Clarisens is an anti-allergic drug with anti-exudative, antihistamine and antipruritic action.
Release form and composition
Dosage forms of Clarisens:
- tablets: flat-cylindrical, from white to almost white (in a blister contour packaging 7, 10 or 20 pieces, in a cardboard box 1, 2, 3 or 5 packages);
- syrup: slightly yellowish or colorless liquid, with a characteristic fruity aroma, with an acceptable presence of bubbles (in a dark glass bottle of 50, 100 or 125 ml, in a cardboard box 1 bottle complete with a measuring spoon of 2.5 and 5 ml).
Composition of 1 tablet:
- active substance: loratadine - 10 mg;
- auxiliary components: milk sugar (lactose monohydrate), potato starch, talc, magnesium stearate.
Composition of 100 ml syrup:
- active substance: loratadine - 100 mg;
- auxiliary components: succinic acid, citric acid (citric acid monohydrate), sorbitol (sorbitol), propylene glycol, glycerin (glycerol), methyl parahydroxybenzoate (methylparaben, nipagin), propyl parahydroxybenzoate (propylparaben, nipazole), purified aroma aspartame, purified aspartame
Pharmacological properties
Pharmacodynamics
Loratadine, the active component of Clarisens, is a long-term selective blocker of peripheral H1-histamine receptors of the second generation. By suppressing the release of histamine (a mediator of allergic reactions) and leukotriene C4 from mast cells and reducing the migration of eosinophils, loratadine prevents the development of the histamine-dependent stage of allergic reactions.
The drug has an anti-exudative and antipruritic effect, facilitating the course of allergic reactions. It has practically no anticholinergic and antiserotonin effects. Loratadine helps to reduce capillary permeability, relieves spasms of smooth muscles, and prevents the development of tissue edema.
Antiallergic effect is observed within 30 minutes after application and lasts 24 hours. The maximum effect is achieved after 8-12 hours after taking Clarisens. The drug does not have a sedative effect on the central nervous system, does not cause addiction, since it practically does not penetrate the blood-brain barrier.
Pharmacokinetics
- absorption: in the gastrointestinal tract is absorbed quickly and almost completely. The maximum concentration (Cmax) is reached from 1.3 to 2.5 hours after ingestion. The time to reach Cmax is lengthened by about 1 hour with simultaneous food intake;
- distribution: about 97% of the dose binds to blood plasma proteins. The equilibrium concentration (Css) of the active substance and its metabolite is observed on the 5th day of loratadine use. Loratadine does not have the ability to penetrate the blood-brain barrier;
- metabolism: metabolized in the liver by the action of the enzymes CYP3A4 and CYP2D6 (to a lesser extent) with the formation of an active metabolite of descarboethoxyloratadine;
- elimination: the half-life (T1 / 2) of loratadine is in the range from 3 to 20 hours (average value is 8.4 hours), its active metabolite is from 8.8 to 92 hours (average value is 28 hours). The substance is excreted by the kidneys and through the intestines.
In elderly patients, the Cmax of loratadine increases by 50%, T1 / 2 varies from 6.7 to 37 hours (18.2 hours on average), the active metabolite - from 11 to 38 hours (17.5 hours is the average).
With chronic impairment of renal function and hemodialysis, pharmacokinetic parameters practically do not change.
In patients suffering from alcoholic liver damage, Cmax and T1 / 2 of loratadine increase according to the severity of the disease.
Indications for use
- perennial and seasonal allergic rhinitis (runny nose) and conjunctivitis;
- urticaria, including chronic idiopathic;
- angioedema (Quincke);
- allergic dermatoses with pruritus (including chronic eczema, contact dermatitis);
- allergic reactions to insect bites.
Contraindications
Absolute:
- phenylketonuria (for syrup);
- pregnancy and lactation period;
- children under 2 years old;
- increased individual sensitivity to any component of the drug.
Clarisens should be taken with caution in patients with impaired liver function.
Instructions for use of Clarisens: method and dosage
Clarisens tablets or syrup are taken orally with a small amount of water, regardless of the meal.
Recommended dosage regimens:
- adults and children over 12 years old: 10 mg (1 tablet or 10 ml of syrup) 1 time per day;
- children from 2 to 12 years old with body weight less than 30 kg: 5 mg (1/2 tablet or 5 ml of syrup) 1 time per day;
- children from 2 to 12 years old weighing more than 30 kg: 10 mg (1 tablet or 10 ml of syrup) once a day.
Side effects
Possible side reactions from systems and organs:
- central nervous system: in adults - drowsiness, fatigue, headache; in children - nervousness, headache, sedation;
- digestive system: in adults - nausea, dry mouth, gastritis, liver dysfunction;
- cardiovascular system: in adults - tachycardia, palpitations;
- allergic reactions: in adults - skin rash, anaphylactic shock;
- organism as a whole: in adults - alopecia.
The listed side effects were observed with a frequency of 2% or more - approximately with the same frequency as after taking a placebo.
Overdose
In case of an overdose of Clarisens, symptoms such as headache, tachycardia, and drowsiness may occur.
Treatment in such cases consists in induction of vomiting, gastric lavage, intake of activated carbon, and symptomatic therapy.

Patients with impaired liver function are advised to take Clarisens every other day.
Influence on the ability to drive vehicles and complex mechanisms
Since loratadine can cause such adverse reactions as drowsiness and fatigue, while taking the drug, care should be taken when driving vehicles and performing potentially hazardous activities that require increased concentration and a quick psychomotor reaction.
Application during pregnancy and lactation
According to the instructions, Clarisens is contraindicated in pregnant and breastfeeding women.
Pediatric use
Prescribing Clarisens to children under 2 years of age is prohibited.
For violations of liver function
Loratadine should be used with caution in patients with impaired liver function. Such patients are recommended to take Clarisens every other day.
Drug interactions
The use of loratadine concurrently with certain drugs can lead to the development of the following effects:
- ethyl alcohol: decreased effectiveness of loratadine;
- ketoconazole, cimetidine, erythromycin: an increase in the concentration of loratadine in the blood plasma (without clinical manifestations and without affecting the electrocardiogram);
- barbiturates, phenytoin, tricyclic antidepressants, phenylbutazone, rifampicin, zixorin and other inducers of microsomal oxidation: decrease in the effectiveness of loratadine.
