Lugol spray is an antiseptic agent for topical use in ENT practice.
Release form and composition
The drug is produced in the form of a spray for local use: a viscous transparent, red-brown liquid, with the smell of iodine (25, 30, 50 or 60 g each in a polymer / orange glass bottle, sealed with a lid with a dispenser; in a cardboard box 1 bottle included with a spray and instructions for the use of Lugol spray).
Composition of 100 g spray:
- active substance: iodine - 1 g;
- additional components: glycerol, potassium iodide, purified water.
The main active ingredient of the drug is molecular iodine, which has antiseptic and local irritating properties. It demonstrates bactericidal action against gram-negative and gram-positive microflora, and also causes the death of pathogenic fungi (including yeast). Staphylococcus spp. are more resistant to iodine, however, with prolonged treatment with an antiseptic, inhibition of staphylococcal flora is recorded in 80% of cases. Pseudomonas aeruginosa is resistant to the action of the agent. If Lugol spray is applied to large areas of the skin and mucous membranes, iodine can have a resorptive effect - take part in the production of triiodothyronine (T3) and thyroxine (T4).
The iodide included in the potassium preparation promotes the dissolution of iodine in water, and glycerol exhibits a softening effect.
If the spray is used in recommended doses, there is a slight resorption of iodine through the mucous membranes of the oral cavity or skin. In the process of contact with mucous membranes, the active substance is converted to iodides by 30%. If accidentally swallowed, iodine is rapidly adsorbed and penetrates well into organs and tissues, accumulating in the thyroid gland.
The agent is excreted mostly by the kidneys and in smaller quantities with sweat and feces. Found in breast milk.
Indications for use
Lugol spray is recommended for use in adults and children for the treatment of diseases of the mucous membrane of the oral cavity and pharynx, of an infectious and inflammatory nature.
Contraindication to the use of the spray is hypersensitivity to iodine or other components of the product.
Caution is required to use Lugol spray for the following diseases / conditions:
- dermatitis herpetiformis;
- decompensated liver and kidney diseases;
- hyperthyroidism (thyrotoxicosis);
- age up to 12 years.

The drug is applied topically.
It is recommended to irrigate the mucous membrane of the mouth, pharynx, pharynx 4–6 times a day, injecting a spray with one press on the spray head. At the time of administration of the Lugol spray solution, you should hold your breath.
Do not allow the drug to get into the eyes. If this does happen, flush the eyes with sodium thiosulfate solution or plenty of water.
Lugol spray is not recommended for a long (over 14 days) course of therapy. In the case when, after 2-3 days of treatment, it is not possible to reduce the severity of the symptoms of inflammation or there is an increase in the latter, it is necessary to consult a doctor.
During treatment, allergic reactions may develop. With prolonged therapy, the phenomena of iodism may occur: lacrimation, rhinitis, salivation, acne, urticaria, angioedema. If this or any other side effect appears when using the spray, you should consult a doctor.
Symptoms of an overdose of the drug include irritation of the upper respiratory tract (including burns and laryngobronchospasm). In case of accidental oral administration, irritation of the mucous membranes of the digestive tract, the development of hemoglobinuria, hemolysis may occur; the lethal dose is approximately 3 g of iodine, which corresponds to ~ 300 ml of Lugol spray solution.
In case of an overdose, gastric lavage is prescribed with sodium bicarbonate solutions, 0.5% sodium thiosulfate solution, 30% sodium thiosulfate is injected intravenously at a dose of up to 300 ml.
The decomposition of active iodine is accelerated by sunlight and temperatures exceeding 40 ° C.
It is required to avoid regular use of the spray in patients with hyperthyroidism (thyrotoxicosis).
Lugol spray may affect laboratory results when conducting thyroid hormone studies.
Application during pregnancy and lactation
It is not recommended to use Lugol spray during pregnancy.
As iodine passes into breast milk, it can affect the thyroid function in infants. The use of the drug during lactation is possible only on the recommendation of the attending physician, in the case when the expected benefit of therapy for the mother significantly exceeds the possible threat to the health of the child.
Caution is required to prescribe Lugol spray for children under 12 years of age. It is recommended that you consult your pediatrician before taking it.
With impaired renal function
In the presence of decompensated kidney disease, the drug should be used with extreme caution.

Iodine is inactivated by sodium thiosulfate.
The iodine included in the spray oxidizes metals, which can cause damage to metal objects in contact with the solution.
The drug is pharmaceutically incompatible with ammonia solutions, essential oils.
The acidic / alkaline environment, the presence of blood, pus, fat reduce the antiseptic activity of the spray.
In case of accidental ingestion of Lugol spray inside, it is possible to weaken the action of drugs leading to suppression of the function of the thyroid gland, as well as to change the indicators of the latter. Iodine preparations can aggravate the irritating effect of a number of medicinal substances (including acetylsalicylic acid) on the gastrointestinal mucosa.
