Powercourt external cream
Powercourt external cream

Powercourt - glucocorticosteroid, drug for external use; has antipruritic, anti-inflammatory, anti-allergic and anti-exudative effects.

Release form and composition

The drug is produced in the form of a cream for external use: a homogeneous mass of white color (15 g each in aluminum tubes, in a cardboard box 1 tube and instructions for use of Powercourt).

100 g of cream contains:

  • active substance: clobetasol propionate - 0.05 g;
  • auxiliary components: sodium phosphate dihydrate, liquid paraffin, soft white paraffin, propylene glycol, cetomacrogol, propyl parahydroxybenzoate, sodium phosphate monohydrate, methyl parahydroxybenzoate, butylhydroxytoluene, purified water.

Pharmacological properties

Pharmacodynamics

Powercourt is a drug for external use with anti-inflammatory, antipruritic, anti-allergic and anti-exudative effects.

The active substance, clobetasol, is a local glucocorticosteroid (GCS), which has the property of preventing the marginal accumulation of neutrophils, reducing inflammatory exudation and production of lymphokines, and slowing the migration of macrophages. Helps to reduce the intensity of the processes of infiltration and granulation.

Pharmacokinetics

GCS, applied externally, can undergo systemic absorption from intact healthy skin. Factors such as the integrity of the epidermal barrier and the base of the drug affect the degree of percutaneous absorption of GCS for external use. Occlusion, the presence of inflammation or other pathological process on the part of the skin can increase the degree of percutaneous absorption of clobetasol.

GCS for external use after absorption through the skin are metabolized in the same ways as GCS for systemic use (mainly in the liver).

Indications for use

  • psoriasis, other than plaque (vulgar) psoriasis;
  • lichen planus;
  • discoid lupus erythematosus;
  • eczema;
  • skin diseases resistant to other topical corticosteroids.

Powercourt, instructions for use: method and dosage

Powercourt cream is applied externally by applying a thin layer to the skin of the affected area of the body.

The recommended frequency of procedures: 1-2 times a day until the onset of the therapeutic effect. If, after 28 days of treatment, its extension is required, a thorough assessment of the feasibility of continuing the course of using Powercourt is necessary. In this case, a transition to GCS with less pronounced activity may be required. For the relief of exacerbations of skin diseases, repeated short courses of therapy are indicated.

To enhance the anti-inflammatory effect of the cream in the treatment of particularly resistant forms of skin pathologies, including hyperkeratosis, you can use a bandage made of plastic film, covering the area of application of the drug at night. Further, the achieved effect should be maintained without the use of an occlusive dressing.

Side effects

In adult patients, the use of a cream at a dose of not more than 50 g per week causes a transient suppression of the function of the pituitary and adrenal glands. The patient's condition quickly recovers after discontinuation of Powercourt's treatment.

Against the background of long-term therapy in high doses of local GCS, including when the cream is applied to large areas of the skin of the body, the following undesirable phenomena may develop: hypercortisolism, atrophic skin changes [including thinning of the skin, the appearance of striae (atrophic stripes), dilatation of superficial blood vessels (more often when applying occlusive dressings or treating areas of skin with folds)], skin pigmentation, hypertrichosis, sometimes - exacerbation of symptoms of the disease.

In the treatment of psoriasis or after the abolition of corticosteroids for local use, it is possible to develop pustular psoriasis.

The risk of developing hypercortisolism is increased in children, especially when using an occlusive dressing. It should be borne in mind that the diaper can serve as such a bandage.

Overdose

Symptoms: as a result of chronic excess of the recommended dosage or incorrect use - hypercortisolism.

Treatment: the gradual cancellation of Powercourt, monitoring the patient's condition.

special instructions

The use of Powercourt for the treatment of children must be accompanied by careful observation, the doctor must examine the child at least once a week.

Long-term use of corticosteroids for topical use, especially in young children, significantly increases the risk of suppression of adrenal function.

Atrophic changes in the skin against the background of long-term cream therapy often occur in the treatment of facial skin.

When applying the cream on the eyelids, do not allow it to get into the eyes due to the risk of developing glaucoma.

Special care must be taken when treating psoriasis. This is due to the risk of developing tolerance to the cream, relapses of the disease, generalized pustular psoriasis, local or systemic toxic reactions caused by impaired skin barrier function against the background of GCS use.

When a secondary infection occurs, appropriate antibiotic therapy is required. With the development of symptoms of generalized infection, cream treatment should be discontinued.

Before each procedure, thorough cleansing of the skin of the problem area of the body is required, especially after using occlusive dressings. It must be remembered that a warm, humid environment is conducive to creating favorable conditions for bacterial infection.

The cream must not be diluted.

Influence on the ability to drive vehicles and complex mechanisms

The development of systemic undesirable effects of clobetasol propionate, affecting the patient's ability to drive vehicles and mechanisms, is unlikely.

Application during pregnancy and lactation

During gestation, it is not recommended to prescribe GCS for topical use if it is necessary to apply the drug in high doses for a long period.

The safety of clobetasol propionate during lactation has not been established, therefore, Powercourt is contraindicated for breastfeeding.

Pediatric use

Powercourt is contraindicated for the treatment of infants under 1 year of age.

Drug interactions

The interaction of Powercourt cream with other drugs has not been established.

Analogs

Powercourt's analogues are: Afloderm, Locoid, Ftorocort, Triacort, Latikort, Cloveit, Dermovate, Halovat, Etrivex.

Powercourt reviews

Reviews of Powercourt are rare, but positive. The cream allows you to quickly get rid of such negative manifestations as itching, inflammation and redness of the skin, and peeling.

When treating psoriasis, patients indicate the clinical effect of the cream when it is applied to small foci of spread, that is, in the first stages of exacerbation of the disease. The use of the drug for the treatment of large areas, in their opinion, is ineffective.

The advantages of the cream include good tolerance, economy of consumption.
