Zayed (also angular stomatitis, angular cheilitis, angulitis) is a pathological condition in which there is a violation of the integrity of the skin and (or) the mucous membrane of the corners of the mouth.
Stuck in the corners of the mouth
Causes
The causative agents of seizure are bacteria (more often streptococci, staphylococci), yeast-like fungi of the genus Candida, viruses (usually the herpes virus).
The occurrence of a jam is facilitated by:
- decreased immunity;
- hypovitaminosis (vitamin B 2);
- non-observance of oral hygiene;
- allergic reactions;
- malocclusion;
- caries;
- long-term treatment with antibacterial drugs, cytostatics;
- chronic diseases;
- metabolic disorders (especially diabetes mellitus);
- trauma to the skin of the corners of the mouth, adverse environmental effects;
- Iron-deficiency anemia;
- respiratory diseases; etc.
The incidence of seizure is higher in children.
Kinds
Depending on the type of infectious agent, seizures are distinguished:
- bacterial;
- fungal;
- viral;
- mixed etiology;
- non-infectious nature (occur against the background of other disorders).
Distinguish between acute and chronic forms of the pathological process.
In addition, depending on the location, seizures can be one-sided or two-sided.
Signs
When the pathogen is introduced into the skin and (or) mucous membranes, small bubbles or papules appear in the corners of the mouth; the process of their formation is accompanied by burning or itching. Further, a slight erosion appears on the affected area, causing discomfort when talking, eating, laughing, etc.
In case of bacterial seizures, erosion is covered by a purulent-bloody crust, when removed, a moist surface (often with a crack in the middle) is exposed, which bleeds easily with minor trauma. The seizure becomes crusty again after a few hours.
Bacterial seizures
In the case of a viral jam, the affected areas are located in separate points.
In case of seizures of a fungal nature, there are no crusts on the wound surface. The erosion is bright red, sometimes covered with a grayish-white bloom, which can be easily removed. Fungal seizures are usually chronic.
Diagnostics
The diagnosis of seizure is established on the basis of the clinical picture. To identify the pathogen, laboratory tests are carried out. In a scraping taken from the affected area, the presence of bacteria, fungi, and other pathogens is determined by a microscopic (bacterioscopic) method. In order to exclude diabetes mellitus, a blood glucose test is performed. For differential diagnosis with syphilis, a serological blood test is performed.
Treatment
Seizures are easy to treat, unless their occurrence is due to severe pathology. For seizures of a non-infectious nature, alcoholic solutions of aniline dyes are used.
In the case of a jam of infectious etiology, local treatment is prescribed; the choice of a drug depends on the causative agent of the pathology. In addition, vitamin complexes and general health-improving agents are prescribed.
Alcohol solutions of aniline dyes are effective for seizures of a non-infectious nature.
Elimination of factors that can contribute to the recurrence of seizures (smoking, the habit of licking lips, etc.) is of no small importance. The use of special cosmetics (lip balms, hygienic lipsticks) is recommended. In order to avoid relapses, treatment should be continued 1-1.5 weeks after the disappearance of clinical signs of pathology.
Prevention
In order to prevent the occurrence of seizure, it is recommended to observe oral hygiene, use cosmetics that prevent chapping of the lips, and give up the habit of licking lips. In the case of long-term treatment with antibacterial drugs, vitamin complexes can be prescribed.
