TeraFlu Max is a combined preparation for eliminating symptoms of colds and acute respiratory diseases (analgesic non-narcotic drug + alpha-adrenergic agonist + vitamin).
Release form and composition
Dosage form - powder for preparation of solution for oral administration: from yellow to light yellow color, including colorless shiny crystals, without large agglomerates, with a characteristic lemon odor; the solution prepared from the powder is yellow, opalescent, has a characteristic lemon odor, without foreign inclusions and sediment (5 g of powder in four-layer sachets, fastened in pairs by perforation or placed individually, in a cardboard box 1–25 pcs. and instructions for use of TeraFlu Max).
Active ingredients in 1 sachet of powder:
- paracetamol - 1000 mg;
- ascorbic acid - 100 mg;
- phenylephrine hydrochloride - 12.2 mg.

Pharmacodynamics
TeraFlu Max with lemon flavor is one of the combined preparations for eliminating the manifestations of the common cold. The drug has antipyretic, analgesic and vasoconstrictor effects.
The pharmacological actions due to the active ingredients that make up TheraFlu Max include:
- preventing the formation of prostaglandins in the central nervous system by inhibiting the cyclooxygenase of arachidonic acid; antipyretic and analgesic effects (paracetamol);
- relief of nasal breathing due to a decrease in edema of the mucous membranes of the sinuses (phenylephrine);
- replenishment of the increased need for vitamin C in case of flu and colds (ascorbic acid).
Indications for use
Powder for preparation of a solution for oral administration TeraFlu Max is intended to eliminate the manifestations of influenza and colds - sore throat and sinuses, nasal congestion, muscle and joint pain, chills, headache and fever.
Contraindications
Absolute:
- pathology of the cardiovascular system - tachyarrhythmia, acute myocardial infarction, severe aortic stenosis;
- severe hepatic and / or renal failure;
- arterial hypertension;
- hyperthyroidism;
- angle-closure glaucoma;
- hyperplasia of the prostate;
- diabetes;
- hereditary disorders of glucose absorption;
- diseases of the blood system;
- combination therapy with other paracetamol-containing drugs, beta-blockers, tricyclic antidepressants, monoamine oxidase inhibitors (including within 2 weeks after their cancellation);
- age up to 18 years;
- lactation period;
- established hypersensitivity to the components of TheraFlu Max.
Relative (taking the drug requires special care and careful medical supervision):
- deficiency of glucose-6-phosphate dehydrogenase;
- benign hyperbilirubinemia;
- pregnancy.
TeraFlu Max, instructions for use: method and dosage
The solution prepared from TeraFlu Max powder is intended for oral administration.
Immediately before use, the contents of 1 sachet are poured with 250 ml of hot water and mixed thoroughly until completely dissolved.
The daily dose is 1 sachet every 4–6 hours, but no more than 4 sachets per day. The solution should not be taken for more than 5 days without consulting a doctor. If symptoms persist, it is important to seek medical attention.
It is not recommended to exceed the daily dose of TeraFlu Max. In case of overdose, you should immediately seek medical attention.
Side effects
TeraFlu Max is well tolerated when taken in recommended doses. In rare cases, undesirable effects caused by paracetamol are possible. Sometimes there are allergic reactions in the form of urticaria, skin rash and angioedema; rarely - agranulocytosis, leukopenia and thrombocytopenia.
Prolonged use of high doses of the drug can lead to the development of nephrotoxic and hepatotoxic effects.
Phenylephrine can cause mild increases in blood pressure, headache, nausea, and, in rare cases, palpitations. These undesirable effects go away on their own after treatment is discontinued.

Overdose
Possible symptoms: stomach pain, vomiting, pallor of the skin, nausea, anorexia, sweating, increased activity of hepatic transaminases, cardiac arrhythmias, hepatonecrosis, increased prothrombin time, coma.
Therapy: during the first 4 hours of intoxication, gastric lavage is performed. The appointment of activated carbon and symptomatic treatment are recommended. The specific antidote of the drug is acetylcysteine.

The drug should not be used before laboratory tests that assess the concentration of uric acid and glucose in the blood plasma, as it distorts their results.
The combined use of TeraFlu Max with other paracetamol-containing drugs, chloramphenicol, rifampicin, antiepileptic drugs, barbiturates, non-steroidal anti-inflammatory drugs (ibuprofen, acetylsalicylic acid, metamizole and others) and non-narcotic analgesics is not recommended.
It should be taken into account that a single dose of powder (5 g) contains 1.936 g of sucrose, which is equivalent to 0.2 XE (bread unit).
Application during pregnancy and lactation
- pregnancy: you can use the drug only on strict indications in cases where the benefits of therapy for the mother outweigh the existing risk for the developing fetus;
- lactation period: taking TeraFlu Max is contraindicated.
Pediatric use
Prescribing TeraFlu Max for children and adolescents under the age of 18 is contraindicated.
With impaired renal function
Severe renal failure is an absolute contraindication to the use of the drug.
For violations of liver function
Patients with severe hepatic impairment are not prescribed TeraFlu Max.
Drug interactions
Paracetamol
The production of hydroxylated active metabolites of the substance increases under the influence of stimulants of microsomal oxidation in the liver - tricyclic antidepressants, phenylbutazone, rifampicin, barbiturates, ethanol and phenytoin. This is due to the risk of severe poisoning with small overdoses.
TeraFlu Max reduces the effectiveness of diuretics, enhances the effects of sedatives, ethanol and monoamine oxidase inhibitors. Combination therapy with phenothiazine derivatives, antidepressants, antipsychotic and antiparkinsonian drugs is accompanied by a high probability of constipation, dry mouth and urinary retention.
Cholestyramine reduces the rate of absorption of paracetamol, while domperidone and metoclopramide, on the contrary, increase.
Phenylephrine
With the combined use of phenylephrine and guanethidine, the hypotensive effect of the second decreases and the alpha-adrenostimulating effect of the first increases.
When used with glucocorticosteroids, the likelihood of glaucoma increases.
Tricyclic antidepressants enhance the sympathomimetic properties of the drug.
Vitamin C
Recommended combinations to take into account:
- iron, penicillin: their absorption increases;
- indirect anticoagulants, heparin: their effectiveness decreases;
- salicylates: the risk of crystalluria increases;
- oral contraceptives: absorption of vitamin C decreases;
- acids: their excretion by the kidneys slows down;
- drugs with an alkaline reaction, including alkaloids: their reabsorption decreases;
- deferoxamine: increases the toxic effect of iron on tissues (especially on the heart, which leads to heart failure). Taking preparations containing vitamin C in such cases is possible only after determining the excretion of iron and the content of deferoxamine, but not earlier than 2 hours after its administration.
