Spazmalin is a spasmodic analgesic.
Release form and composition
Dosage forms of Spazmalin:
- tablets: yellowish-white or white, round, flat, on one side there is a dividing line (10 pcs. in blisters, in a cardboard box 2 or 10 blisters);
- solution for injection: clear liquid, from yellow to light yellow (5 ml each in dark glass ampoules, in blisters of 5 ampoules, in a cardboard box of 5 blisters).
1 tablet contains:
- active ingredients: metamizole sodium - 500 mg, pitofenone hydrochloride - 5 mg, fenpiverinium bromide - 0.1 mg;
- auxiliary components: colloidal silicon dioxide, sodium starch glycolate, magnesium stearate, microcrystalline cellulose, talc.
1 ml of solution contains:
- active ingredients: metamizole sodium - 500 mg, pitofenone hydrochloride - 2 mg, fenpiverinium bromide - 0.02 mg;
- auxiliary components: citric acid monohydrate, water for injection.
Pharmacological properties
Pharmacodynamics
The antispasmodic and analgesic properties of Spazmalin are provided by its combined composition.
Metamizole sodium - analgesic-antipyretic, pyrazolone derivative; has a pronounced non-narcotic analgesic, antipyretic and weak anti-inflammatory effect. Its therapeutic effect is based on the inhibition of prostaglandin synthesis.
Pitofenone is a myotropic antispasmodic, has a direct effect on the smooth muscles of the internal organs, causes its relaxation.
Fenpiverinium bromide is an m-anticholinergic antagonist and contributes to additional antispasmodic effects on smooth muscles.
Indications for use
- pain syndrome with spasms of smooth muscles of internal organs: biliary colic, renal colic, algodismenorrhea;
- symptomatic short-term therapy: arthralgia, neuralgia, sciatica, myalgia.
- condition after surgery and diagnostic interventions - as an aid to relieve pain.
In addition, the use of tablets is indicated for migraines and headaches.
Contraindications
- severe liver dysfunction;
- severe renal dysfunction;
- diseases of the hematopoietic system;
- granulocytopenia;
- deficiency of glucose-6-phosphate dehydrogenase;
- angle-closure glaucoma;
- tachyarrhythmia;
- benign prostatic hyperplasia;
- acute intermittent porphyria;
- collaptoid states;
- intestinal obstruction;
- megacolon;
- I trimester and the second half of the III trimester of pregnancy;
- breast-feeding;
- age up to 3 months or the patient's body weight is less than 5 kg;
- hypersensitivity to pyrazolone derivatives (including butadione), analgesics-antipyretics or nonsteroidal anti-inflammatory drugs (NSAIDs);
- individual intolerance to the components of the drug.
Care should be taken when prescribing Spazmalin to patients who are prone to arterial hypotension and bronchospasm, with impaired liver and (or) kidney function, under the age of 15 years.
Instructions for the use of Spazmalin: method and dosage
Pills
Spazmalin tablets are taken orally, after meals.
The dose of the drug is prescribed taking into account the age and clinical condition of the patient.
Recommended dosage for patients over 15 years old: 1–2 pcs. 2-3 times a day. The daily dose should not exceed 6 pcs. The duration of the course is no more than 5 days. An increase in the daily dose or prolongation of the course of therapy is possible only under the close supervision of a physician.
For children under 15 years of age, the drug can be prescribed only under strict indications and under medical supervision.
The recommended dosage for a single dose of Spazmalin:
- children aged 6–8 years: 1/2 pcs.;
- children aged 9–12 years: 3/4 pcs.;
- children aged 13-15: 1 pc.
Frequency rate of admission - 2-3 times a day.
Injection
Parenteral administration of the drug is indicated in emergency cases, including renal colic, biliary colic, and in conditions of the patient that do not allow taking tablets by mouth (including in case of impaired absorption from the gastrointestinal tract).
The solution is intended for intramuscular (i / m) and intravenous (i / v) administration.
Do not mix the Spazmalin solution in the same syringe with other drugs.
The dose is prescribed taking into account the patient's age.
The recommended dosage of Spazmalin for adults and adolescents over 15 years old:
- IV injection (slowly, in a horizontal position of the patient, monitoring the heart rate, blood pressure (BP), respiratory rate): 2 ml once;
- IM administration: 2–5 ml 2-3 times a day, but not more than 10 ml.
