Terrilitin is a preparation containing proteolytic enzymes that effectively breaks down necrotic tissue, dilutes purulent exudate and blood clots.
Release form and composition
Dosage form - lyophilisate for preparation of a solution for external use: white or white with a yellowish tinge porous mass or powder; hygroscopic, odorless [in a cardboard box 1 blister strip with 1, 3 or 5 vials or ampoules containing 100 or 200 proteolytic units (PU) of lyophilisate, and instructions for the use of Terrilitin].
The active substance in the lyophilisate is the enzyme terrilitin (obtained from native solutions of Aspergillus N-20).
Pharmacological properties
Terrilitin is one of the proteolytic enzymes - the waste products of molds. It is characterized by the ability to liquefy blood clots and purulent exudate, and to break down necrotic tissue.
Indications for use
Terrilitin is recommended for cleaning wound surfaces by splitting necrotic tissue, thinning blood clots and purulent exudate in case of burns, purulent wounds, bedsores and trophic ulcers.
Contraindications
Absolute:
- bleeding wounds and ulcers;
- ulceration of malignant neoplasms;
- pregnancy;
- lactation period;
- established hypersensitivity to the components of the drug.
Relative (the use of Terrilitin requires special care and careful medical supervision): the presence of a main blood vessel in the wound, as well as pathologies that are accompanied by slow blood coagulation.
Terrilitin, instructions for use: method and dosage
The solution obtained from the Terrilitin lyophilisate is intended for external use. 1 ml of the drug contains from 40 to 50 PE.
Immediately before use, the contents of one bottle of 100/200 PE, respectively, are filled with 2–2.5 / 4–5 ml of a 0.25% novocaine solution, 0.9% sodium chloride solution or purified water.
The resulting solution is moistened with a sterile napkin, which is applied to the wound surface and covered with a moisture-proof bandage on top.
In cases of deep burns and other lesions with a large area of necrotic tissue, as well as for rejection of the scab, the drug is used in the form of a powder - the affected surface is powdered with it. A sterile napkin slightly moistened in a solution of novocaine 0.25% or sodium chloride 0.9% and a bandage made of a moisture-proof material are applied on top of the drug. The bandage is changed every 1–2 days. In this case, areas of necrotic altered tissues are removed, and the wound surface is washed with a solution of hydrogen peroxide 3%, dried and again powdered with lyophilisate.
Terrilitin therapy is carried out until there is a complete rejection of necrotic tissues, melting of fibrinous-purulent plaques and the formation of clean, juicy granulations.
Side effects
During treatment with Terrilitin, allergic reactions may develop.
Overdose
Overdose cases of Terrilitin have not been reported.

When using Terrilitin externally, it is important to ensure adequate drainage of the wound.
Application during pregnancy and lactation
Terrilitin is not prescribed during pregnancy / lactation.
Drug interactions
Terrilitin promotes better penetration of other drugs into the inflammation focus.

- antibacterial agents - ampicillin, streptomycin, ampiox, lincomycin;
- antimicrobial and other chemotherapy drugs.
