Ubistezin forte is a combined local anesthetic drug containing an anesthetic and a vasoconstrictor.

Release form and composition

Dosage form Ubistezina forte - solution for injection: transparent liquid without color (1.7 ml of solution in colorless glass cartridges, sealed on both sides with corks, 50 cartridges in a tin can).

Composition of 1 ml solution:

  • active substances: articaine hydrochloride - 40 mg; epinephrine hydrochloride - 0.012 mg (which corresponds to 0.01 mg of epinephrine);
  • auxiliary components: sodium sulfite, sodium chloride, water for injection.

Pharmacological properties

Pharmacodynamics

Ubistezin forte is a combined preparation for local anesthesia used in dentistry, the action of which is due to the properties of its constituent components:

  • articaine: local anesthetic, amide type of thiaphene group. Begins to act within 1-3 minutes, the minimum time of anesthesia is 75 minutes, for soft tissues 120-240 minutes;
  • epinephrine (adrenaline): a vasoconstrictor, constricts blood vessels at the injection site, ensures slow absorption of the drug, and prolongs its effect. When using Ubistezin forte, wound healing usually proceeds without complications.

Pharmacokinetics

The diffusion capacity of articaine is high. Protein binding - 95%. Metabolized in the liver. The half-life is 25 minutes. It is excreted mainly in the urine. The substance in minimal amounts overcomes the placental barrier and is practically not excreted during lactation.

Indications for use

According to the instructions, Ubistezin Forte is indicated for infiltration and conduction anesthesia, especially in patients with severe somatic diseases, for the following manipulations:

  • uncomplicated extraction of one or more teeth;
  • filling of carious cavities, preparation of teeth before prosthetics;
  • surgical interventions on the mucous membranes of the oral cavity or on the bones, requiring a more pronounced decrease in blood supply (the effect of ischemia);
  • sinusitis according to Caldwell - Luke;
  • percutaneous osteosynthesis.

Contraindications

Absolute contraindications:

  • deficiency of cholinesterase activity;
  • hemorrhagic diathesis;
  • porphyria;
  • hyperthyroidism;
  • severe liver failure;
  • angle-closure glaucoma;
  • severe heart disease (uncontrolled congestive heart failure, unstable angina pectoris, condition after a recent myocardial infarction or coronary artery bypass grafting, severe uncontrolled arterial hypertension, hypotension, tachyarrhythmia, severe cardiac conduction disturbances);
  • bronchial asthma with hypersensitivity to sulfites;
  • inflammation at the site of planned anesthesia;
  • cardiogenic shock;
  • age up to 4 years;
  • hypersensitivity to drug components and other sulfites.

Relative contraindications (Ubistezin forte is used with extreme caution):

  • blood clotting disorders;
  • pheochromocytoma;
  • diabetes;
  • epilepsy;
  • bronchial asthma;
  • heart disease (arterial hypertension, atherosclerosis, chronic heart failure, atrioventricular and intraventricular block I degree);
  • hepatic / hepatic impairment;
  • elderly age;
  • weakened and severe general condition of the patient.

Instructions for the use of Ubistezin forte: method and dosage

Ubistezin forte is administered parenterally. Intravenous (IV) administration of the drug, as well as injection at the site of inflammation, is unacceptable.

For uncomplicated tooth extraction, 1 dose of the drug (1.7 ml) is usually sufficient for each tooth injected into the submucosa in the region of the transitional fold from the vestibular side. In some cases, to achieve complete anesthesia, additional administration of 1 to 1.7 ml of the drug is possible.

In preparation for filling or installing a crown, 0.5-1.7 ml of the drug is injected (depending on the desired depth and duration of the procedure) by the type of infiltration anesthesia from the vestibular side. For palatal anesthesia, 0.1 ml of the drug is usually sufficient.

