Azarga is a combined antiglaucoma agent containing a carbonic anhydrase II inhibitor and a non-selective beta-adrenergic receptor blocker.

Release form and composition

Dosage form - eye drops: a homogeneous suspension of almost white or white color (5 ml in plastic dropper bottles "Droptainer ™", in a cardboard box 1 bottle).

1 ml drops contain:

  • Active ingredients: brinzolamide - 10 mg; timolol (in the form of maleate) - 5 mg;
  • Auxiliary components: benzalkonium chloride (50% solution), sodium chloride, sodium hydroxide, disodium edetate, tyloxapol, mannitol, purified water.

Indications for use

Azarga is used to reduce increased intraocular pressure in intraocular hypertension and open-angle glaucoma in case of ineffectiveness of monotherapy.

Contraindications

Absolute:

  • Closed-angle glaucoma;
  • Severe allergic rhinitis;
  • Bronchial hyperreactivity;
  • Chronic obstructive pulmonary disease of severe course;
  • Bronchial asthma (including history);
  • Cardiogenic shock;
  • Severe heart failure;
  • Atrioventricular block II-III degree;
  • Sinus bradycardia;
  • Severe renal failure (creatinine clearance less than 30 ml / minute);
  • Hyperchloremic acidosis;
  • Age under 18;
  • Pregnancy and lactation period;
  • Concomitant use of oral carbonic anhydrase inhibitors;
  • Hypersensitivity to the components of Azarga, sulfonamides or beta-blockers.

  • Hyperthyroidism;
  • Prinzmetal's angina;
  • Arterial hypotension and cardiovascular disease;
  • Central and peripheral circulatory disorders;

Method of administration and dosage

Azarga is applied topically. Shake the bottle before use.

The drug is prescribed 1 drop in the conjunctival sac of the eye 2 times a day.

To reduce the risk of systemic side effects after instillation, it is recommended to gently press for 1-2 minutes with a finger on the projection area of the lacrimal sacs at the inner corner of the eye (this helps to reduce the systemic absorption of the drug).

In case of skipping the next instillation, it is necessary to continue using the drug from the next dose in accordance with the schedule. The recommended daily dose should not be exceeded.

In the case of the previous use of another antiglaucoma drug, you need to start using Azarga the next day after canceling the previous drug.

Side effects

Even with local use, timolol can cause the same side effects from the respiratory and cardiovascular systems as systemic beta-blockers. For this reason, before prescribing the drug and during its use, the patient's condition should be monitored. There are cases of severe cardiovascular and respiratory disorders, including deaths, due to bronchospasm in patients with bronchial asthma and death from heart failure.

Before a planned operation with the use of general anesthesia, it is necessary to stop using Azarga in 48 hours, because timolol, like all beta-blockers, can reduce the sensitivity of the myocardium to sympathetic stimulation necessary for the heart to work.

Benzalkonium chloride, which is part of the drug (as a preservative), can cause eye irritation and discoloration of soft contact lenses. Lenses should be removed before instillation and reinstalled no earlier than 15 minutes later.

Benzalkonium chloride can cause punctate keratopathy and / or toxic ulcerative keratopathy, and brinzolamide can disrupt corneal hydration, therefore, with long-term treatment, it is necessary to monitor the patient's general well-being, including the state of his cornea. Special supervision is required for patients with diabetes mellitus, corneal abnormalities and dystrophy.

Care should be taken when instilling: do not touch the eye and other surfaces with the dropper to avoid contamination of the drug. Close the bottle after each use.

Due to a possible decrease in visual clarity immediately after instillation, it is recommended to refrain from driving and performing potentially dangerous types of work that require increased attention.

Oral carbonic anhydrase inhibitors may interfere with reaction rates and the ability to concentrate in the elderly. This effect must be taken into account when prescribing Azarga, since its components (including the carbonic anhydrase inhibitor brinzolamide) penetrate the systemic circulation.

Drug interactions

Studies on the interaction of Azarga with other drugs have not been conducted.

Oral carbonic anhydrase inhibitors should not be administered concurrently, as there is a risk of increased systemic adverse reactions.

Care should be taken to concomitantly use drugs that inhibit the CYP3A4 isoenzyme, such as ritonavir, clotrimazole, troleandomycin, itraconazole and ketoconazole.

With the simultaneous use of CYP2D6 inhibitors (for example, quinidine or cimetidine), it is possible to increase the systemic effect of timolol (decrease in heart rate).

There is a possibility of increased hypotensive action and / or the development of severe bradycardia with the combined use of Azarga with oral calcium channel blockers, antiarrhythmic drugs, beta-blockers, cardiac glycosides, parasympathomimetics, guanethidine.

Timolol can mask the symptoms of hypoglycemia and enhance the hypoglycemic effect of antidiabetic agents.

After abrupt withdrawal of clonidine, timolol may increase hypertension.

If it is necessary to use other local ophthalmic agents between instillations, at least 15-minute intervals should be observed.
