Blogir-3 is a blocker of histamine H 1 receptors. It has an anti-allergic effect.
Release form and composition
Blogir-3 is available in the following dosage forms:
- tablets dispersed in the oral cavity: flat, round, with beveled edges, pink with darker blotches, on one side with embossed "5" (5 pcs. in aluminum foil blisters, in a cardboard box 1 blister; 10 pcs.. in blisters made of aluminum foil, in a cardboard box 1, 2 or 3 blisters);
- syrup: a transparent colorless solution (60 or 120 ml in dark glass vials sealed with a plastic screw cap, in a cardboard box 1 bottle complete with a dosing spoon, graduated to 2.5 ml and 5 ml).
Each pack also contains instructions for the use of Blogir-3.
1 tablet contains:
- active ingredient: desloratadine - 5 mg;
- auxiliary components: croscarmellose sodium, aspartame, citric acid monohydrate, mannitol, potassium polacrilin, magnesium stearate, red iron oxide dye, microcrystalline cellulose, tutti-frutti flavor (maltodextrin, modified starch E1450, propylene glycol substances E1520), flavoring agents.
1 ml of syrup contains:
- active ingredient: desloratadine - 0.5 mg;
- auxiliary components: sorbitol, sucralose, citric acid, propylene glycol, sodium citrate, hypromellose, tutti-frutti flavor (propylene glycol E1520, fragrances), water.
Pharmacological properties
Pharmacodynamics
Blogir-3 is a long-acting non-sedating antihistamine. Desloratadine is the primary active metabolite of loratadine, which selectively blocks the activity of peripheral H 1 -histamine receptors. It inhibits a whole cascade of reactions characteristic of allergic inflammation: the release of chemokines and cytokines (including interleukins IL-4, IL-6, IL-8 and IL-13), the synthesis of superoxide anions (produced by polymorphonuclear neutrophils), chemotaxis and eosinophil adhesion, IgE-mediated release of histamine, leukotriene C4 and prostaglandin D2, release of P-selectin and other adhesion molecules.
Thus, Blogir-3 prevents the occurrence of allergic reactions and facilitates their course, reduces capillary permeability, has an anti-exudative and antipruritic effect, prevents smooth muscle spasm and tissue edema.
Desloratadine does not affect the central nervous system and the speed of psychomotor reactions; there is practically no sedation (the drug does not cause drowsiness). Clinical and pharmacological studies have shown that in the recommended therapeutic doses, desloratadine does not lengthen the QT interval on the electrocardiogram.
The action of Blohir-3 begins within half an hour after taking a pill or syrup and continues for 24 hours.
Pharmacokinetics
Desloratadine is well absorbed in the digestive tract and is found in the blood approximately 30 minutes after taking Blohir-3 inside. The maximum plasma concentration is reached after 3 hours.
The drug is 83–87% bound to plasma proteins. After the use of desloratadine at a dose of 5–20 mg once a day for 2 weeks in adolescents and adult patients, no clinically significant cumulation of the active substance was detected. Grapefruit juice and other food did not affect the distribution of Blogir-3 (when taking 7.5 mg once a day). Desloratadine does not cross the blood-brain barrier.
The metabolism is intense, occurs in the liver. The main route is hydroxylation. As a result of metabolism, 3-OH-desloratadine is formed, combined with glucuronide. Desloratadine does not inhibit the isoenzymes CYP2D6 and CYP3A4, nor is it a substrate of P-glycoprotein and does not inhibit it.
The drug is excreted mainly in the form of a glucuronide compound. In unchanged form, about 9% of desloratadine is excreted (less than 2% by the kidneys and less than 7% through the intestines). The half-life is approximately 27 hours.
Indications for use
Blogir-3 is used to treat allergic rhinitis (to relieve or eliminate nasal congestion, sneezing, itching in the nose, watery eyes, nasal mucus, redness and itching of the eyes, itching of the palate) and urticaria (to eliminate or reduce rash and itching) …
Contraindications
Absolute:
- hereditary fructose intolerance (for syrup, since it contains sorbitol);
- phenylketonuria (for tablets);
- children under 1 year of age (for syrup) and up to 12 years (for tablets);
- pregnancy;
- breast-feeding;
- hypersensitivity to the components of the drug or loratadine.
In patients with severe renal failure, Blogir-3 tablets and syrup are used with caution.
Blogir-3, instructions for use: method and dosage
Oral Dispersible Tablets
In the form of tablets, Blogir-3 should be taken orally, regardless of food.
The drug should not be removed from the blister using normal pressing, as this method may damage the tablet. To remove the tablet, you must open the blister by carefully removing the protective film. The tablet should be put on the tongue and wait for it to dissolve (usually within a few seconds), and then swallowed with saliva. The drug is taken immediately after being removed from the blister. The tablet does not need to be taken with water or other liquid.
The usual dose of Blogir-3 for adolescents over 12 years of age and adults is 5 mg (1 tablet) once a day.
Syrup
In the form of a syrup, Blogir-3 should be taken orally, regardless of food, with a small amount of liquid.