For the removal of the lower premolars, infiltration anesthesia with 1.7 ml of Ubistezine Forte is usually recommended per tooth, which avoids mandibular anesthesia. If necessary, it is allowed to additionally inject 1–1.7 ml of the drug into the submucosa in the region of the transitional fold of the lower jaw from the vestibular side. If complete anesthesia cannot be achieved, it is advisable to perform a mandibular nerve block.

The maximum allowable dose of Ubistezine Forte is 7 mg of articaine per 1 kg of body weight (1 ml of solution contains 40 mg of articaine).

Side effects

  • central nervous system (CNS): headache; muscle tremors and / or muscle twitching, up to generalized seizures; respiratory failure, up to a stop; violation of consciousness, up to its loss;
  • digestive organs: nausea, vomiting, diarrhea;
  • organ of vision: blurred eyes, transient blindness, diplopia;
  • cardiovascular system: hemodynamic disorders [decrease or increase in blood pressure (BP), bradycardia, tachycardia, arrhythmia], suppression of cardiovascular activity, up to collapse and cardiac arrest;
  • allergic reactions: edema and inflammation at the injection site, itching of the conjunctiva, redness and itching, angioedema, anaphylactic shock.

At the site of administration of Ubistezin forte, ischemic zones may appear, and with accidental intravascular administration, tissue necrosis is likely to develop.

Overdose

Symptoms of an Ubistezine Forte overdose are dizziness, impaired consciousness, motor restlessness, lowering blood pressure, tachycardia, bradycardia.

When symptoms of an overdose appear, the injection must be stopped immediately, the patient must be given a horizontal position, and the airway must be free. Blood pressure and heart rate (HR) should be kept under control.

Recommendations for symptomatic treatment of overdose:

  • breathing disorder: oxygen, endotracheal intubation, artificial ventilation of the lungs (central analeptics are contraindicated);
  • convulsions: shown in / in the slow administration of short-acting barbiturates, oxygen, hemodynamic control;
  • severe circulatory disorders, shock: intravenous infusion solution of electrolytes, plasma substitutes, glucocorticosteroids, albumin;
  • vascular collapse and increasing bradycardia: IV, and then IV drip epinephrine (initial dose - 0.1 mg), control of heart rate and blood pressure;
  • severe tachycardia and tachyarrhythmia: intravenous selective beta-blockers;
  • increased blood pressure: peripheral vasodilators.

Eating after the completion of surgical procedures is recommended after the restoration of sensitivity.

Use only new and sterile syringes and needles each time you draw up solution. An open cartridge should not be reused due to the risk of hepatitis infection. It is also unacceptable to use the drug from a damaged cartridge.

It is also unknown whether articaine and epinephrine are excreted in breast milk, and therefore requires a careful assessment of the benefit / risk ratio when deciding whether to continue breastfeeding while using the drug.

In the case when it is decided to continue lactation after anesthesia with Ubistezin Forte, it is recommended to skip one feeding, replacing it with an artificial one, while milk should be expressed and not used.

Pediatric use

Recommendations for dosage of the drug in children aged 4 to 18 years, depending on body weight:

  • 20-30 kg: 0.25-1 ml;
  • 30–45 kg: 0.5–2 ml.

Recommended maximum daily dose, depending on body weight:

  • 20-30 kg: 3.5 ml;
  • 30-40 kg: 5.25 ml;
  • 40–45 kg: 7.0 ml;
  • 45-50 kg: 7.9 ml;
  • 50-60 kg: 8.7 ml;
  • 60–70 kg: 10.5 ml;
  • 70-80 kg: 12.2 ml.

In children, infiltration anesthesia may be used instead of conduction anesthesia, due to the lower bone density.

Drug interactions

The use of Ubistezine forte in conjunction with non-selective beta-blockers, monoamine oxidase inhibitors and tricyclic antidepressants is contraindicated.

It is recommended to use the drug with caution in combination with phenothiazine, anticoagulants, antiplatelet agents, cardioselective beta-blockers.

Vasoconstrictor drugs enhance and lengthen the local anesthetic effect of articaine.
