Formetin is an oral hypoglycemic agent from the biguanide group.

Release form and composition

Dosage form Formetin - tablets: 500 mg - round, flat-cylindrical, white, with a line and a chamfer; 850 mg and 1000 mg are oval, biconvex, white, with a line on one side. Packing: blisters - 10 pcs., In a cardboard box 2, 6 or 10 packages; 10 and 12 pcs., in a carton box 3, 5, 6 or 10 packs.

Composition of the preparation:

  • active substance: metformin hydrochloride, in 1 tablet - 500, 850 or 1000 mg;
  • additional components and their content for tablets 500/850/1000 mg: magnesium stearate - 5 / 8.4 / 10 mg, croscarmellose sodium (primellose) - 8 / 13.6 / 16 mg, povidone (povidone K-30, medium molecular weight polyvinylpyrrolidone) - 17/29/34 mg.

Pharmacological properties

Pharmacodynamics

Metformin hydrochloride is the active substance of Formetin, a substance that inhibits gluconeogenesis in the liver, enhances peripheral glucose utilization, reduces the absorption of glucose from the intestine, and increases the sensitivity of body tissues to insulin. In this case, the drug does not affect the secretion of insulin by the beta cells of the pancreas, and also does not cause the development of hypoglycemic reactions.

Metformin lowers the level of low density lipoproteins and triglycerides in the blood. Reduces or stabilizes body weight.

Due to the ability to suppress tissue-type plasminogen activator inhibitor, the drug has a fibrinolytic effect.

Pharmacokinetics

After oral administration, metformin is slowly absorbed from the gastrointestinal tract. After taking a standard dose, bioavailability is about 50-60%. The maximum plasma concentration reaches within 2.5 hours.

Practically does not bind to plasma proteins. Accumulates in kidneys, liver, muscles and salivary glands.

The half-life is from 1.5 to 4.5 hours. It is excreted by the kidneys unchanged. In patients with impaired renal function, metformin accumulation may occur.

Indications for use

Formetin is prescribed for type II diabetes mellitus (non-insulin dependent) patients in whom diet therapy was ineffective, especially for obese patients.

Contraindications

  • diabetic ketoacidosis;
  • diabetic precoma / coma;
  • impaired liver function;
  • severe renal dysfunction;
  • severe infectious diseases;
  • lactic acidosis at present or in history;
  • dehydration, acute disturbance of cerebral circulation, acute phase of myocardial infarction, heart and respiratory failure, chronic alcoholism and other diseases / conditions that can contribute to the development of lactic acidosis;
  • serious injury or surgery when insulin therapy is indicated;
  • acute alcohol poisoning;
  • adherence to a hypocaloric diet (less than 1000 kcal / day);
  • pregnancy and the period of breastfeeding;
  • X-ray / radioisotope studies using iodine-containing contrast agent (within 2 days before and 2 days after);
  • hypersensitivity to the drug.

Formetin is not recommended for people over 60 years of age who perform heavy physical work, since they have an increased risk of developing lactic acidosis.

Instructions for the use of Formetin: method and dosage

Formetin tablets are indicated for oral administration. They should be taken whole, without chewing, with plenty of water, during or after meals.

The optimal dosage for each patient is set individually and is determined by the level of glucose in the blood.

At the initial stage of therapy, usually 500 mg is prescribed 1-2 times a day or 850 mg once a day. In the future, no more than 1 time per week, the dose is gradually increased. The maximum allowable dose of Formetin is 3000 mg per day.

Elderly people should not exceed the daily dose of 1000 mg. In severe metabolic disorders due to the high risk of developing lactic acidosis, it is recommended to reduce the dose.

Side effects

  • from the endocrine system: when used in inadequate doses - hypoglycemia;
  • from the side of metabolism: rarely - lactic acidosis (requires discontinuation of the drug); with prolonged use - hypovitaminosis B 12 (malabsorption);
  • on the part of the digestive system: metallic taste in the mouth, diarrhea, lack of appetite, nausea, abdominal pain, flatulence, vomiting;
  • on the part of the hematopoietic organs: very rarely - megaloblastic anemia;
  • allergic reactions: skin rashes.

Overdose

An overdose of metformin can lead to fatal lactic acidosis. Lactic acidosis can also develop as a result of drug accumulation in patients with impaired renal function. Early signs of this condition are: a decrease in body temperature, general weakness, muscle and abdominal pain, diarrhea, nausea and vomiting, reflex bradyarrhythmia, and a decrease in blood pressure. In the future, dizziness, increased breathing, impaired consciousness, coma are possible.

If symptoms of lactic acidosis appear, you should immediately stop taking Formetin tablets, and the patient should be hospitalized. The diagnosis is confirmed based on lactate concentration data. Hemodialysis is the most effective way to remove lactate from the body. Further treatment is symptomatic.

Patients receiving metformin therapy should be closely monitored for renal function. At least 2 times a year, as well as in the case of the development of myalgia, the determination of the plasma lactate content is required.

If necessary, Formetin can be administered in combination with sulfonylurea derivatives. However, treatment must be carried out under close monitoring of blood glucose levels.

Cimetidine slows down the elimination of metformin and, as a result, increases the risk of lactic acidosis.

Cationic drugs secreted in the tubules (quinine, amiloride, triamterene, morphine, quinidine, vancomycin, procainamide, digoxin, ranitidine) compete for tubular transport systems, therefore, with prolonged use, they can increase the concentration of metformin by 60%.

Nifedipine enhances the absorption and maximum concentration of metformin, slows down its excretion.

Metformin can reduce the effect of coumarin-derived anticoagulants.
