Efoks belongs to the groups of peripheral vasodilators and antianginal drugs.

Pharmacological action of Efoks

Efoks is an organic nitrate, the therapeutic effect of which extends mainly to the venous vessels. The action of the drug is aimed at stimulating the formation of nitric oxide in the vascular endothelium. In turn, nitric oxide promotes the activation of intracellular guanylate cyclase, thereby increasing cGMP.

Efoks is able to reduce preload and afterload on the myocardium, thereby reducing its oxygen demand. Also, the drug has the ability to have a coronary dilating effect.

The use of Efoks leads to a decrease in blood flow to the right atrium, as a result of which there is a regression of a number of symptoms in pulmonary edema and a decrease in pressure in the pulmonary circulation. In addition, the use of Efoks has a positive effect on the distribution of coronary blood flow in areas with reduced blood circulation.

While taking Efoks, there is an increase in exercise tolerance in patients with angina pectoris and coronary artery disease, vasodilatation of the hard shell of the GM (pain reactions may occur).

Tolerance develops to the drug, however, after its cancellation or with a break in treatment, sensitivity to Efoks is soon restored.

The antianginal effect of taking occurs 35-40 minutes after the application of Efoks, its duration is from 8 to 10 hours.

Release form

Efoks is available in the form of tablets, flat and round. The color of the tablets is white, on one side there is a chamfer and a notch for breaking. Above the notch, engraving "E" is discernible, under the notch - engraving "20".

Indications for the use of Efoks

According to the instructions for Efoks, the drug is prescribed for the prevention of angina attacks in patients with coronary artery disease (including patients with previous myocardial infarction), as well as in chronic heart failure, as an element of combination therapy.

Contraindications to the use of Efoks

Efoks is not recommended for use in the following cases:

  • In acute circulatory disorders (including vascular collapse and shock);
  • In cardiogenic shock, if the correction of left ventricular diastolic pressure with the help of intra-aortic counterpulsation or with the introduction of agents affecting the inotropic effect causes difficulties;
  • With severe arterial hypotension (if the systolic pressure is less than 90 mm Hg, and the diastolic pressure is less than 60 mm Hg), with hypovolemia;
  • When taken simultaneously with phosphodiesterase inhibitors (including sildenafil, tadalafil, vardenafil), because they enhance the hypotensive effect of taking Efoks;
  • With myocardial infarction in acute form, accompanied by arterial hypotension;
  • With anemia in severe form;
  • With hereditary galactose intolerance, lactase deficiency, with malabsorption of galactose and glucose;
  • During breastfeeding;
  • With toxic pulmonary edema;
  • If the patient is less than 18 years old;
  • with hypersensitivity to any of the components of the Efoks composition, in particular, to organic nitrates.

In some cases, the drug is permissible for use, however, careful medical supervision is required during the entire course of treatment. These conditions include:

  • Constrictive pericarditis;
  • Hypertrophic obstructive cardiomyopathy;
  • Pericardial tamponade;
  • Mitral and / or aortic stenosis;
  • Acute myocardial infarction and other conditions that reduce pressure in the left ventricle;
  • Tendency to arterial hypotension;
  • Glaucoma, because there is a risk of increased intraocular pressure;
  • Heart failure, accompanied by low filling pressure of the left ventricle;
  • Diseases accompanied by increased intracranial pressure, including hemorrhagic stroke and recent head trauma;
  • Renal and / or hepatic impairment;
  • Severe anemia.

Efoks tablets are administered orally, after meals. You should not chew the medicine; you must drink plenty of water.

The dosage of taking Efoks, as well as the duration of treatment and repeated courses, is set by the doctor, focusing on the patient's condition and the severity of the disease.

The instructions for Efoks are recommended to start using the drug with small doses: 10 mg of the drug twice a day. The dose is subsequently recommended to be increased to a therapeutic one: from 20 to 40 mg, divided into 1-2 doses per day. If necessary, the daily dose can be increased to 60 mg. The maximum dosage of Efoks per day should not exceed 80 mg.

Side effects of Efoks

The instruction for Efoks warns of the possibility of adverse reactions during drug treatment. Among them are noted:

  • Headache that occurs at the beginning of the use of Efoks. More often than not, it goes away with time. Dizziness, fever, facial flushing, tachycardia, decreased blood pressure, lethargy, increased heart rate, feeling of weakness, orthostatic collapse, increased angina attacks, collaptoid states, fainting are also noted;
  • Vomiting, nausea, dry mouth, a slight burning sensation of the tongue;
  • Blurred vision, drowsiness, stiffness, decreased speed of mental and motor reactions;
  • Skin rash;
  • Exfoliative dermatitis;
  • Development of tolerance.

Overdose with Efoks

The use of Efoks in doses exceeding those recommended by the doctor can lead to an overdose, the symptoms of which are presented below:

  • Increased heart rate;
  • Weakness;
  • Decrease in blood pressure, accompanied by orthostatic dysregulation;
  • Dizziness;
  • Headache;
  • Lethargy;
  • Nausea;
  • Diarrhea;
  • Collapse;
  • Convulsions;
  • Sweating;
  • Dyspnoea;
  • Paralysis;
  • Coma.

Recommended treatment in the form of gastric lavage, fluid injection. In some cases, it is possible to administer dopamine and / or norepinephrine as an infusion. Epinephrine administration is prohibited.

Efoks antidotes are:

  • Vitamin C in the form of sodium salt. 1 g i / v;
  • Methylene blue. Introduced up to 50 ml of 1% solution in the form of intravenous infusion;
  • Toluidine blue. First, from 2 to 4 mg per 1 kg of body weight is administered intravenously. Re-introduction is possible after an hour at a minimum dosage;
  • Hemodialysis, oxygen therapy, exchange blood transfusion.

Treatment with Efoks is incompatible with alcohol intake.

During the use of the drug, you should refuse to drive vehicles and perform other activities that require rapid psychomotor reactions.

Abrupt cancellation of Efoks is unacceptable. The dosage should be reduced gradually.

The development of symptoms of intolerance is the reason for a complete refusal to take the drug.
