Tetracycline-LekT is a bacteriostatic drug. It is an antibiotic of the tetracycline group.

Release form and composition

The drug is available in the form of film-coated tablets: round, biconvex, red; on the break, a yellow core is visible (10 pcs. in blisters, in a cardboard box 2 or 3 packs; 20 or 40 pcs. in plastic bottles or dark glass jars, in a cardboard box 1 bottle or 1 can and instructions for the use of Tetracycline -LekT).

Composition for 1 film-coated tablet:

  • active substance: tetracycline hydrochloride - 100 mg;
  • auxiliary components of the tablet core: edible gelatin, talc, calcium stearate, potato starch, sugar;
  • film shell: macrogol 6000, tropeolin, methylcellulose, azorubin dye, titanium dioxide.

Pharmacological properties

Pharmacodynamics

The drug belongs to the group of tetracycline antibiotics; has a bacteriostatic effect. It inhibits protein synthesis by disrupting the formation of a complex between the ribosome and tRNA [transport RNA (ribonucleic acid)].

Tetracycline-LekT is active against the following microorganisms:

  • gram-negative bacteria: Bordetella pertussis, Bartonella bacilliformis, Brucella spp. (in combination with streptomycin), Acinetobacter spp., Shigella spp., Haemophilus ducreyi, Haemophilus influenzae, Salmonella spp., Rickettsia spp., Enterobacter spp. (including Enterobacter aerogenes), Francisella tularensis, Escherichia coli, Vibrio fetus, Vibrio cholerae, Yersinia pestis, Klebsiella spp.;
  • gram-positive bacteria: Streptococcus pneumoniae, Bacillus anthracis, Staphylococcus spp. (including Staphylococcus aureus, including strains producing penicillinase), Actinomyces israelii, Listeria spp., Clostridium spp.;
  • other microorganisms: Chlamydia psittaci, Chlamydia trachomatis, Calymmatobacterium granulomatis, Entamoeba spp., Treponema spp., Mycoplasma pneumonia.

The drug is used for infections caused by Neisseria gonorrhoeae, Clostridium spp. or Actinomyces spp., in patients for whom penicillin antibiotics are contraindicated.

Viruses are resistant to tetracycline, most fungi, as well as Pseudomonas aeruginosa, Streptococcus faecalis, Streptococcus pyogenes, Serratia spp., Proteus spp. and almost all Bacteroides spp.

Pharmacokinetics

Tetracycline is absorbed in the gastrointestinal tract by 75–77%. Food intake slows down the rate of absorption. About 55–65% of the drug binds to plasma proteins.

When taken orally, the maximum plasma concentration is reached after 2-3 hours, therapeutic - within 2-3 days. In the next 8 hours after taking tetracycline, its plasma concentration gradually decreases. The maximum concentration is from 1.5 to 3.5 mg / l, and to achieve a therapeutic effect, a concentration of 1 mg / l is sufficient.

Tetracycline is unevenly distributed in the body: its maximum concentrations are determined in the lymph nodes, lungs, spleen, kidneys and liver. In bile, the content of the active substance is 5-10 times higher than in the blood serum. In the tissues of the prostate and thyroid glands, the concentration of tetracycline corresponds to plasma; in saliva, ascitic fluid, pleural fluid and breast milk ranges from 60 to 100% of the plasma concentration. The drug accumulates in large quantities in the enamel and dentin of milk teeth, tumor tissues and bone tissue. It passes through the blood-brain barrier poorly. With intact meninges, tetracycline is not detected in the cerebrospinal fluid, or its amount there is insignificant and is no more than 5-10% of the plasma concentration. In patients with pathologies of the central nervous system, especially if the inflammatory process affects the membranes of the brain, the concentration in the cerebrospinal fluid fluctuates and is equal to 8–36% of the plasma concentration. Tetracycline enters the fetus and is secreted in breast milk. The volume of distribution is 1.3-1.6 l / kg.

It undergoes a slight hepatic metabolism. The half-life is 6–11 hours, and in patients with anuria it lengthens to 57–108 hours. High concentrations of tetracycline in the urine are noted already 2 hours after administration and remain so for 6–12 hours. Up to 10–20% of the dose taken excreted by the kidneys in the first 12 hours. About 5-10% of tetracycline enters the intestines with bile and is partially absorbed back there. This contributes to a longer circulation of the active substance in the body (the so-called intestinal-hepatic circulation). From 20 to 50% of the drug is excreted through the intestines.

It is slowly removed from the body during hemodialysis.

Indications for use

Tetracycline-LekT is used for infectious and inflammatory diseases caused by microorganisms that are sensitive to tetracycline:

  • respiratory tract infections caused by Klebsiella spp. and Haemophilus influenzae;
  • respiratory tract infections and pneumonia caused by Mycoplasma pneumoniae;
  • bacterial infections of the genitourinary system (chlamydia, uncomplicated gonorrhea, chancroid, lymphogranuloma venereum, syphilis);
  • acne;
  • conjunctivitis;
  • other infections (intestinal amebiasis, ulcerative necrotizing gingivostomatitis, actinomycosis, brucellosis, cholera, listeriosis, psittacosis, tularemia, typhus, yaws, Rocky Mountain spotted fever, anthrax, bartonellosis, plague rhymes, vesiciasis)

Contraindications

Absolute:

  • severe renal failure (with creatinine clearance 10-50 ml / min);
  • low white blood cell count (leukopenia);
  • fructose intolerance, sucrase / isomaltase deficiency, glucose-galactose malabsorption syndrome;
  • period of pregnancy (second and third trimesters);
  • period of breastfeeding;
  • children under 8 years old;
  • hypersensitivity to the main or auxiliary components of the drug.

