Trimetazidine-Teva is an antihypoxant drug that improves myocardial metabolism in conditions of ischemia.
Release form and composition
Dosage form - film-coated tablets: round, biconvex, red-brown shell (10 pieces in blisters, in a cardboard box with or without a protective sticker 3 or 6 blisters and instructions for use of Trimetazidine-Teva).
Composition for 1 tablet:
- active substance: trimetazidine dihydrochloride - 20 mg;
- auxiliary components: mannitol - 44.5 mg; MCC (microcrystalline cellulose) - 20 mg; copolyvidone - 4 mg; magnesium stearate - 1 mg; colloidal silicon dioxide - 0.5 mg;
- film shell: polyethylene glycol (macrogol 6000) - 0.23 mg; hypromellose - 2.3 mg; dye iron oxide red (E172) - 0.1 mg; titanium dioxide (E171) - 0.49 mg.
Pharmacological properties
Pharmacodynamics
Trimetazidine-Teva helps to normalize energy metabolism in cells exposed to hypoxia or ischemia. It prevents a drop in the intracellular level of adenosine triphosphoric acid (ATP), maintains the normal function of membrane ion channels, ensures the transmembrane transport of sodium and potassium ions, as well as the maintenance of cellular homeostasis.
Trimetazidine inhibits fatty acid oxidation by selectively blocking long-chain 3-ketoacetyl-CoA thiolase. Thus, it enhances glucose oxidation and restores the conjugation between oxidative decarboxylation and glycolysis, which protects the heart muscle from ischemia. The antianginal action of trimetazidine is based on the substitution of glucose for fatty acids in the redox reaction.
Pharmacological properties of trimetazidine, confirmed in experimental studies conducted on animals:
- support of energy metabolism of the myocardium and neurosensory organs during episodes of ischemia;
- reducing the amount of intracellular acidosis and the degree of changes that occur during ischemia in the transmembrane ion flow;
- decrease in the degree of infiltration and migration of polynuclear neutrophils in ischemic and reperfusion tissues of the heart muscle, decrease in the area of the damaged myocardium;
- no effect on hemodynamics.
Pharmacokinetics
When taken orally, trimetazidine is absorbed quickly and almost completely by the intestinal mucosa.
The bioavailability of the substance is 90%. C max (maximum concentration) in plasma is noted at 55 ng / ml and is reached 2 hours after a single oral dose of 20 mg. V d (volume of distribution) - 4.8 l / kg.
Trimetazidine easily crosses histohematogenous barriers. It binds to plasma proteins at a level of 16%.
T 1/2 (half-life) varies from 4.5 to 5 hours. Basically, the substance is excreted by the kidneys, mainly unchanged (up to 60%).
Indications for use
Trimetazidine-Teva is recommended for long-term use for monotherapy or as part of a complex treatment of coronary heart disease in order to prevent angina attacks.
The drug is effective in the treatment of such cochleovestibular disorders of ischemic etiology as tinnitus, dizziness, and hearing impairment.
In addition, Trimetazidin-Teva is used for the treatment of chorioretinal vascular pathologies with an ischemic component.
Contraindications
- renal failure in the terminal phase, with a glomerular filtration rate below 15 ml / min / 1.73 m 2;
- severe violations of hepatic function;
- pregnancy and breastfeeding;
- children and adolescents under 18 years of age;
- hypersensitivity to trimetazidine and / or auxiliary components of the drug.
