Fluimucil is an expectorant mucolytic drug.

Release form and composition

Fluimucil dosage forms:

  • oral solution: 20 mg / ml - transparent or slightly opalescent, colorless, with a characteristic raspberry smell and a slight sulfuric smell; 40 mg / ml - transparent, colorless or slightly yellow, with a characteristic smell of pomegranate and strawberries and a faint sulfur smell (in dark glass bottles of 100, 150 or 200 ml, 1 bottle in a cardboard box);
  • solution for injection and inhalation: transparent, colorless, with a faint sulfur odor; with prolonged contact with air after opening the ampoule, a slight pink-purple tint may appear (in dark glass ampoules of 3 ml, 5 ampoules in contour plastic packaging, 1 package in a cardboard box);
  • granules for preparation of a solution for oral administration: white-yellow with orange inclusions, have a characteristic orange, slightly sulfurous smell (1 g each in multilayer laminated bags, 20 or 60 bags in a cardboard box);
  • effervescent tablets: round, white, have a rough surface and a lemon, slightly sulfurous odor; the solution after reconstitution in water should be somewhat opalescent, have a characteristic smell and taste of lemon flavor (2 or 10 tablets in blisters, 1, 2, 5 or 10 packs in a cardboard box).

Composition of 1 ml solution for oral administration:

  • active substance: acetylcysteine - 20 or 40 mg;
  • auxiliary components (20/40 mg): methyl parahydroxybenzoate - 1 / 1.8 mg; sodium benzoate - 1.5 / 0 mg; edetate disodium - 1/1 mg; carmellose sodium - 4/4 mg; sodium saccharinate - 0.4 / 0.4 mg; propyl parahydroxybenzoate - 0 / 0.2 g; 70% sorbitol - 0/120 mg; raspberry flavor - 2.5 / 0 mg; strawberry flavor - 0/10 mg; pomegranate flavor - 0/2 mg; sodium hydroxide - up to pH 6.5 / 6.5; purified water - up to 1/1 ml.

Composition of 1 ampoule of solution for injection and inhalation:

  • active substance: acetylcysteine - 300 mg;
  • auxiliary components: sodium hydroxide - up to pH 6.5 (about 74 mg); edetate disodium - 3 mg; water for injection - up to 3 ml.

Composition of 1 sachet for preparation of solution for oral administration:

  • active substance: acetylcysteine - 200 mg;
  • auxiliary components: sorbitol - 662.7 mg; aspartame - 25 mg; orange flavor - 100 mg; betacarotene - 12.3 mg.

Composition of 1 effervescent tablet:

  • active substance: acetylcysteine - 600 mg;
  • auxiliary components: sodium bicarbonate - 500 mg; aspartame - 20 mg; citric acid - 680 mg; lemon flavor - 100 mg

Indications for use

  • violation of sputum discharge associated with blockage of the bronchi with a mucous plug, in diseases such as atelectasis of the lung, pneumonia, bronchitis, bronchiolitis, tracheitis, cystic fibrosis, bronchiectasis, lung abscess, laryngotracheitis, pulmonary emphysema, interstitial lung diseases;
  • otitis media (purulent and catarrhal), sinusitis, sinusitis (to facilitate secretion discharge);
  • viscous secretion in the airways in postoperative / post-traumatic conditions (for removal).

Additional indications for Fluimucil solution for injection and inhalation:

  • preparation for bronchoscopy, bronchography, aspiration drainage;
  • treatment of fistulas, operating field during operations on the mastoid process and nasal cavity;
  • washing the middle ear, nasal passages, abscesses, maxillary sinuses.

Contraindications

Absolute:

  • exacerbation of gastric ulcer and duodenal ulcer;
  • intolerance to fructose (for oral solution 40 mg / ml);
  • age up to 2 years (for oral solution and granules for the preparation of oral solution), up to 6 years (for solution for injection and inhalation) and up to 18 years (for effervescent tablets);
  • lactation period;
  • individual intolerance to the components of the drug.

Relative (Fluimucil is prescribed with caution in the presence of the following diseases / conditions):

  • adrenal disease;
  • pulmonary bleeding;
  • arterial hypertension;
  • peptic ulcer of the stomach and duodenum;
  • hemoptysis;
  • varicose veins of the esophagus;
  • bronchial asthma (for intravenous administration of Fluimucil solution for injection, since there is a high likelihood of bronchospasm);
  • renal / hepatic impairment;
  • phenylketonuria (for granules for the preparation of oral solution and effervescent tablets; associated with the presence of aspartame in the preparation);
  • age up to 1 year (for intravenous administration of Fluimucil solution for injection, the use of the drug is possible only in stationary conditions in cases of vital indications);
  • pregnancy (the use of Fluimucil is possible only after evaluating the ratio of benefits to possible risks).

Method of administration and dosage

Oral solution

Fluimucil is taken orally.