Tetracycline-LekT is used with caution when creatinine clearance is 50–80 ml / min (mild to moderate renal failure) and in the first trimester of pregnancy (only after a careful assessment of the ratio of benefits to the mother and risks to the fetus).

Tetracycline-LekT, instructions for use: method and dosage

Tetracycline-LecT tablets are taken orally with plenty of water or other liquid. Adults are usually prescribed 300–500 mg four times a day or 500–1000 mg every 12 hours. The maximum dose for adults is 2000 mg per day.

Recommended doses of the drug for certain diseases (for adults):

  • acne: 500-2000 mg per day in divided doses; after improvement of the condition (as a rule, after 3 weeks), the dose is gradually reduced to a maintenance dose, which is 100–1000 mg per day; to achieve an adequate remission of acne, Tetracycline-Lect should be used every other day or use intermittent therapy;
  • syphilis: 500 mg every 6 hours; with early syphilis, treatment is continued for 15 days, with late syphilis - within 30 days;
  • uncomplicated gonorrhea: the initial dose is 1500 mg, then 500 mg is prescribed every 6 hours for 4 days;
  • brucellosis: 500 mg every 6 hours for 3 weeks; simultaneously with tetracycline, streptomycin is administered intramuscularly (1000 mg every 12 hours for 1 week, and then once a day for 2 weeks);
  • uncomplicated rectal, endocervical and urethral infections caused by Chlamydia trachomatis: 500 mg 4 times a day, the course of treatment is at least 1 week.

In children over the age of 8 years, Tetracycline-Lect is used in individual doses, taking into account body weight (at the rate of 20-25 mg / kg of body weight). Frequency rate of application - every 6 hours.

In patients with renal failure, a single dose of the drug should not exceed 300-500 mg (depending on the degree of renal dysfunction), and the interval between doses should be at least 6 hours.

Side effects

  • digestive system: nausea and vomiting, decreased appetite, glossitis, hypertrophy of the papillae of the tongue, stomatitis, decreased appetite, dysphagia, gastritis, pancreatitis, esophagitis, colitis, ulceration of the mucous membrane of the stomach and duodenum, intestinal dysbiosis, hepatitis, increased activity of liver enzymes,
  • hematopoietic system: a decrease in the number of platelets and neutrophils, hemolytic anemia;
  • nervous system: headache, instability or dizziness, increased intracranial pressure;
  • urinary system: increased blood creatinine levels, azotemia;
  • allergic reactions: skin flushing, photosensitivity, urticaria, exfoliative dermatitis, maculopapular rash, arthralgia, eosinophilia, pericarditis, fever, drug systemic lupus erythematosus, Quincke's edema, anaphylactoid reactions;
  • other reactions: candidiasis, changes in tooth enamel (in children), hypovitaminosis B, superinfection, hyperbilirubinemia.

Overdose

In case of an overdose of Tetracycline-Lect, the patient develops nausea or vomiting, cramps or burning sensations in the stomach area, instability or dizziness is observed, and diarrhea may occur.

Treatment is symptomatic and supportive.

Due to the risk of photosensitization during the period of drug treatment, it is necessary to avoid prolonged exposure to direct sunlight.

With prolonged therapy, the function of the liver, kidneys and hematopoietic system should be periodically monitored.

Tetracycline can mask the symptoms of syphilis, so if a mixed infection is likely for 4 months, a monthly serological test for syphilis should be performed.

Reception of Tetracycline-LekT during the development of teeth is carried out with caution, since tetracyclines can cause enamel hypoplasia and long-term staining of tooth enamel in a yellow-gray-brown color (due to persistent binding with calcium ions).

In order to prevent vitamin deficiency during treatment with Tetracycline-LekT, it is necessary to additionally take brewer's yeast and vitamins of groups K and B.

Influence on the ability to drive vehicles and complex mechanisms

Tetracycline-LekT can cause dizziness or instability, therefore, during the period of its use, you should be very careful in work that requires increased concentration of attention and a quick reaction.

Application during pregnancy and lactation

Tetracycline-LekT is contraindicated in pregnant women in the second and third trimesters. In the first trimester, the drug is used only after a careful assessment of the ratio of benefits to the mother and risks to the fetus.

Tetracyclines suppress the intestinal microflora, which can lead to a decrease in the prothrombin index (a decrease in the dose of indirect anticoagulants is required).

Tetracycline reduces the effectiveness of oral estrogen-containing contraceptives and increases the risk of breakthrough bleeding; reduces the therapeutic effect of cephalosporins, penicillins and other bactericidal antibiotics (due to a violation of cell wall synthesis).

With simultaneous use with retinol, the risk of increased intracranial pressure increases.

Antacids containing aluminum, magnesium or calcium salts, as well as cholestyramine and iron preparations, reduce the absorption of tetracycline.

Co-administration with chymotrypsin increases the plasma concentration and duration of tetracycline circulation in the blood.
