Telfast is an antihistamine.
Release form and composition
Dosage form - tablets, film-coated: oblong biconvex shape, light pink, engraved on one side in the form of a stylized letter "e", on the other - "012" or "018" (10 pcs. In a blister, in cardboard box 1 blister).
1 tablet contains:
- active substance: fexofenadine hydrochloride - 0.12 g or 0.18 g;
- auxiliary components: microcrystalline cellulose, croscarmellose sodium, magnesium stearate, pregelatinized starch;
- the composition of the film shell: povidone, hypromellose (E15), macrogol 400, hypromellose (E5), colloidal silicon dioxide, iron oxide red (E172), iron oxide yellow (E172), titanium dioxide (E171).
Pharmacological properties
Pharmacodynamics
Fexofenadine belongs to the blockers of H 1 -histamine receptors and is a metabolite of terfenadine with pharmacological activity. It helps to stabilize the membranes of mast cells and inhibits the release of histamine and other biologically active compounds from them. The drug has practically no sedative effect.
The antihistaminic effect of Telfast is manifested 1 hour after ingestion, reaches a peak after 6 hours and lasts for 24 hours. After a course of treatment lasting 28 days, tolerance and addiction to the drug do not develop. When taken orally in the dose range of 10–130 mg, a dose-dependent effect is observed.
Pharmacokinetics
After oral administration, fexofenadine is rapidly absorbed from the gastrointestinal tract. Its maximum concentration is recorded in about 1-3 hours. Due to a single dose of the drug at a dose of 30 mg, the maximum concentration of its active component reaches 128 ng / ml.
The degree of binding of fexofenadine to blood plasma proteins is 60–70%. A small amount of the substance is involved in metabolic processes, carried out both by the extrahepatic and hepatic pathways.
Fexofenadine is excreted in a two-phase manner. After repeated administration, the half-life varies from 11 to 15 hours. With single and repeated use of the drug in a dosage of up to 120 mg 2 times a day (oral administration), the pharmacokinetics of the drug is linear.
Fexofenadine is excreted mainly in feces (up to 80%), about 10% of the substance is excreted unchanged in the urine.
Indications for use
According to the instructions, Telfast is used to reduce the symptoms of the following diseases:
- seasonal allergic rhinitis;
- chronic idiopathic urticaria.
Contraindications
- the period of pregnancy and breastfeeding;
- age up to 12 years;
- individual hypersensitivity to the components of the drug.
The drug should be used with caution in chronic renal and / or hepatic failure, cardiovascular diseases (including history), in elderly patients.
Instructions for the use of Telfast: method and dosage
The tablets are taken orally 1 time per day before meals.
Recommended daily dosage for patients over 12 years old:
- seasonal allergic rhinitis: 0.12 g;
- chronic idiopathic urticaria: 0.18 g.
With renal and / or hepatic insufficiency, in elderly patients, dosage adjustment is not required.
Side effects
- often: headache, nausea, drowsiness, dizziness;
- infrequently: palpitations, tachycardia, weakness, diarrhea, nervousness, insomnia, sleep disturbances, nightmares;
- rarely: exanthema, urticaria, itching, hypersensitivity reactions (shortness of breath, Quincke's edema, difficulty breathing, skin hyperemia, anaphylactic reactions of a systemic nature).
Overdose
The main symptoms of a Telfast overdose are dry mouth, drowsiness and dizziness. In this case, gastric lavage should be performed and activated charcoal should be taken. If necessary, supportive and symptomatic therapy is prescribed. Hemodialysis is considered ineffective in eliminating fexofenadine.

With concomitant therapy with antacids containing magnesium or aluminum hydroxide, it is recommended to observe the interval between taking fexofenadine at least two hours.
Telfast does not affect the patient's ability to drive vehicles and mechanisms, except in cases with an individual non-standard reaction to fexofenadine.
Influence on the ability to drive vehicles and complex mechanisms
Reception of Telfast does not exclude the performance of types of work that require the speed of psychomotor reactions and increased concentration of attention, except for cases of non-standard patient reaction to the drug. Before engaging in potentially hazardous activities, it is recommended to check the patient's response to fexofenadine on a case-by-case basis.
Application during pregnancy and lactation
In limited animal studies, no signs of adverse effects on fetal development, pregnancy, labor, and postnatal development were found. But there is not enough data on the use of fexofenadine during human pregnancy, and therefore Telfast is prohibited from prescribing to pregnant women.
There are no reliable data on the content of fexofenadine in the milk of breastfeeding women. But due to the use of terfenadine, the metabolite of which is fexofenadine, its penetration into breast milk was observed. Therefore, it is not recommended to prescribe Telfast during lactation (breastfeeding).
Pediatric use
In pediatric practice, the drug is not prescribed for children under 12 years of age. For the treatment of children from 6 to 11 years old Telfast tablets (for children) are intended at a dosage of 30 mg.
With impaired renal function
The drug is prescribed with caution in chronic renal failure, since there is insufficient clinical experience in treating such patients. Correction of the dosage regimen is not required.
For violations of liver function
With caution, the drug is prescribed for liver failure, since there is insufficient clinical experience in the treatment of such patients. Correction of the dosage regimen is not required.
Use in the elderly
Elderly patients do not need to adjust the dosage regimen of Telfast, but caution should be exercised when using the drug.
Drug interactions
The simultaneous use of erythromycin or ketoconazole increases the plasma concentration of fexofenadine 2-3 times, but this is not associated with a significant lengthening of the QTc interval.
Interaction with omeprazole has not been established.
Telfast does not interact with drugs that are metabolized in the liver.
The use of antacids, which include magnesium or aluminum, 15 minutes before taking the drug, causes a decrease in the bioavailability of fexofenadine.