The duration of the course is no more than 5 days.
Due to the risk of a sharp decrease in blood pressure when prescribing a dose of 2 ml or more, special care is required.
The dose for children is determined in strict accordance with the age of the child.
Recommended dosage for a single intramuscular or intravenous injection of Spazmalin solution:
- Infants 3-11 months: 0.1 ml;
- children aged 1-4: 0.2 ml;
- children aged 5-7: 0.4 ml;
- children aged 8-11: 0.5 ml;
- children aged 12-14 years: 0.8 ml.
If it is necessary to re-administer Spazmalin, the procedure is carried out in the initial dose.
Side effects
- on the part of the cardiovascular system: possibly - tachycardia, lowering blood pressure, cyanosis;
- from the central nervous system: very rarely - accommodation paresis; isolated cases - headache; possibly dizziness;
- from the digestive system: isolated cases - dry mouth, burning sensation in the epigastric region;
- on the part of the urinary system: rarely (usually against the background of prolonged therapy or the use of high doses of the drug) - oliguria, proteinuria, renal dysfunction, anuria, urine staining red, interstitial nephritis; very rarely - urinary retention;
- on the part of the hematopoietic system: isolated cases (against the background of long-term use) - leukopenia, thrombocytopenia, agranulocytosis;
- allergic reactions: in patients with a predisposition to bronchospasm - provoking an attack; sometimes - a rash, itchy skin; very rarely - anaphylactic shock; isolated cases - Stevens-Johnson syndrome or malignant exudative erythema, Lyell's syndrome (toxic epidermal necrolysis);
- others: very rarely - decreased sweating.
Overdose
Symptoms: pain in the epigastric region, nausea, vomiting, drowsiness, lowering blood pressure, convulsions, functional impairment of the kidneys and liver, confusion.
Treatment: immediate gastric lavage, intake of activated charcoal. If necessary, the appointment of symptomatic therapy.
special instructions
The use of Spazmalin in therapeutic doses is usually well tolerated and does not cause the development of undesirable effects.
If the patient has thrombocytopenia or suspected agranulocytosis, immediate discontinuation of the drug is required.
If it is necessary to use the drug for more than 7 days, the patient should ensure careful monitoring of the functional state of the liver and the picture of peripheral blood.
During the course of therapy with Spazmalin, alcohol consumption is contraindicated.
Influence on the ability to drive vehicles and complex mechanisms
Due to the risk of developing side effects that affect the concentration of the patient's attention, during the period of using the drug, potentially hazardous activities, including driving vehicles and mechanisms, should be avoided.
Application during pregnancy and lactation
According to the instructions, Spazmalin is contraindicated for use in the first trimester and the second half of the third trimester of the pregnancy period.
Do not use Spazmalin solution in ampoules or tablets during breastfeeding. If it is absolutely necessary to prescribe the drug during lactation, breastfeeding should be temporarily stopped.
Pediatric use
The appointment of Spazmalin is contraindicated in children under the age of 3 months or with a body weight of up to 5 kg.
Since the appointment of the drug to children under the age of 15 is indicated only under strict indications, the doctor makes the decision on its use in each case individually.

With impaired renal function
The drug should not be prescribed to patients with severe renal impairment.

With mild and moderate liver dysfunction, the drug should be used with caution, under the close supervision of a physician.
Drug interactions
With simultaneous use with Spazmalin:
- non-opioid analgesics: can cause mutual reinforcement of action;
- inducers of microsomal liver enzymes, including phenylbutazone and barbiturates: cause a weakening of the pharmacological action of metamizole sodium;
- tricyclic antidepressants, allopurinol, oral contraceptives: contribute to the disruption of the metabolism of metamizole sodium in the liver, increasing its toxicity;
- cyclosporine: lowers the level of its concentration in blood plasma;
- butyrophenones, phenothiazines, quinidine, amantadine, blockers of histamine H1 receptors: can enhance the m-cholinolytic effect of the drug;
- tranquilizers, sedatives: enhance the analgesic effect of the drug;
- ethanol: may cause synergy.