Recommended regimen of the drug (the daily dose of acetylcysteine is indicated in brackets):

  • children 2-5 years old: 2-3 times a day, 5 ml of a solution of 20 mg / ml (200-300 mg);
  • children 6-14 years old: 3-4 times a day, 5 ml of a solution of 20 mg / ml, or 2 times a day, 4 ml of a solution of 40 mg / ml (300-400 mg);
  • children from 14 years old and adults: once a day, 15 ml of a solution of 40 mg / ml (600 mg).

Maximum per day - 600 mg of acetylcysteine.

Solution for injection and inhalation

How to use Fluimucil:

  • parenteral: route of administration - intramuscularly and intravenously (preferably drip or slow stream over 5 minutes); dosage regimen - 1–2 times a day, 3 ml (adults), 1–2 times a day, 1.5 ml (children 6-14 years old) or 10 mg / kg per day (children under 6 years old; it is preferable to prescribe the drug in oral forms; intravenous administration to children under 1 year old is possible only if there are vital indications in stationary conditions);
  • inhalation (aerosol therapy): a 10% solution of 3–9 ml (in ultrasound devices) or 6 ml (in devices with a control valve) is used. Duration of inhalation is from 15 to 20 minutes, frequency of application is 2–4 times a day. The average course duration for acute conditions is 5-10 days, for chronic conditions - up to 6 months. Depending on the therapeutic effect and the patient's condition, the doctor may make changes to the dosage regimen. For children, the drug is prescribed in adult doses. With a strong secretolytic effect, the secret is sucked out, and the frequency of inhalations and the daily dose decrease;
  • topically (instillation into the nasal passages and external auditory canal): 1.5-3 ml for 1 procedure;
  • intratracheally (during medical bronchoscopy for washing the bronchial tree): 1–2 ampoules or more per day (depending on clinical indications).

The duration of the course is determined by the results of treatment. Fluimucil has a high general and local tolerance, so long-term use is possible.

Before intravenous administration, the solution should be additionally diluted with 5% dextrose solution or 0.9% NaCl solution (ratio - 1: 1).

For patients over 65 years of age, Fluimucil is usually prescribed in the minimum effective dose.

Granules for preparation of solution for oral administration

Fluimucil is taken orally, after dissolving a single dose in 1/3 cup of water.

Recommended dosage regimen:

  • newborns (only in the presence of vital indications and under medical supervision) and children under 1 year of age: 10 mg / kg (from a feeding bottle or spoon);
  • children 1-2 years old: 2 times a day, 100 mg;
  • children 2–6 years old: 3 times a day, 100 mg, or 2 times a day, 200 mg;
  • children from 6 years old and adults: 2-3 times a day, 200 mg.

The duration of the therapeutic course is determined individually:

  • acute course of the disease: 5-10 days;
  • chronic course of the disease: up to several months.

Effervescent tablets

Fluimucil is taken orally (solution - 1 tablet for 1/3 cup of water).

The recommended dosage regimen for adults: 1 tablet once a day.

The duration of the therapeutic course is determined individually:

  • acute course of the disease: 5-10 days;
  • chronic course of the disease: up to several months.

Side effects

Oral solution, granules for solution preparation, effervescent tablets

In rare cases, the following adverse reactions may develop: nosebleeds, tinnitus, disturbances from the digestive system (in the form of nausea, heartburn, a feeling of full stomach, skin rash, vomiting, diarrhea), urticaria, itching.

There is also evidence of the development of collapse, bronchospasm, a decrease in platelet aggregation, stomatitis.

Solution for injection and inhalation

  • inhalation administration: rhinorrhea, respiratory tract irritation, reflex cough; rarely - bronchospasm, stomatitis;
  • intramuscular injection: skin rash, burning sensation at the injection site, urticaria; with prolonged therapy - functional disorders of the kidneys / liver.

special instructions

Patients with bronchial asthma and obstructive bronchitis should be prescribed acetylcysteine with caution under systematic monitoring of bronchial patency.

A slight sulfuric odor is characteristic of the active ingredient Fluimucil.

The ampoule with a solution for injection and inhalation must be opened before use. After opening, the drug can be used for 24 hours only for inhalation, provided it is stored in the refrigerator.

With a shallow intramuscular injection of Fluimucil and in the presence of hypersensitivity, a mild and rapidly passing burning sensation may develop, therefore it is recommended to inject the drug deep into the muscle.

Fluimucil solution (ready-made or obtained by dissolving tablets or granules) must not come into contact with metal / rubber surfaces.

Drug interactions

With the combined use of Fluimucil with certain drugs / substances, the following effects may develop:

  • antitussive drugs: increased stagnation of sputum, which is associated with suppression of the cough reflex;
  • antibacterial drugs - ampicillin, tetracyclines (except doxycycline), amphotericin B: decrease in the effectiveness of both drugs (the recommended interval between doses is at least 2 hours);
  • nitroglycerin: enhancing its antiplatelet and vasodilating action;
  • paracetamol: elimination of its toxic effects.

Fluimucil solution is pharmaceutically incompatible with other drug solutions.

Terms and conditions of storage

Store at temperatures up to 25 ° C. Keep out of the reach of children.

  • solution for oral administration - 2 years (after the first opening of the package - 15 days);
  • solution for injection and inhalation - 5 years;
