Trombless Plus is an external preparation with antithrombotic, local anesthetic, venotonic and tissue regeneration effect.

Release form and composition

Dosage form - gel for external use: translucent or transparent, with a weak characteristic odor, from yellow to light yellow, let’s say a greenish tint (in a cardboard box 1 aluminum or polyethylene tube of 10, 20, 30, 40, 50 or 100 g and instructions for the use of Trombless Plus).

Active ingredients in 1000 mg gel:

  • sodium heparin - 1000 IU (international units; with the activity of sodium heparin 120 IU / mg - 8.33 mg);
  • dexpanthenol - 100 mg;
  • troxerutin - 20 mg;
  • benzocaine - 1 mg.

Additional components: ethanol, glycerol, methyl parahydroxybenzoate, carbomer, trolamine, water.

Pharmacological properties

Pharmacodynamics

Trombless Plus is a combined drug, the therapeutic effect of which is due to the properties of the active ingredients.

Heparin sodium is a direct anticoagulant, has antiproliferative, anti-inflammatory, analgesic and decongestant effects. Main effects:

  • a decrease in platelet aggregation, which is due to the binding of antithrombin III and the prevention of the transition of prothrombin to thrombin;
  • inhibition of thrombin activity;
  • improvement of microcirculation and activation of tissue metabolism, due to which the processes of resorption of hematomas and blood clots are accelerated and, as a result, the patency of veins is restored; clinically, this is accompanied by a pronounced anti-inflammatory and analgesic effect;
  • decreased activity of hyaluronidase, increased fibrinolytic properties of blood.

Dexpanthenol is a B 5 provitamin that is converted to pantothenic acid in the skin. It is a part of coenzyme A, which is involved in acetylation processes, fat and carbohydrate metabolism. It has a vitamin, regenerating, anti-inflammatory and metabolic effect. Other effects: stimulating skin regeneration, helping to increase the strength of collagen fibers, normalizing cell metabolism.

Troxerutin is a rutin derivative (flavonoid). It has a decongestant, venotonic, anti-inflammatory, angioprotective and antioxidant effect, has P-vitamin activity. Main effects:

  • influence on redox processes, blocking of hyaluronidase, stabilization of hyaluronic acid of cell membranes and a decrease in the permeability and fragility of capillaries, an increase in their tone;
  • promoting an increase in the density of the vascular wall, a decrease in the exudation of the liquid part of the plasma and diapedesis of blood cells;
  • reduction of inflammation in the vascular wall, which is provided by limiting the adhesion of platelets to its surface.

Benzocaine is a local anesthetic for surface anesthesia. Promotes the absorption of heparin, due to which the surface vessels expand. Main effects:

  • decrease in the permeability of the cell membrane for sodium ions, displacement of calcium ions from receptors that are located on the inner surface of the membrane, blocking the conduction of nerve impulses;
  • preventing the appearance of pain impulses in the endings of sensory nerves, as well as their conduction along the nerve fibers.

Pharmacokinetics

Heparin is absorbed in a small amount into the systemic circulation from the skin surface. After application, C max (maximum concentration of the substance) in the blood is reached in 8 hours. Excretion is carried out mainly through the kidneys, T 1/2 (half-life) is 12 hours.

When applied externally, troxerutin is well absorbed from the skin surface.

Dexpanthenol is rapidly absorbed by the skin, converting into pantothenic acid. Its concentration: in the blood serum - 0.1 mg / l, in the blood - 0.5–1 mg / l. Pantothenic acid in the body is not metabolized (except for inclusion in coenzyme A), excretion is carried out unchanged.

When applied to the skin, benzocaine is almost not absorbed.

Indications for use

  • bruises and injuries of joints, muscle tissue, tendons;
  • subcutaneous hematomas, aseptic infiltrates and localized edema;
  • symptomatic complex of chronic venous insufficiency (including against the background of varicose veins of the lower extremities), thrombophlebitis of superficial veins, migrating phlebitis;
  • complications that have arisen after vein surgery.

Contraindications

Absolute:

  • wounds with profuse exudation or open infected wounds at the site of application of the gel;
  • pregnancy and lactation;
  • age up to 18 years;
  • individual intolerance to the components of the drug.

Relative (Trombless Plus gel is prescribed under medical supervision):

  • thrombocytopenia;
  • increased tendency to bleeding.

Trombless Plus, instructions for use: method and dosage

Trombless Plus is applied externally.

The gel is applied to the skin over and around the affected area in a thin layer, evenly spreading with light circular movements. Frequency rate of application - 2-3 times a day. The course of therapy is from 2 to 3 weeks, with a relapse of the disease, repetition of the application is possible.

In patients with trophic ulcers with weak exudation, before use, the wound surface is cleaned of exudate and necrotic tissues, if necessary, washed with a 3% solution of hydrogen peroxide, nitrofural 1: 5000 or 0.05% chlorhexidine and dried. Trombless Plus is applied to the entire affected surface with a uniform thin layer, after which a sterile gauze bandage is applied, which is changed once a day. With an open method of therapy, the gel is applied 1–2 times a day. The duration of application depends on the dynamics of epithelialization.

In case of localized edema, subcutaneous hematomas, aseptic infiltrates, bruises, trauma and similar conditions, Trombless Plus is applied to the affected area 2-3 times a day. The drug is used until the normal state of damaged tissues is completely restored.

Side effects

With prolonged use of Trombless Plus, the following violations may develop:

  • local reactions at the site of application in the form of decreased sensitivity, skin hyperemia;
  • allergic reactions in the form of skin rash, itching.
