Ebrantil is an antiadrenergic drug from the group of α 1 -adrenergic blockers.

Composition and form of release

According to the instructions, Ebrantil is available in the form of a solution for intravenous injection (in ampoules of 5, 10 ml), as well as in the form of capsules (30, 60 and 90 mg).

1 ml of Ebrantil contains 5 mg of urapidil - the main active ingredient of the drug. Auxiliary components: propylene glycol, sodium hydrogen phosphate dihydrate, sodium dihydrogen phosphate dihydrate, water for injection.

One capsule of the medication contains 30, 60 or 90 mg of urapidil, respectively. Additional components: methyl methacrylate copolymer (type B), talc, diethyl phthalate, hypromellose, fumaric acid, ethyl cellulose, hypromellose phthalate, stearic acid, spherical sugar.

Pharmacological action of Ebrantil

Ebrantil, blocking postsynaptic α 1 -adrenergic receptors, reduces peripheral resistance. In addition, Ebrantil has a weakly expressed beta-adrenergic blocking effect, is involved in the regulation of the mechanism of maintaining vascular tone. Cardiac output and heart rate do not change with the introduction of Ebrantil.

The drug lowers blood pressure in a balanced way (diastolic and systolic), helps to reduce peripheral resistance, without causing reflex tachycardia.

The action of urapidil helps to increase the reduced minute volume of the heart, reduce the load on the heart muscle and increase the efficiency of heart contractions.

In the central nervous system, the active substance of the drug affects the activity of the vasomotor center, preventing a reflex decrease (or increase) in the tone of the sympathetic part of the nervous system.

Urapidil does not in any way affect the metabolism of uric acid and the indicators of carbohydrate metabolism, does not cause fluid retention in the body.

Ebrantil easily penetrates the placental and blood-brain barriers. The maximum concentration of urapidil in the blood plasma is achieved four to six hours after the use of Ebrantil. Most of urapidil is metabolized in the liver. Urapidil and its metabolites are excreted by the kidneys (55-70%) and through the intestines.

Indications for use

According to the instructions, Ebrantil is used for hypertensive crisis, refractory arterial hypertension, as well as for the controlled reduction of high blood pressure after and / or during surgery.

According to the instructions, Ebrantil is administered intravenously by continuous infusion or by jet. Capsules of the drug Ebrantil should be taken twice a day, morning and evening, during meals, with a small amount of liquid. With refractory hypertension, severe arterial hypertension, hypertensive crisis, 10-50 mg of Ebrantil is administered intravenously under the control of blood pressure. The drop in blood pressure should occur five minutes after injection. Depending on the therapeutic effect, repeated administration of the medication is possible.

For continuous or intravenous drip injections, a perfusion pump is used. The maintenance dose is about 9 mg / h, i.e. 250 mg of the medication is diluted in 0.5 l of solution for injection. The maximum allowable concentration is 4 mg of Ebrantil per 1 ml of solution for injection (5% or 10% glucose (dextrose) solution or saline). The rate of intravenous drip of the drug depends on the patient's blood pressure.

When using a perfusion pump, 100 mg of Ebrantil is injected into a syringe and diluted to 50 ml with 5% or 10% glucose (dextrose) solution or saline.

With an increase in blood pressure after and / or during surgical interventions for the purpose of its controlled reduction, the drug Ebrantil is administered intravenously at a dose of 25 mg. A decrease in blood pressure indicators should occur within two minutes after administration. Next, blood pressure is stabilized by infusion.

If the stabilization of pressure indicators does not occur for a long time, then a slow intravenous administration of the drug Ebrantil at a dose of 50 mg is performed.

If other antihypertensive drugs were previously used, then Ebrantil can be administered no earlier than the onset of the action of the administered drug.

For a gradual decrease in blood pressure, 30 mg of the medication is used twice a day.

The recommended dose of Ebrantil for a rapid decrease in blood pressure is 60 mg twice a day. The total amount of urapidil should be between 60 and 180 mg per day.

Contraindications to the use of Ebrantil

  • aortic stenosis;
  • children up to age 18;
  • pregnancy;
  • lactation period;
  • hypersensitivity to Ebrantil;
  • open botallic duct.

Ebrantil should be used with caution in old age, with hypovolemia, as well as with impaired renal and / or liver function.

Side effect

With prolonged use of Ebrantil, the following side symptoms may occur: vomiting, nausea, arrhythmia, shortness of breath, headache, nephropathy, dizziness. Sometimes itching occurs, bradycardia or tachycardia, palpitations, anxiety, thrombocytopenia, nephrotic syndrome.

Ebrantil overdose

Symptoms of an overdose with Ebrantil: orthostatic hypotension, dizziness, collapse, decreased reaction rate, fatigue.

Therapy: reducing the progressive decrease in blood pressure by raising the lower extremities upward, replacing the circulating blood volume; in the absence of effect, slow intravenous administration of vasoconstrictors. In rare cases, intravenous administration of catecholamines is used.
