Suprastin is an anti-allergic drug, a blocker of histamine H1 receptors.

Release form and composition

  • solution for intravenous (intravenous) and intramuscular (intramuscular) administration: clear, colorless liquid with a weak specific odor (1 ml each in ampoules with a code ring of dark red color and a break point; in a blister strip, 5 ampoules each; in a cardboard box 1 or 2 packages);
  • tablets: disc-shaped, chamfered, with SUPRASTIN engraved on one side, and at risk on the other; color from grayish-white to white, practically odorless (10 pcs. in blisters, in a cardboard box 2 blisters; 20 pcs. in dark glass bottles, in a cardboard box 1 bottle).

1 ampoule with solution contains:

  • active substance: chloropyramine hydrochloride - 20 mg;
  • auxiliary components: water for injection.

1 tablet contains:

  • active substance: chloropyramine hydrochloride - 25 mg;
  • auxiliary components: gelatin, stearic acid, sodium carboxymethyl starch (type A), potato starch, talc, lactose monohydrate.

Pharmacological properties

Pharmacodynamics

Chloropyramine hydrochloride is a chlorinated analogue of tripelenamine (pyribenzamine), it is a classic antihistamine drug from the group of ethylenediamine antihistamines. Has antihistamine and m-anticholinergic effect, has antiemetic, moderate antispasmodic and peripheral anticholinergic effect.

When the drug is taken internally, the therapeutic effect of chloropyramine develops within 15-30 minutes, reaches a maximum within the first hour after administration and lasts at least 3-6 hours.

Pharmacokinetics

  • absorption: chloropyramine hydrochloride after oral administration is almost completely absorbed from the gastrointestinal tract (GIT);
  • distribution and metabolism: the substance is well distributed throughout systems and organs, including the central nervous system (CNS) and is extensively metabolized through the system of liver enzymes;
  • excretion: Suprastin is excreted mainly by the kidneys; in children, the rate of elimination of the drug is higher than in adults.

Indications for use

  • serum sickness;
  • food and drug allergies;
  • hives;
  • conjunctivitis;
  • seasonal and perennial allergic rhinitis;
  • contact dermatitis;
  • eczema in acute and chronic course;
  • allergic reactions to insect bites;
  • itchy skin;
  • atopic dermatitis;
  • Quincke's edema (angioedema) - for solution, tablets in this condition are used only as an adjuvant.

Contraindications

Absolute:

  • acute attacks of bronchial asthma;
  • lactase deficiency, lactose intolerance, glucose-galactose malabsorption syndrome (tablets, since 1 piece contains 116 mg of lactose);
  • the period of pregnancy and lactation (breastfeeding);
  • age up to 3 years (tablets);
  • neonatal period (full-term and premature babies);
  • hypersensitivity to any component of the drug.

Relative contraindications (requiring caution): angle-closure glaucoma, urinary retention and prostatic hyperplasia, impaired liver / kidney function, cardiovascular disease, old age.

Instructions for use of Suprastin: method and dosage

Pills

Suprastin tablets are taken orally at the same time as meals, do not chew and drink plenty of water.

Dosing regimen:

  • children aged 3–6 years: ½ each. (12.5 mg) 2 times a day;
  • children aged 6-14 years: ½ each. (12.5 mg) 2-3 times a day;
  • children over 14 years old and adult patients: 1 pc. (25 mg) 3-4 times a day; the daily dose is 75-100 mg.

In the absence of side effects in the patient, the dose can be gradually increased, but the maximum daily dose should not exceed 2 mg / kg of body weight.

The course of therapy depends on the symptoms of the disease, its duration and clinical course.

Solution for intramuscular and intravenous administration

Suprastin in the form of a solution is administered intravenously and intramuscularly. The drug is injected into a vein only in emergency cases, always under the supervision of a specialist.

Recommended initial doses of Suprastin for children:

  • 1–12 months: ¼ ampoules (0.25 ml) IM;
  • 1–6 years: ½ ampoule (0.5 ml) IM;
  • 6–14 years: ½ – 1 ampoule (0.5–1 ml) i.m.

For adult patients, the drug is recommended to be administered intravenously, in a daily dose of 1–2 ampoules (1–2 ml). In the case of a severe course of allergy, treatment should be started with a careful, slow intravenous injection of the drug, continuing therapy with intramuscular injections or taking pills.

It is allowed to increase the dose under the control of the observed side effects and the patient's response, but not more than up to 2 mg / kg of body weight.

