Aponil is a drug from the NSAID group that has antipyretic, analgesic and anti-inflammatory effects.

Release form and composition

Dosage form Aponil - tablets: white with a yellowish tinge, round flat shape, on one side with a risk (in a cardboard box 1, 2 or 10 blisters of 10 tablets).

Composition of 1 tablet:

• active substance: nimesulide - 100 mg;
• auxiliary components: lactose monohydrate, magnesium stearate, hydrogenated vegetable oil, microcrystalline cellulose, sodium docusate, starch, hydroxypropyl cellulose, sodium glycolate.

Pharmacological properties

Pharmacodynamics

Aponil is a non-steroidal anti-inflammatory drug. It has anti-inflammatory, antipyretic and analgesic effects. It is a selective inhibitor of COX-2 (cyclooxygenase-2).

The mechanism of action of Aponil is based on the selective inhibition of COX-2, which leads to a decrease in the synthesis of prostaglandins in the focus of inflammation, as well as in the hypothalamus. It is well tolerated, since it affects the synthesis of prostaglandins in the gastric mucosa and in the kidneys much less pronounced.

Other properties of nimesulide:

• inhibition of lipid peroxidation and the formation of free oxygen radicals;
• decrease in the release of histamine, the formation of platelet activating factor and tumor necrosis alpha, proteinases and some other inflammatory mediators.

Pharmacokinetics

After oral administration, Nimesulide is well absorbed from the gastrointestinal tract. C _max (maximum concentration) in blood plasma is achieved in 1–2 hours, up to a maximum of 2.5 hours. Food intake reduces the rate of absorption, while its degree is not affected.

Binds to plasma proteins at a level of 99%.

V _d (volume of distribution) ranges from 0.19 to 0.35 l / kg. The therapeutic plasma concentration of nimesulide is maintained for 6-8 hours. With prolonged use, cumulation is not observed.

Nimesulide is biotransformed in the liver, with the formation of an active (hydroxynimesulide, less than 25% of the dose) and several inactive metabolites.

T _1/2 (half-life) is 2-3 hours. The substance is excreted in the urine (from 65 to 98% of the dose over 24 hours) and in the feces (up to 35% of the dose).

Indications for use

• Pain syndrome of various etiologies (dental, headache, postoperative, post-traumatic pain, algomenorrhea);
• Fever;
• Inflammatory diseases of the musculoskeletal system (myositis, arthritis, osteoarthritis, bursitis, tendovaginitis).

Contraindications

• Ulcerative erosive lesions of the gastrointestinal tract during an exacerbation;
• Aspirin asthma;
• Gastrointestinal bleeding;
• Severe violations of the liver and kidneys;
• Pregnancy in the third trimester;
• Lactation period;
• Age under 12;
• Hypersensitivity to the components of Aponil.

Instructions for use of Aponil: method and dosage

Aponil tablets are taken 2 times a day after meals with water.

For adults, the dose of the drug is from 50 to 200 mg at a time, on average - 100 mg.

For children from 12 years old, the drug is prescribed at the rate of 1.5 mg / kg of body weight, but not more than 100 mg.

The maximum allowable daily dose of Aponil is 400 mg for an adult and 5 mg / kg of body weight for a child.

Side effects

• From the organs of the digestive system: pain in the stomach, ulcers on the gastrointestinal mucosa, nausea and vomiting, heartburn, diarrhea;
• From the nervous system: drowsiness, headache, dizziness;
• From the urinary tract: rarely - edema, fluid retention in the body, blood in the urine, oliguria (decrease in the amount of urine);
• From the hematopoietic system: anemia, agranulocytosis, leukopenia, thrombocytopenia;
• Allergic reactions: urticaria, rash; in rare cases - anaphylactic shock.

Overdose

The main symptoms are vomiting, nausea.

Aponil tablets must be used with caution in high blood pressure, heart failure and type 2 diabetes mellitus.

In case of impaired liver and kidney function, with stomach ulcers and duodenal ulcers, lupus erythematosus, while being prescribed with diuretics and anticoagulants, Aponil should be taken in low doses.

Application during pregnancy and lactation

• III trimester of pregnancy and lactation: Aponil is not prescribed;

When conducting experimental studies, the teratogenic and embryotoxic effects of Aponil have not been established.

Pediatric use

According to the instructions, Aponil is contraindicated in patients under 12 years of age.

With impaired renal function

• severe renal dysfunction (with creatinine clearance less than 30 ml / min): therapy is contraindicated;
• renal impairment: Aponil should be used under medical supervision.

For violations of liver function

• severe liver dysfunction: therapy is contraindicated;

Drug interactions

• acetylsalicylic acid: displacement of nimesulide from the bond with blood plasma proteins occurs, while there is an increase in the concentration of free nimesulide in plasma by 2–3 times;
• hydantoin, digitalis preparations, methotrexate, cyclosporine, sulfonylurea derivatives (including oral hypoglycemic agents): nimesulide competes for binding with blood plasma proteins, and the likelihood of adverse reactions increases;
• indirect anticoagulants: there is an increase in their action;
• lithium preparations: there is an increase in the plasma concentration of lithium in the blood.
