Ortofen is a non-steroidal anti-inflammatory drug (NSAID), a phenylacetic acid derivative with a pronounced analgesic, anti-inflammatory and moderate antipyretic effect.
Release form and composition
Dosage forms:
- Enteric-coated tablets (in a blister strip: 10 pcs., 3 packs in a cardboard box, 15 pcs., 2 packs in a cardboard box);
- Solution for intramuscular (i / m) administration (3 ml in ampoules, in a contour package 5 ampoules, in a cardboard box 1 or 2 packages);
- Ointment for external use 2% (20, 30 or 50 g in aluminum tubes, in a cardboard box 1 tube);
- Gel for external use 5%: with a yellow tint or a colorless transparent mass with a characteristic ethanol odor, air bubbles and opalescence are possible (30 or 50 g each in aluminum tubes, 1 tube in a cardboard box).
The active substance of Ortofen is diclofenac sodium:
- 1 tablet: 0.025 g;
- 1 ml solution: 0.025 g;
- 1 g ointment: 0.02 g;
- 1 g gel: 0.05 g.
Auxiliary components of the gel: propylene glycol, ethanol 95%, hyetellose, purified water.
Indications for use
- Chronic and degenerative pathologies of the musculoskeletal system of inflammatory etiology (osteoarthritis, osteochondrosis, periarthropathies);
- Articular syndrome: gout, ankylosing spondylitis, rheumatoid arthritis, osteoarthritis;
- Inflammation of the musculoskeletal system and soft tissues during sprains and bruises;
- Neuralgia, back pain, arthralgia, myalgia;
- Inflammation and pain after surgery and injury;
- Pain with gout;
- Migraine;
- Algodismenorrhea;
- Pain syndrome with proctitis, adnexitis;
- Biliary and renal colic;
- Pain syndrome in infectious and inflammatory otorhinolaryngological diseases.
Contraindications
- Exacerbation of erosive and ulcerative lesions of the gastrointestinal tract (GIT);
- Aspirin triad;
- Violations of the hematopoiesis process of unknown origin;
- Age up to 6 years;
- Hypersensitivity to drug components and other NSAIDs.
With extreme caution, Ortofen should be used in patients with a history of kidney, liver, gastrointestinal tract diseases, patients with dyspeptic symptoms, bronchial asthma, heart failure, arterial hypertension, in the period after major surgical operations, in old age.
During pregnancy and breastfeeding, use is possible only as directed by a doctor in special cases, if the expected benefit to the mother outweighs the potential threat to the fetus or child.
Method of administration and dosage
The dosage form of the drug, the dose and the period of use is prescribed by the doctor based on clinical indications.
The tablets are intended for oral administration.
Recommended dosage:
- Adults: 1-2 tablets 2-3 times a day, the daily dose should not exceed 6 tablets;
- Children 6 years of age and older: the daily dose is determined at the rate of 0.002 g per 1 kg of the child's weight.
To relieve severe pain in the treatment of acute or chronic pathologies, intramuscular administration of Ortofen at a dose of 0.075 g is indicated.
The gel and ointment are applied externally, applying a thin layer to painful areas of the body. Frequency rate of procedures 3-4 times a day for 2-4 g of the drug.
Side effects
Enteric-coated tablets, solution for (i / m) administration
- Digestive system: discomfort and pain in the epigastrium, nausea, vomiting, constipation, diarrhea, flatulence, anorexia; in some cases - erosion, bleeding, ulcerative lesions and perforation of the gastrointestinal tract; rarely - liver dysfunction;
- Hematopoietic system: rarely - anemia, thrombocytopenia, leukopenia, agranulocytosis;
- Nervous system: dizziness, agitation, headache, insomnia, tired feeling, irritability; rarely - paresthesia, diplopia, blurred vision, tinnitus, convulsions, tremors, depression, mental disorders;
- Urinary system: rarely - impaired renal function; possibly - edema (in patients with a predisposition);
- Allergic reactions: itching, rash;
- Dermatological reactions: rarely - hair loss;

- Rarely: burning, itching, rash, redness;

For patients with a history of allergic reactions to sulfites and NSAIDs, the use of diclofenac is indicated only in urgent cases with mandatory systematic monitoring of the peripheral blood picture, kidney and liver function.
During the period of using the tablets and solution, you must not drink alcohol.
The cream and gel are contraindicated to be applied to areas with compromised skin integrity.
Avoid getting the drug in the eyes or on the mucous membranes of the body.
During the period of treatment with tablets and solution, the speed of psychomotor reactions may decrease, therefore, patients are advised to drive vehicles and mechanisms with caution.
Drug interactions
With the simultaneous use of Ortofen:
- Antihypertensive drugs - can weaken their therapeutic effect;
- Glucocorticosteroids - increase the risk of side effects from the digestive system;
- Diuretics - reduce their effect;
- Potassium-sparing diuretics - increase the level of potassium concentration in the blood;
- Hypoglycemic drugs - the development of hyperglycemia or hypoglycemia in patients with diabetes is possible;
- Acetylsalicylic acid - reduces the concentration of diclofenac in blood plasma;
- Warfarin - in some cases, it can lead to bleeding;
- Colestyramine, colestipol - reduce the absorption of diclofenac from the gastrointestinal tract;
- Methotrexate - may increase toxicity due to an increase in its concentration in blood plasma;
- Digoxin, lithium, phenytoin - an increase in their concentration in blood plasma is possible;
- Cyclosporine - increases its nephrotoxicity;
- Rifampicin - may decrease the concentration of diclofenac in blood plasma;
- Pentazocine - causes the risk of developing a large seizure;
- Ceftriaxone - increases its excretion in bile;
- Morphine - increases the risk of respiratory depression and other side effects of the morphine metabolite.
The risk of seizures in patients increases when Ortofen is combined with quinolone antibacterial agents.
The combination of the drug with other NSAIDs increases the risk of unwanted effects.
