginning of treatment usually resolves in 1–2 days.

Skin reactions in the form of a rash are possible.

Long-term use of high doses of lactulose in the treatment of hepatic encephalopathy can cause imbalance in the water-electrolyte balance in patients, which results in weakness, increased fatigue, nausea, headache, dizziness, convulsions, arrhythmias, myalgia.

Overdose

Symptoms of an overdose of lactulose are diarrhea, imbalance in water and electrolyte balance.

For the purpose of treating the condition, you should immediately stop taking the laxative.

If, after several days of taking Portalak, the constipation persists or if constipation recurs after discontinuation of the drug, consult your doctor.

Patients with lactose intolerance and diabetes mellitus should take into account that 15 ml of syrup contains up to 1.7 g of galactose and up to 1 g of lactose.

Treatment of patients with gastrocardial syndrome should be started with lower doses, then gradually increased, this will minimize the risk of flatulence.

In the treatment of hepatic encephalopathy, other laxatives should not be used simultaneously, especially in the initial phase of therapy. Intense bowel movements may erroneously indicate the achievement of an adequate dose for the treatment of encephalopathy.

When taken simultaneously with drugs containing mesalazine, it should be borne in mind that the environment created by lactulose in the large intestine can contribute to their inactivation.

With concomitant therapy with antibiotics (including neomycin) and antacids, the clinical effect of Portalac is reduced.