Recommended doses:
- children aged 1–5 years: 2.5 ml (1.25 mg) once a day;
- children aged 6-11: 5 ml (2.5 mg) once a day;
- adolescents over 12 years old and adults: 10 ml (5 mg) once a day.
Patients with intermittent (seasonal) allergic rhinitis (symptom duration less than 4 days per week or less than 4 weeks per year) should be assessed for the course of the disease. Reception of Blogir-3 is stopped after the disappearance of the signs of rhinitis, and if the symptoms recur, treatment is started again.
Persistent (year-round) allergic rhinitis (the duration of symptoms is more than 4 days a week or more than 4 weeks a year) requires constant treatment, that is, Blogir-3 is taken throughout the entire period of action of the allergen.
Side effects
Most often in adolescents over 12 years old and adults, Blogir-3 tablets caused such side effects as increased fatigue (1.2%), headache (0.6%) and dry mouth (0.8%). As a result of clinical studies of the syrup, it was shown that the following adverse reactions were observed in children under 2 years of age (their frequency slightly exceeded the frequency when using placebo): fever, diarrhea, insomnia. The incidence of these side effects in children 2–11 years old did not differ from that with placebo. According to the results of clinical studies, adolescents 12–17 years old during treatment with syrup most often developed headaches.
Blogir-3 can cause adverse reactions from the following systems and organs:
- gastrointestinal tract: often (≥ 1/100 and <1/10) - dry mouth; very rarely (<1/10 000) - vomiting, nausea, dyspeptic disorders, abdominal pain, diarrhea;
- hepatobiliary system: very rarely - hepatitis, increased levels of bilirubin and the activity of liver enzymes; with an unknown frequency (available data do not allow us to accurately estimate the incidence of a side effect) - jaundice;
- cardiovascular system: very rarely - palpitations, tachycardia; with an unknown frequency - bradycardia and arrhythmia (in children), prolongation of the QT interval;
- central nervous system and psyche: often - headaches; very rarely - insomnia, dizziness, hallucinations, drowsiness, convulsions, psychomotor hyperactivity;
- musculoskeletal system: very rarely - muscle pain;
- allergic reactions: very rarely - rash (including nettle), itching, Quincke's edema, anaphylaxis; with an unknown frequency - photosensitization;
- other disorders: often - increased fatigue; very rarely - shortness of breath; with an unknown frequency - asthenia.
Overdose
As a result of taking a dose of Blogir-3, exceeding the recommended dose by 5 times, no symptoms of intoxication were found. Clinical trials, in which desloratadine was used in adults and adolescents in a daily dose of up to 20 mg for two weeks, also did not reveal any clinically or statistically significant changes in the cardiovascular system. In one of the clinical and pharmacological studies, when the drug was used in a daily dose of 45 mg (this dose is 9 times the recommended) for 10 days, there was no prolongation of the QT interval and there were no serious adverse reactions.
If the patient accidentally took inside a large amount of Blogir-3 tablets or syrup, it is necessary to consult a doctor; the patient should wash the stomach and give activated charcoal. If necessary, symptomatic treatment is prescribed. Hemodialysis is ineffective. The feasibility of peritoneal dialysis has not been established.

Special studies of the effectiveness of Blogir-3 in rhinitis of infectious origin have not been conducted.
Influence on the ability to drive vehicles and complex mechanisms
Reception of Blogir-3 in recommended doses did not affect the patient's ability to drive vehicles and work with other complex and potentially dangerous mechanisms. However, it should be borne in mind that in rare cases, desloratadine can cause drowsiness, which can affect the patient's psychomotor functions.
Application during pregnancy and lactation
Blogir-3 is prohibited to use during pregnancy, as there is no data on its safety for the fetus.
Because desloratadine is secreted in breast milk, it should not be taken by women who are breastfeeding.
Pediatric use
In the form of tablets, Blogir-3 is contraindicated in children under 12 years of age (since the drug may be taken incorrectly at a younger age), and in the form of a syrup - for babies under 1 year old (since there is no data on the safety and effectiveness of desloratadine for children under 1 year of age).
With impaired renal function
Desloratadine is used with caution in patients with severe renal impairment.
Drug interactions
With the simultaneous use of Blogir-3 with azithromycin, ketoconazole, cimetidine, erythromycin and fluoxetine, no clinically significant drug interaction has been identified.
Grapefruit juice or food does not affect the effectiveness of desloratadine.
Blogir-3 does not enhance the effect of ethanol on the nervous system. However, in the course of post-marketing use, there have been isolated cases of alcohol intoxication and intolerance to alcoholic beverages, so desloratadine should be used with caution in conjunction with alcohol.

There are not very many reviews of Blogyr-3, however, in almost all reports, patients share a positive experience of using this drug. The drug is effective and works well for allergic rhinitis. The tablets are easy to dissolve, they do not need to be washed down with water. The drug tastes good and is not addictive.
Sometimes side effects may occur, such as lethargy, drowsiness, and allergic reactions. The cost of the antihistamine is adequate, but, nevertheless, there are cheaper analogues.