Side effects

Side effects are usually extremely rare and disappear on their own after drug withdrawal:

  • hematopoietic system: very rarely - agranulocytosis, leukopenia, hemolytic anemia, other changes in the cellular composition of the blood;
  • central and peripheral nervous system: fatigue, drowsiness, dizziness, tremor, nervous agitation, euphoria, headache, convulsions, encephalopathy;
  • organ of vision: blurred vision, increased intraocular pressure, glaucoma;
  • cardiovascular system: tachycardia, lowering blood pressure (BP), arrhythmia (direct connection of these side effects with the drug is not always proven);
  • digestive system: dry mouth, abdominal discomfort, nausea / vomiting, constipation, diarrhea, loss or increased appetite, pain in the epigastric region;
  • urinary system: urinary retention, difficulty urinating;
  • musculoskeletal system: muscle weakness;
  • other reactions: photosensitivity, hypersensitivity.

If any of the above effects appear, or other negative reactions occur, the drug should be interrupted and immediately consult a doctor.

Overdose

Symptoms of chloropyramine hydrochloride overdose are: anxiety, impaired coordination of movements, convulsions, ataxia, athetosis, hallucinations. In young children: dry mouth, anxiety, agitation, fixed dilated pupils, sinus tachycardia, facial flushing, fever, urinary retention, coma. In adults: facial flushing and fever occur sporadically, followed by a period of excitement, convulsions, postconvulsive depression, and coma.

To treat the condition (when the drug is taken within a period of no more than 12 hours), gastric lavage is performed, taking into account the anticholinergic effect and antiemetic action of Suprastin. The administration of activated carbon is prescribed, the control of respiration and blood pressure parameters, symptomatic therapy are carried out. If necessary, resuscitation measures are taken. There are currently no data on a specific antidote for chloropyramine.

Taking Suprastin in combination with ototoxic drugs can mask early signs of their negative effect on hearing and the functioning of the vestibular apparatus.

Patients with diseases of the liver and kidneys may need to adjust the dose of the drug downward, therefore, the doctor should be informed about the presence of renal / hepatic insufficiency.

When taking the drug at night, the symptoms of reflux esophagitis may increase.

During therapy with Suprastin, one should refrain from drinking alcoholic beverages, since chloropyramine can enhance the effect of ethanol on the central nervous system.

Long-term use of antihistamines can cause disorders of the hematopoietic system, such as agranulocytosis, hemolytic anemia, leukopenia, thrombocytopenia. In case of an inexplicable increase in the patient's temperature during prolonged use of Suprastin, observation of pallor of the skin, jaundice, laryngitis, ulceration in the mouth, the appearance of hematomas, unusual and prolonged bleeding, it is necessary to conduct an extended clinical blood test to determine the number of formed elements. When the blood count changes, according to the test results, the drug is stopped.

Influence on the ability to drive vehicles and complex mechanisms

Suprastin, especially at the beginning of the course of treatment, can cause increased fatigue, drowsiness, dizziness, and therefore at this time it is prohibited to perform all types of work associated with an increased risk of accidents, including driving vehicles. The duration of the initial period and the degree of restriction on working with complex machines and mechanisms is determined by the doctor for each patient individually.

Application during pregnancy and lactation

Due to the lack of adequate clinical studies of the use of antihistamines during pregnancy, the use of Suprastin by pregnant women, especially in the first trimester and in the last month, is possible only if the potential benefit to the mother exceeds the possible risk to the fetus.

If the drug must be used during lactation, breastfeeding should be suspended.

Pediatric use

The Suprastin solution is contraindicated for use in children (including premature babies) during the neonatal period, from birth to 28 days.

The drug in the form of tablets is prohibited from giving Suprastin to children under the age of 3 years.

With impaired renal function

The drug is used with caution in case of impaired renal function. It may be necessary to change the regimen and reduce the dose, since chloropyramine is excreted mainly by the kidneys.

For violations of liver function

According to the instructions, Suprastin is used with caution in case of liver dysfunction. A dose reduction may be required due to a decrease in its metabolism in liver disease.

  • barbiturates, M-anticholinergics, opioid analgesics: chloropyramine enhances their effect;
  • monoamine oxidase (MAO) inhibitors: can increase and prolong the anticholinergic effect of chloropyramine;
  • ototoxic drugs: Chloropyramine may mask signs of ototoxicity.
